    Job Description Responsible for planning, coordinating, and managing clinical and administrative volunteer program activities in the organization. Additionally, the Volunteer Coordinator will support community outreach efforts by participating in community resource events. This is a full time position requiring travel accross Heartlinks' service area (Benton and Yakima Counties). Essential Job Functions/Responsibilities Develop and maintain volunteer training and orientation program. Recruits, educates, and selects volunteers from at least three recruitment sessions a year. Establish a need for volunteer services with the patient and family/caregiver and entire hospice team. Supervise and evaluate volunteers in a timely and appropriate manner. Maintains monthly statistics. Maintains current personnel files. Reevaluate volunteer program as necessary. Coordinate support/education meetings. Attends interdisciplinary group meetings. Provides information to individuals and community groups about hospice volunteer services. Participates as needed in the budget preparation for program development. Actively participates in quality assessment performance improvement teams and activities. Assist in community outreach efforts as needed, including traveling to various resource fairs and events. Accepts and performs other related duties and responsibilities. Position Qualifications High school education required. Minimum of two years experience in a health care setting. Hospice and/or volunteer experience preferred. Ability to supervise, coordinate, and evaluate volunteer services. Understands hospice philosophy. Ability to organize and develop volunteer hospice personnel for both patient and organizational needs. Ability to manage a group of individuals providing volunteer time and who will be considered employees. Benefits: Full-time employees accrue up to 168 hours of PTO annually, in addition to sick time accrued at the rate of 1 hour for every 40 hours worked. This role serves as a liaison between departments, patients, families, facilities, vendors, staff, and community partners by providing education, outreach, follow-up after discharge, volunteer support, and onboarding education to promote understanding of hospice philosophy and smooth transitions across programs and care settings. Essential Job Functions / Responsibilities Transitions of Care & Interdisciplinary Support (40% - 20% Hospice / 10% Palliative Care / 5% Health Homes / 5% Adult Family Home) Assists with intakes, informational visits, and pre-admission education as assigned, providing non-clinical support and coordination with admissions and clinical staff. Provides education and support to patients transitioning between organizational programs (e.g., palliative care, hospice, health homes, pediatric services, bereavement, community services). Serves as a resource for patients and families during transitions to reduce gaps in services and improve continuity of care. Collaborates with clinical staff, admissions, social services, volunteer services, and leadership to support smooth transitions and consistent messaging. Monitors and follows up with patients and families after discharge from hospice or palliative services to ensure understanding of ongoing plan of care, available resources, and next steps, and coordination of care. Communicates transition-related concerns to appropriate clinical and administrative leadership. Participates in Quality Assessment and Performance Improvement (QAPI) activities related to transitional care monitoring, outreach and education. Maintains strict confidentiality in compliance with HIPAA, Medicare CoPs, and Washington State privacy regulations. Performs other duties as assigned to support organizational mission and regulatory compliance. Education and Outreach (45% - 40% Hospice / 5% Palliative Care) Community Services (20% of Hospice / 5% Palliative Care) Provides ongoing education and support to partnered facilities, referral sources, and vendors regarding hospice philosophy, eligibility, services, and organizational processes. Maintains positive working relationships with community partners to support patient access and continuity of care. Participates in outreach activities, marketing initiatives, in-services, and community education events. Promotes, strengthens, and expands bereavement and community support services through outreach and community engagement efforts. Implements strategies to increase the reach and utilization of community support programs throughout the service area. Staff, Volunteer & Organizational Education (20%) Provides high level education to staff and volunteers related to all areas of hospice philosophy, communication, boundaries, grief awareness, transitions of care, community resources, stress reduction, and best practices, etc. Assists with the supervision and field support of hospice volunteers, including assisting with educational sessions. Provides hospice philosophy education to all new hires, including orientation support and reinforcement of mission, values, and scope of services. Assists with the onboarding of non-clinical staff, including training coordination, education tracking, and initial role support. Provide collaboration and consultation with members of the MSW team for high acuity or difficult patient cases. Participate and manage ongoing support for organizational committees including Social Justice and Equity Committee and the Outreach Committee. Monitor and maintain all records of internal and external education including participants, hours, topics, etc. Caseload Management (15% - 10 % Hospice / 5% Palliative Care) Expected to manage a dynamic caseload of 10-15 hospice and palliative care patients based on other duties including: transition follow-ups, outreach accounts, facilities or volunteer education, onboarding schedules and additional responsibilities across multiple programs. Assist with PRN visits or admissions as needed based on community and organizational need. Position Qualifications: Bachelor's degree in Social Work, Human Services, Healthcare Administration, Education, or related field required; Master's degree preferred. Minimum of one (1) year of experience in hospice, palliative care, healthcare, outreach, case management, or community education preferred. Knowledge of hospice philosophy, transitions of care, community resources, and end-of-life services strongly preferred. Strong verbal, written, presentation, and organizational skills. Ability to work independently, manage multiple priorities, and collaborate effectively with interdisciplinary teams. Comfort with community education, public speaking, and relationship-building. Current CPR certification required per organizational policy. Must possess a valid Washington State driver's license, reliable insured vehicle, and ability to travel within the service area. The MSW is responsible for assessing psychosocial needs, implementing the plan of care, supporting patients and families through the end-of-life process, and collaborating with the interdisciplinary group (IDG) to ensure patient-centered, compliant, and compassionate care. Essential Job Functions/Responsibilities: Conducts initial and ongoing comprehensive psychosocial assessments of patients and families/caregivers within required timeframes per Medicare CoPs and agency policy. Assesses emotional status, social support, financial concerns, living environment, cultural and spiritual needs, advance care planning, safety risks, and caregiving capacity. Communicates assessment findings to the Registered Nurse and other IDG members in a timely manner. Develops, implements, evaluates, and updates the individualized Plan of Care in collaboration with the IDG. Provides counseling, crisis intervention, and emotional support related to anticipatory grief, terminal illness, loss, family dynamics, and end-of-life decision-making. Educates patients and families on available community resources, benefits, legal planning, funeral planning, and advance directives. Serves as a liaison between patients/families, hospice staff, physicians, facilities, and community agencies to support continuity of care. Maintains accurate, complete, timely, and compliant clinical documentation in accordance with Medicare CoPs, Washington State regulations, and agency policy. Participates in Interdisciplinary Group (IDG) meetings at least every 15 days, contributing psychosocial updates and recommendations as required by CoPs. Participates in Quality Assessment and Performance Improvement (QAPI) activities. Assists the physician and clinical team in understanding significant social, emotional, environmental, and family factors impacting the patient's care and disease progression. Identifies risk factors including abuse, neglect, exploitation, housing insecurity, caregiver burnout, and safety concerns, and reports per Washington State mandatory reporting laws. Maintains and develops professional relationships with public and private community agencies to support patient and staff needs. Provides bereavement support and appropriate referrals in coordination with the bereavement program, as assigned. Maintains patient confidentiality in compliance with HIPAA, Medicare CoPs, and Washington State privacy laws. Completes required training, in-services, competencies, and continuing education as required by regulation and agency policy. Caseload Expectations: Maintains an average caseload of approximately 30 hospice patients, with a maximum caseload of 35 patients based on patient acuity, geographic coverage, and organizational need.     Responsible for planning, coordinating, and managing clinical and administrative volunteer program activities in the organization. Additionally, the Volunteer Coordinator will support community outreach efforts by participating in community resource events. This is a full time position requiring travel accross Heartlinks' service area (Benton and Yakima Counties). Essential Job Functions/Responsibilities Develop and maintain volunteer training and orientation program. Recruits, educates, and selects volunteers from at least three recruitment sessions a year. Establish a need for volunteer services with the patient and family/caregiver and entire hospice team. Supervise and evaluate volunteers in a timely and appropriate manner. Maintains monthly statistics. Maintains current personnel files. Reevaluate volunteer program as necessary. Coordinate support/education meetings. Attends interdisciplinary group meetings. Provides information to individuals and community groups about hospice volunteer services. Participates as needed in the budget preparation for program development. Actively participates in quality assessment performance improvement teams and activities. Assist in community outreach efforts as needed, including traveling to various resource fairs and events. Accepts and performs other related duties and responsibilities. Position Qualifications High school education . Minimum of two years experience in a health care setting. Hospice and/or volunteer experience preferred. Ability to supervise, coordinate, and evaluate volunteer services. Understands hospice philosophy. Ability to organize and develop volunteer hospice personnel for both patient and organizational needs. Ability to manage a group of individuals providing volunteer time and who will be considered employees. Benefits: Full-time employees accrue up to 168 hours of PTO annually, in addition to sick time accrued at the rate of 1 hour for every 40 hours worked. Bereavement and jury duty leave are also available. Option to enroll in medical, dental, vision, and life insurance. Heartlinks pays for 100% of the employee's medical premiums on the base plan, and a portion of the employee's dental and vision premiums. Health benefits begin the first of the month after the start date. Miles driven for work are reimbursed at 62.5 cents per mile. Holiday pay for 6 observed holidays. Ability to enroll in 401(k) and begin contributing at the time of hire. Heartlinks will match dollar for dollar up to 5% of earnings after one year of employment and 1080 hours worked. Provides a complete physical assessment and history of current and previous illness(es). Provides professional nursing care by utilizing all elements of the nursing process. Assesses and evaluates patient's status by: Writing and initiating a plan of care Regularly re-evaluating patient and family/caregiver needs Participating in revising the plan of care as necessary Uses health assessment data to determine nursing diagnosis Develops a care plan that establishes goals based on nursing diagnosis and incorporates palliative nursing actions. Includes the patient and the family in the planning process. Initiates appropriate preventive and rehabilitative nursing procedures. Administers medications and treatments as prescribed by the physician in the physician's plan of care. Counsels the patient and family to meet nursing and related needs. Provides health care instructions to the patient as appropriate per assessment and plan. Position Qualifications: Registered nurse with current licensure to practice professional nursing in the state of Washington. Graduate of an accredited school of nursing. Maintains a current BLS certification. Must be a licensed driver with an automobile that is insured in accordance with state or organization requirements and is in good working order.
