SUMMARYWe are currently seeking a CDI Quality Coordinator to join our team. This full-time role will primarily work remotly (Day, M- F). Purpose of this position: Chiefly responsible for the reviewing of PSI/HAC reported data. Assists in the daily workload for clinical documentation specialists. And assists in the continuing education of CDI staff and providers (attending providers, PAs and APPs) and assists the Physician Liaisons as necessary. Position will require considerable knowledge, experience, judgment and ability to resolve problem related to ICD documentation requirements, overall documentation requirements within HCMC. RESPONSIBILITIESConduct coding consults and PSI/HAC reviews on daily basis Validate reported PSI codes against AHRQ technical specification and CMS reported HAC's with assistance of the Medical Director of ClinicalDocumentation Improvement and Physician LiaisonsMaintain a copy of updated AHRQ PSI technical specifications from AHRQ website. And updates reportable HAC's from CMSReview and compare quarterly or monthly reported PSI/HAC cases from Quality Department and SASreports for accuracy Input data for each PSI/HAC Excel spreadsheets and Epic as directed. Enter events into MCCM and event system as needed for reporting purposes Report monthly PSI/HAC data to CDI and Inpatient Coding Manager, CDI Supervisor, Medical Director of Clinical Documentation Improvement and other departments and leadership as needed Participate and represent Clinical Documentation in meetings Identify documentation trends around PSI/HACs and assist in creating education for coding and clinical documentation staff Assist the Physician Liaisons with education for providers (Attending Physician's, PAs and APPs) Participate in the final decision and recommendation regarding issues regarding PSI and HAC with Medical Director of Clinical Documentation Improvement, CDI and Inpatient coding SupervisorsRun reports as requested after appropriate training Assist the CDI Supervisor with daily communication and workload for clinical documentation specialists Assist Clinical Documentation specialists with follow- up on query responses. Assist in training and education of new clinical documentation specialists Be a resource for clinical documentation specialist regarding documentation and coding issues. SUMMARY: We are currently seeking a Teen and Family Case Management Assistant to join our Aqui Para Ti team. This part-time role will work 40 hours within a two week pay period on days. Hours fall between 8 AM and 5 PM but primarily will work afternoon hours from 12 PM to 5 PM. Candidates must be bilingual in Spanish. Purpose of this position: Under general supervision, provides service coordination to youth in specific communities between 10-24 and their families for various psychosocial issues, advocacy, and care. Delivers information, referrals, coordinates, negotiates, and procures services and resources for clients. Assists in the care coordination of complex patients to facilitate the delivery of cost-effective health care and identify the appropriate utilization of resources. Coaches patients and parents/caregivers in developmentally appropriate positive parenting style. Works collaboratively with interdisciplinary staff internal and external to the organization. Participates in quality improvement and evaluation processes related to the management of patient care. RESPONSIBILITIES: Explain policies about teen confidential services to patients and parents during the first visit Administer health and parenting screening questionnaires to parents of new patients and then every year after their first visit Educate patients and parents about the meaning of mental health illness, treatment and resources, and how to navigate school and health system Provide coaching to patients and parents and their identified supports on health issues and client identified goals Provide education to parents or caregivers about mental health conditions, prevention, and resources. Refer clients to appropriate health resources and social services and support clients in accessing resources, particularly in regards to parents/caregivers Prepare and facilitate Aqui Para Ti (APT) multidisciplinary team clinic huddle; monitor and review clients' needs, risks, and progress and develop/revise preliminary action plans accordingly during APT team case/care management sessions and teen clinic visits Actively coordinate services and follow-up with high-risk clients and parents, and work collaboratively with professionals involved in the care of the client Perform face-to-face, video, and phone contact with a specified number of patients on a scheduled basis and as needed Develop and nurture relationships with schools, community and social support providers to aid in effective referrals and timely access to services Create and maintain the connection with schools to support patient's educational goals. Candidates ideally will integrate in the division focusing predominantly on chest imaging, including chest X-rays, oncologic chest CT studies, lung cancer screening studies. Interstitial lung disease cases are also available to candidates who have interest. The Senior Data Integrity Analyst must apply this knowledge when evaluating and working multiple inbaskets and interface error queues requiring deductive reasoning for the purpose of ensuring accurate documentation in the patient's EHR. This role requires excellent written and verbal communication skills. The position also requires working independently with little to no supervision.
