    We are currently seeking a Teen and Family Case Management Assistant to join our Aqui Para Ti team. This part-time role will work 40 hours within a two week pay period on days. Hours fall between 8 AM and 5 PM but primarily will work afternoon hours from 12 PM to 5 PM. Candidates must be bilingual in Spanish. Purpose of this position: Under general supervision, provides service coordination to youth in specific communities between 10-24 and their families for various psychosocial issues, advocacy, and care. Delivers information, referrals, coordinates, negotiates, and procures services and resources for clients. Assists in the care coordination of complex patients to facilitate the delivery of cost-effective health care and identify the appropriate utilization of resources. Coaches patients and parents/caregivers in developmentally appropriate positive parenting style. Works collaboratively with interdisciplinary staff internal and external to the organization. Participates in quality improvement and evaluation processes related to the management of patient care. RESPONSIBILITIES: * Explain policies about teen confidential services to patients and parents during the first visit * Administer health and parenting screening questionnaires to parents of new patients and then every year after their first visit * Educate patients and parents about the meaning of mental health illness, treatment and resources, and how to navigate school and health system * Provide coaching to patients and parents and their identified supports on health issues and client identified goals * Provide education to parents or caregivers about mental health conditions, prevention, and resources. Refer clients to appropriate health resources and social services and support clients in accessing resources, particularly in regards to parents/caregivers * Prepare and facilitate Aqui Para Ti (APT) multidisciplinary team clinic huddle; monitor and review clients' needs, risks, and progress and develop/revise preliminary action plans accordingly during APT team case/care management sessions and teen clinic visits * Actively coordinate services and follow-up with high-risk clients and parents, and work collaboratively with professionals involved in the care of the client * Perform face-to-face, video, and phone contact with a specified number of patients on a scheduled basis and as needed * Develop and nurture relationships with schools, community and social support providers to aid in effective referrals and timely access to services * Create and maintain the connection with schools to support patient's educational goals. Support the patient and families with the process of obtain a 504 and/or Individualized Educational Plans * Collect and track data for evaluation purposes * Participate in scholarly activities, presentations, train the trainer opportunities as fits APT's team needs in its National and State leadership role * Other duties as assigned, but only when trained and qualified to do so QUALIFICATIONS: Minimum Qualifications: * College degree in the social sciences or a related field * Two (2) year's experience working with specific communities, depending on assignment * May require bilingual Spanish/English depending on assignment * OR- * An approved equivalent combination of education and experience Preferred Qualifications: * Healthcare professional with Bachelor's degree in the social sciences Knowledge/ Skills/ Abilities: * Skill in youth/family engagement and referral * Basic computer skills * Knowledge of diversity issues; grief and loss; social, economic, legal and medical systems; safety; and behavioral and psychosocial implications of acculturation * Ability to work within medical hierarchy * Ability to keep accurate and timely records * Ability to assist in complex problem-solving with clients * Ability to work autonomously, network with related agencies, build coalitions, and establish boundaries with clients, team members, and agencies * Ability to be flexible and adapt to changing priorities * Must demonstrate effective written and oral communication * Ability to deal with death and difficult issues sensitively License/Certifications: For employees working in a Behavioral Health Home certified clinic, employees must be certified as one of the following: * Community health worker * Peer support specialist * Family peer support specialist * Case management associate * Mental health rehabilitation worker * Community paramedic * Certified health education specialist * OR- * Meet the requirements of a case manager

Acceptable concentrations would include but not be limited to theology, study of sacred texts, medical or professional ethics, psychology, sociology, counseling, world religions, organizational development, and the relationship of spirituality and health * At least 800 hours of clinical training in spiritual/pastoral care such as Clinical Pastoral Education * Experience working with individuals from all religious backgrounds. * Experience working with a diverse population. Candidates ideally will integrate in the division focusing predominantly on chest imaging, including chest X-rays, oncologic chest CT studies, lung cancer screening studies. Interstitial lung disease cases are also available to candidates who have interest. Candidates may choose to focus predominantly on thoracic imaging or have a mixed practice with abdominal or multispecialty imaging. Our department is in a growth phase currently providing on-site and remote interpretive services for 8 hospitals and multiple outpatient imaging centers that compose the radiology practices of Emory Healthcare and the Grady Health System.The Division is strongly oriented toward innovation with several Artificial Intelligence tools available to support the daily practice and operation. In collaboration with successful candidates, physician schedules are carefully composed to optimize work-life balance and prioritize long-term personal and division success. We have several template schedules within the department including a 26 weeks on 26 weeks off schedule consisting of 8am-5pm weekday shifts and 7am-4pm shifts on the weekends/holidays. This equates to 182 shifts of clinical duty per year. We have additional schedules which can involve evening work and multispecialty imaging if this is an interest for the candidate. A full-time hospitalist will provide 24 weeks of inpatient service per year, providing direct patient care (including admissions) as well as supervising and teaching advanced practice providers and trainees (fellows and residents). Part-time positions are available for highly qualified candidates. Qualified candidates will have expertise in inpatient management of acute illnesses and complications/exacerbations of chronic illnesses, preferably with prior experience as an academic hospitalist. Ideal candidates will have additional training/experience in Palliative Medicine or Medical Oncology. Candidates without these preferred qualifications should have a passion for developing the additional competencies required to provide excellent care to patients with cancer through on-the-job training and learning opportunities. Hospitalists will support clinical trials where applicable in the inpatient setting, while collaborating with other clinicians in the Department of Hematology and Medical Oncology, Winship Cancer Institute, and others across Emory University.
