We are currently seeking a Pharmacist Intern to join our Community Pharmacy team. This position is casual, no benefits, day/evening shifts and one shift every other weekend, 4-8 hour shifts. We will be interested in interviewing UMN pharmacy school students who are currently enrolled in their 1st year of pharmacy school (i.e. anticipated graduation year 2029).
Required work schedule include:
* One 4-hour weeknight shift per week
* One weekend shift every-other week
Either a 4- or 8-hours shift depending on the day and location
* Summer Schedule:
Up to 40-hour work weeks between our Community Pharmacy locations
Purpose of this position: Responsible for acting as an information resource to patients, physicians, nurses, and other healthcare professionals regarding medication therapy and drug-related information and literature. Maintains
standards set by the department and translates policies and procedures into the daily operation of the
pharmacy. Performs within scope of practice as defined by the Minnesota Board of Pharmacy.
RESPONSIBILITIES:

Community Pharmacy
* Provides continuous, patient specific assessment of the pharmacotherapy needs of a patient population, making real time interventions with the medical staff and nursing staff to facilitate optimal drug therapy and facilitate the provision of other pharmacy services
* Responsible for interpretation of physician medication orders for accuracy and appropriateness, informing them of any errors, omissions, or discrepancies and, based on an evaluation of the patient's condition, suggests appropriate or alternate drug therapy to the physician
* Responsible for maintaining accurate and proper patient medication profiles, monitoring to insure the proper choice of drug, dosage form, strength, length of therapy and interaction potential of the prescribed drug
* Responsible for the dispensing, control and record keeping of all controlled substances
* Responsible for maintaining a working knowledge of, and conforming to legal requirements concerning the distribution of drugs, as well as good working knowledge of recent developments in the field of pharmacy
Inpatient Pharmacy
Pre-pharmacy student:
* Prepares and dispenses medications including, but not limited to:
* Oral Medications
* Injectable Medications
* IV Medications
* Parenteral Nutrition
* Maintains records of all narcotics received and dispensed
* Supports operational needs, including purchasing and inventory management.
* Routinely checks expiration dates of medications in the pharmacy department and on the nursing units. Rotates and recycles stock when needed
* Promotes effective working relations and works effectively as part of a department/unit team and interdepartmentally to facilitate the department's/unit's ability to meet its goals and objectives
* Participates in special projects as assigned by supervisor
First Year Pharmacy Student:
* Performs all the functions of a Pre-Pharmacy Student
* Compounds non-sterile products based on assessment of drug utilization
Second - Fourth Year Pharmacy Students (Interns):
* Performs all the functions of a First Year Pharmacy Student
* Performs pharmacist-level product checks for accuracy and validity of the following:

Poison Center Pharmacy
Pre-pharmacy student:
* Responds to telephone calls from the public regarding nontoxic or minor poison exposures involving non-medication substances such as plants and household products, determines the type and severity of each exposure and recommends initial first aid and treatment advice per poison center guidelines
* Responds to telephone calls requesting tablet identification, basic poison information and poison prevention questions
* Performs follow-up calls on cases within scope
* Accurately records medical case information utilizing computerized data collection software
* Provides public poison prevention education
* Participates in case review and quality assurance activities
First Year Pharmacy Student:
* Performs all of the functions of a Pre-Pharmacy Student
* Also responds to calls from the public regarding nontoxic or minor exposures to over-the-counter medications
Second - Fourth Year Pharmacy Students (Interns)
* Performs all the functions of a First Year Pharmacy Student

Job Description:
Provides optimal pharmaceutical care for patients and oversees medication management through the compounding and dispensing of prescribed medications, evaluation of patient medication therapies and outcomes, the performance of medication history evaluations, and providing consultation and education to patients about medications and their use. Provides pharmaceutical care for Home Infusion patients. Practices in accordance with all laws and regulations, following legal standards in all areas including, work rules, controlled substance requirements, state and federal regulations, and HIPAA regulations. May advise physicians and other health practitioners on the selection, dosage, interactions, and side effects of medications.
Principle Responsibilities
Reviews medications and patient medication profiles for possible interactions.
Evaluates medication orders for appropriateness, including dosage, route, frequency, effectiveness, and cost. Checks for medication over/under utilization.
Conducts medication history reviews, monitoring patient medicine profiles to ensure appropriateness based on factors such as age, indication for use, elimination parameters and other clinically relevant lab results.
Adheres to formulary management principles, published guidelines, and approved protocols.
In collaboration with other health care professionals, may manage the home infusion patient care plan.
Dispenses medications.
Enters, verifies, compounds and dispenses medications and IV's.
Verifies accuracy of product preparation, compounding, and labeling to comply with current standards, laws and hospital policy.
Evaluates compatibilities between medications, IV solutions, and parenteral nutrition.
May program and maintain pump technology and medication delivery devices.
May oversee provision of and compatibility of supplies and equipment.
Provides counseling and consultation.
Counsels patients on new medication, refills and over-the-counter products as needed.
Effectively communicates with patients and staff member based on their specific needs.
Consults with physicians and other health care providers to assist patient with medications. Provides patient-specific consultative services or medication-related information to members of the interdisciplinary care team.
Provides pharmacotherapeutic recommendations to optimize therapy and maximize patient care and cost efficiencies.
Documents professional activities, related medication consults, and other patient care activities in the medical record or other pharmacy reporting databases.
May work collaboratively with Allina Home Care partners, liaisons, nurse care coordinators, and social workers to help facilitate new referrals and admissions.
Provides medication education and patient care service.
Provides medication education to patients to improve adherence and support positive outcomes.

Makes recommendations to optimize medication therapy.
Holds self and team members accountable to provide exemplary service to patients, caregivers, nursing and other professional staff. Provides work direction for Pharmacy team, evaluating and prioritizing workload to ensure processes are performed efficiently. Delegates tasks to technicians and interns, as appropriate.
Assists in orientation and training for pharmacy staff.
May precept pharmacy students, interns and residents.
If applicable by site, promptly responds to and participates in emergent medical situations (e.g. cardiac arrest, stroke, other urgent medical conditions).
Coordinates with third party vendors, delivery couriers, and all partners that interface with home infusion business.

Required Qualifications
Bachelor's degree in Pharmacy OR PharmD from an accredited school of pharmacy
0 to 2 years of identified pharmacy experience
Preferred Qualifications
Completion of an accredited residency program or equivalent Pharmacy experience
Completion of pharmacist board certification
2 to 5 years of hospital pharmacy experience or
2 to 5 years home infusion pharmacy experience including therapeutic drug monitoring and IV admixture, and medication compounding

SUMMARY:We are currently seeking a Pharmacy Tech to join our Enhanced Care Pharmacy team.
This position is part-time with benefits, 40 hours per pay period, day shift and every 3rd weekend.
This is pharmacy technician position for the Enhanced Care Pharmacy located in the Blue Building of the Hennepin Healthcare campus.
This pharmacy exclusively services patients who receive care at the Positive Care Clinic.
The pharmacy is open from 8 am to 5 pm every weekday and has been recently renovated to include a filling robot and additional space for patient care and prescription processing.
The position does include a every 3rd weekend component (Every 3rd Saturday and every 6th Sunday) in our discharge pharmacy.
