Within the ambulatory division, practitioners will rotate amongst the 6 system outpatient sites (Royal Oak/Plymouth/Fairlane/Lakeside/Columbus/Cottage). Cases start at 7am at most sites with intended off the table time of 3pm. Providers stay until the last patient is discharged from PACU. Some sites will require 2 providers depending on the number of rooms/blend of cases that day. Case types are representative of all surgical subspecialities with some sites "specializing" in a more limited number of service lines. Having a background in regional anesthesia is must (neuraxial anesthesia as well as "bread and butter" blocks). Anesthesiologists will also complete online pre-operative evaluations in order to determine patient ASC candidacy and or optimization. Residents rotate commonly at one of the locations and teaching is a component of those days
I. General Summary: Anesthesiologists' responsibilities to patients include:
A. Assessment of consultation for and preparation of patients for anesthesia.
B. Medical management of patients and the anesthetic for the planned procedures.
C. Postanesthetic evaluation and treatment
D. On-site medical direction of any nonphysician who participates in the delivery of anesthesia care to the patient
E. Perioperative pain management
F. The same standards for and quality of anesthetic care should be available for all patients, emergency as well as elective patients.
II. Preanesthesia Care Responsibilities
A. Review the medical records
B. Interview the patient, parents or guardians of a minor, next of kin if patient is unable to communicate, or reviews the available medical information if no information can be supplied by any of the above to:
1. Discuss the medical history, including anesthetic experiences and drug therapy.
2. Perform any examinations that would provide information that might assist in decisions regarding anesthetic risk and management.
C. Order tests and medications necessary to the conduct of anesthesia
D. Obtain consultations as necessary
E. Record an assessment and an anesthetic plan on the patient's chart
F. Ensure consent has been obtained
III. Perianesthetic Care Responsibilities
A. Re-evaluation of the patient immediately prior to induction
B. Prepare and check of equipment, drugs, fluids and gas supplies
C. Appropriate monitoring of the patient
D. Selection and administration of anesthetic agents or render the patient insensible to pain, while providing a level of comfort and relaxation commensurate with the invasiveness and physiologic stress of the planned procedure.
E. Support of life functions under the stress of anesthetic, surgical and obstetrical
manipulations.
F. Record the pertinent events of the procedure.
IV. Postanesthetic Care Responsibilities
A. Give to the health care receiver transfer of care information pertinent to the patient's specific needs and ensure a safe transition.
B. Remain with the patient as long as medically necessary and until the receiving health care provider has all the information needed to assume care.
C. Assure the patient is discharged from the postanesthesia care unit in accordance with policies established by the Department of Anesthesiology.
D. The duration of surveillance in the postanesthesia care unit is determined by the status of the patient and the judgment of the anesthesiologist.
V. Anesthesia Care Team (ACT)
A. Anesthesiologists supervising resident physicians in training and/or directing qualified non-physician anesthesia providers in the provision of anesthesia care wherein the physician may delegate monitoring and appropriate tasks while retaining overall responsibility for the patient.
B. Preanesthetic evaluation of the patient - Non-physicians may contribute to the preoperative collection and documentation of patient data, but the anesthesiologist is responsible for the overall evaluation of each patient.
C. Prescribing the anesthetic plan - The anesthesiologist is responsible for prescribing an anesthesia plan aimed at the greatest safety and highest quality for each patient. The anesthesiologist discusses with the patient (when appropriate), the anesthetic risks, benefits and alternatives and obtains informed consent. When a portion of the anesthetic care will be performed by another qualified anesthesia provider, the anesthesiologist should inform the patient that delegation of anesthetic duties is included in care provided by the Anesthesia Care Team (ACT). After discussion and determination of the anesthetic plan, both anesthesia providers should sign the Pre-Anesthesia Evaluation form.
D. Management of the anesthetic - The anesthesiologist may delegate specific tasks to qualified non-anesthesiologist members of the ACT providing that quality of care and patient safety are not compromised, but should participate in critical parts of the anesthetic and remain immediately physically available for management of emergencies regardless of the type of anesthetic.
E. Postanesthesia care- Patients transported to the PACU shall be accompanied by a member of the anesthesia care team who is knowledgeable about the patient's condition. The patient shall be continually evaluated and treated during transport with monitoring and support appropriate to the patient's condition. Upon arrival in the PACU, the patient shall e re-evaluated and a verbal report provided to the responsible PACU nurse by the member of the anesthesia care team who accompanies the patient. The evaluation and treatment of postanesthetic complications are the responsibility of the anesthesiologist. The anesthesiologist is responsible for the discharge of the patients from the PACU, either by the criteria or signature.
F Anesthesia consultation - Like other forms of medical consultation, this is the practice of medicine and should not be delegated to nonphysicians.
VI. Quality Assurance
The anesthesiologist will participate in a planned program for evaluation of quality and appropriateness of the anesthetic care of patients and will participate in resolving identified problems.
VII. Other responsibilities included, but are not limited to:
1. Supervision, teaching and evaluation of performance of personnel, both medical and paramedical, involved in perioperative care.
2. Provides support during emergency procedures (e.g. cardiac arrest).
3. Participates in the education of anesthesia residents including but not limited to: clinical supervision during administration of anesthesia and conducting education sessions.
4. Participates in administrative support of the Department of Anesthesiology and the Hospital (e.g. quality assurance, infection control).
5. An anesthesiologist will full staff privileges must share on a fair and equitable basis in the responsibility for assuring 24-hour-a-day, 7-day-a-week availability of anesthesia care unless waived by the Chairman, Department of Anesthesiology.
6. Promotes guest relations during interactions with patients, visitors and employees by treating them with respect and dignity. Provides helpful service in a timely manner. Respects others privacy and maintains confidentiality. Maintains a professional appearance and a clean, safe work environment.
7. Performs other related duties as assigned by the Chairman.
1. A minimum of 4 years of training at an accredited graduate school of medicine with a degree in medicine.

Under the direct supervision of Rehab professional staff, provides non-skilled support services such as: set up and clean up for patient care, supply ordering and inventory, and equipment cleaning. May perform a variety of clerical tasks.
PRINCIPAL DUTIES AND RESPONSIBILITIES:
* Facilitates the readiness of patients for treatment.
* Transports patients to and from therapy (in-pt) or escorts patients into the out-pt treatment area from the waiting room. May also transport patient care equipment to in-pt rooms (walker, cane, etc.).
* Provides a variety of duties to assist the Rehab professionals in the provision of care in the presence of the therapist or assistant, i.e. moving the IV pole or wheelchair while a patient is being ambulated, assisting with support of a limb during ROM assessment.
* Clean and organize all treatment equipment.
* Retrieves and returns treatment supplies and equipment to and from storage/cupboards/closets.
* Prepares and clean whirlpools.

* Monitors supply par levels, assists with supply inventory and may assume responsibility for ordering.
* Maintains awareness of patient safety issues within the clinic and in the use of equipment.
* Assists in the maintenance of patient care equipment as needed.
* Provides of a variety of clerical support functions that may include: copying, filing, chart prep/break down, sorting/organizing, use of computer for such tasks as ordering, scheduling, data gathering.
* Enhances knowledge and development through participation in department meetings, in-services and training.
* Maintains patient confidentiality according to HFHS HIPAA policies.
* Maintains adherence to the HFHS Code of Conduct, as well as all other HFHS policies and procedures.
