Manages, coordinates, facilitates and supports the members of the interdisciplinary team in provision of hospice care to patient/families of VNS Health Hospice Care Program, including After Hours. Ensures the implementation of clinical/quality improvement initiatives for hospice services in compliance with regulatory requirements. Demonstrates knowledge and commitment to excellence in clinical practice and customer service. Works under general direction.
• Ensures the use of an interdisciplinary approach to assess the medical, physical, social, emotional and spiritual needs of the patient. Ensures that support and direction to patient's families are provided in accordance with the patient's Interdisciplinary Plan of Care/Hospice Care mission, objectives, policies and procedures.
• Participates in Hospice Team meetings, IDG management meetings, and serves on relevant committees for the purposes of information exchange, team collaboration, development of procedures and documentation tools, development of quality and training.
• Ensures full regulatory compliance with Hospice Conditions of Participation and standards through review, monitoring and audit (ad hoc / regular) of files, processes and procedures.
• Collaborates with Quality Improvement/Education in the development and implementation of quality improvement and educational activities to ensure professional practice standards are consistently met for each member of the team. Participates in the design, implementation, evaluation and modification of quality initiatives and educational plans.
• Initiates, coordinates, performs and assists in the analysis of clinical, regulatory and fiscal practice. Assists leadership in achieving Hospice Care goals, objectives, and fiscal targets by providing support with compliance and/or innovation to achieve improvement.
• Manages the day to day operations of the department, which includes effective caseload or visit management, achievement of productivity and revenue/expenditure targets for Hospice Care.
• Completes all work assignments, which includes audits, reports and projects.
• Performs all duties inherent in a managerial role. Ensures effective staff training, evaluates staff performance, provides input for the development of the department budget, and hires, promotes, and terminates staff and recommends salary actions as appropriate.
• Facilitates and manages the interdisciplinary group for provision of an effective and comprehensive hospice services to patient/families of VNSNY Hospice Program.

Qualifications
Licenses and Certifications:
License and current registration to practice as a Registered Professional Nurse in New York State required
Licensed and currently registered to practice as Nurse Practitioner in New York State and DEA license preferred
Education:
Bachelor's Degree in Nursing from an approved program accredited by the National League of Nursing or Health or Human Services field or the equivalent work experience required
Work Experience:
Minimum of three years clinical experience in hospice required
Effective oral, written and interpersonal communication skills required
Clinical management experience preferred

Conducts daily audits of Key Performance Indicators (KPIs), including case cart and kit audits, room visits, and post-case reviews.

Coordinates communication and planning for priority instruments and turnaround times, informing the surgical team of any delays.
Responds promptly to surgical team calls regarding instrumentation problems or urgent needs.

Assists with documentation, kit updates, pick-list revisions, and maintaining accurate instrument count sheets with coordinators and physicians.

The Patient Care Coordinator is responsible for overseeing the management and coordination of care for the acute inpatient population. The PCC collaborates with rounding MAPMG Hospital Based Service Physicians, patient/family, nursing, utlization review and other members of the healthcare team to assure continuum of patient care progression for clinical and cost-effective outcomes. The PCC facilitates and coordinates with community providers and ambulatory case managers to assist with the appropriate level and transition of care for a safe discharge and preventing a re-admission.
Essential Responsibilities:
Completes an initial face-to-face assessment for every admitted member to identify discharge needs within 24hrs of admission.
Document in KPHC and communicate the assessment outcomes to determine the appropriate transition plan with MAPMG physician healthcare team and patient/family.
Active participant in daily Care Without Delay (CWD) rounds reporting on patient progression towards the established discharge plan.
Review and document discharge plan in accordance with KP discharge planning documentation policies, facility policies, and regulatory requirements.
Document any updates, care progression and barriers to discharge daily, and as indicated on assigned patients.

As a Nuclear Medicine Technologist at Intermountain Health, you will play a vital role in supporting our clinical team, ensuring our patients receive the best care by:
Perform a variety of Nuclear Medicine procedures utilizing multiple types of gamma cameras, utilizing proper injection techniques while injecting radioisotopes.
Perform IV puncture, and administer radiopharmaceuticals via IV when indicated.
Prepare exam room, communicate with the patient, obtain informed consent when necessary, and request an accurate clinical history as necessary for the exam.
Receive patients, explain method of examination, and position patient.
Select appropriate imaging techniques and protocols to complete the exam.
Prepare and administer radiopharmaceuticals and other medications in accordance with state and federal regulations.
Minimum Qualifications
Associate degree or equivalent from two-year College or Technical School; or a minimum of one (1) year minimum related experience and/or training.
Current ARRT (R) (NM) or NMTCB as CNMT registry.
Current BLS certification endorsed by the American Heart Association.
Physical Requirements:
Hearing/listening, lifting, manual dexterity, pulling/pushing, seeing, sitting, speaking, squatting/kneeling, standing, and walking.
