What You'll Do
Evaluate and assess patients' physical therapy needs, creating individualized treatment plans.
Provide direct therapy interventions to restore function, reduce pain, and improve mobility.
Collaborate with physicians, nurses, and interdisciplinary care team members to ensure coordinated care.
Educate patients and families on therapy plans, exercises, and home programs.
Document therapy services accurately and timely in accordance with regulatory and company standards.
What We're Looking For
Graduate of an accredited program in Physical Therapy.
Current Nebraska Physical Therapy license (or eligibility to obtain).
Strong communication and interpersonal skills.

Outpatient or geriatric rehab experience preferred, but not required.

Home Care Occupational Therapist (OT) Are you an Occupational Therapist who loves one-on-one time with your patients and the flexibility of providing care in the comfort of their own homes? Join our Home Care team and help clients stay safe, independent, and living their best lives at home.
What You'll Do:
Provide one-on-one OT services in patients' homes, focusing on safety, function, and independence
Complete comprehensive assessments and develop individualized treatment plans
Train patients and families on adaptive equipment, energy conservation, and home safety strategies
Collaborate with an interdisciplinary team (nursing, PT, social work, aides, etc.) to coordinate care
Document timely and accurately in the EMR according to regulatory and agency standards
Participate in case conferences and team meetings as needed
What We're Looking For:
Current Occupational Therapist (OT) license in the state of practice
Graduation from an accredited Occupational Therapy program
Previous home health, SNF, or rehab experience preferred, but strong new grads will be considered
Valid driver's license, reliable transportation, and appropriate auto insurance
Strong communication, organization, and time-management skills
Compassionate, independent, and comfortable working in a variety of home environments

What does a day in the life look like?
Completes evaluations for referred individuals, utilizes appropriate tests and measures establishing a timely plan of care in compliance with departmental standards and federal and state regulations.

What should I have to be considered?
Graduate from an accredited physical therapy program required.
