Specialist job at Hillside Family of Agencies
The Triage Specialist I responds to calls to Hillside from strategic partners, internal staff, community members, families and individuals in Hillside programs. This role provides callers with the support needed by connecting them to the appropriate staff person, recommending Hillside or community resources or by providing telephone assistance with crisis or emergency management. This position is for at least 13 hours a week, during weekends/evenings.
Essential Job Functions
Provide excellent customer service and crisis management by telephone, email and in person.
Provide consultation and recommendations to customers regarding internal and/or external services.
Maintain thorough knowledge of agency services across regions, in addition to agency point people/contacts, including Hillside's support departments.
Respond to requests for Release of Information (R0I) ensuring the information is gathered and released within required timeframes and follows New York State privacy laws.
Process referrals and admissions to the Emergency Respite Bed Program including assessing youth for ERB level of care.
Assist with processing referrals for residential programs by entering information into EMR and completing Clinical Risk (CANS) and Health Screens, as needed.
Facilitate after hours EMR access by completing enrollments for assigned services.
Run various agency reports including Utilization, Census, Referral and Shift Reports, as requested.
Implement Hillside's emergency management protocol when appropriate in crisis situations. Triage callers according to need, successfully navigating agency on call system when needed.
Document information accurately and timely in progress notes, shift notes, emails, and/or other documentation platforms in accordance with agency policy. Ensure seamless communication between triage team members when changing shifts.
While this job description covers many aspects of the role, employees may be required to perform other duties as assigned.
Education & Experience
Bachelor's degree in social work, psychology, mental health or related human services field required.
Minimum 2 years of experience in a relevant field required.
Special Requirements
Unrestricted, valid NYS driver's license for minimum of 1 year with a clean driving record and minimum insurance coverage that meets agency standards.

Position Summary
The residential Nursing Support Specialist will function as a part of the Nursing department team and at the direction of the Nursing Director or designee. The role of the Nursing Support Specialist will be to assist the RNs and LPNs of Residential Services by attending medical appointments as determined by the needs of the program. Position Standards
Data entry of all Service Recipient's medical information into the Therap system, including updating information as needed to ensure the medical information is accurate and up-to-date.
Clerical filing of medical records/documentation as directed the Director of Nursing or designee.
Working as a team member in the residential programs with scheduling and maintaining medical appointments and follow-up for the Service Recipients as directed by the Director of Nursing or designee.
Assist with the completion of medical appointment consultation forms for medical appointments.
Transportation of Services Recipients to medical appointments or procedures.
Assisting the residential programs with ensuring each Service Recipient has an adequate supply of medications; this includes the ordering of medical supplies as needed and directed.

Position Standards
1. Delivers and documents the services, supports, safeguards, behavioral documentation and activities identified by ISP/CFA and the Day Habilitation plan. Completes the monthly note and ensure it covers the individual's response to service within the seven days of the following month for assigned individuals.
2. Performs individual specific personal care as required, including but not limited to assistance with toileting, transferring, changing needs, handwashing and oral care. Documents any finding on appropriate forms including but not limited skin integrity, bowel and repositioning charts.
3. Ensures active engagement and appropriate communication with staff and individuals.
4. Assumes responsibility for medication administration and appropriate documentation in an accurate and timely manner.
5. Develops a trusting and friendly rapport with individuals served while maintaining professional boundaries.

The Center delivers programs and services to youth and families across Monroe County who represent many racial, cultural and geographic groups.
