Key Responsibilities:

Act as a liaison between members, families/caregivers, and healthcare teams to ensure access to person-centered care.

Clinical Care Manager Requirements:
Degree from an Accredited School of Nursing or bachelor's degree in nursing required.
2+ years of experience in behavioral health management and/or acute behavioral health care setting focusing on outpatient/inpatient utilization, case management and discharge planning
1+ year of experience in current case management as a licensed professional
Active RN License in State of Practice and/or Compact State Licensure required.
BH License
Behavioral Care Manager Requirements:
Master's degree in behavioral health or graduation from an accredited school of nursing with a focus
on behavioral health.
Minimum of 4+ years of related experience in behavioral health care management or a similar healthcare setting.
Must possess a valid Licensed Clinical Behavioral Health Professional license or Registered Nurse (RN) license, depending on state requirements (e.g., LCSW, LMSW, LMFT, LMHC, or RN with behavioral health experience).
