Job Details
Functions as the clinical resource for patients and staff for ostomy, continence disorders, acute and chronic wound and skin breakdown, and fistulae.
Provides direct wound and ostomy care in the inpatient and outpatient setting.
Provides patient/family teaching and instruction of wound and ostomy management both pre and post-operatively.
Assists the healthcare team in the evaluation of skin and wound products and equipment.
Provides educational programs, in-services, and bedside teaching for the nursing staff.
Stays up-to-date with skin care practices and shares advances in wound and skin care assessments, evaluations, treatments, and prevention measures.
Assesses all patients with a pressure injury staged at Three (3) or above.
Sees all patients that have developed a hospital-acquired pressure injury (HAPI) and meets appropriate reporting requirements.
Assesses overall hospital needs and practices to eliminate HAPIs.
Job Requirements
Minimum of four (4) years of clinical experience.

Customer Service

Assists provider with procedures to include: circumcisions, frenotomies, ear piercing, and catheterizations to obtain urine specimens on pediatric patients. Preps patient for procedure using sterile technique, ensures patient safety by utilizing appropriate restraints if required. Provide comfort measures during and after procedure; observe for bleeding and dress wound.
Pediatric measurements: obtain height, weight, and head circumference for newborns to 18 years. Recognizes deficits in growth progression and/ or development and notifies provider.
Screenings: Provides accurate screenings for vision, hearing, transcutaneous bilirubin, autism, etc. Utilizes equipment appropriately and according to age groups as appropriate.
Patient/Guardian Education - Provide education as appropriate for: child development, asthma, autism, breastfeeding, healthy eating, use of medication equipment and/or medical equipment (i.e. Albueterol inhaler, crutches, etc.).
Utilization of CAIR to obtain immunization history for new patients.
Utilization application to obtain vital information for newborn patients. This would include bilirubin levels, vitamin K levels, and vaccines administered during hospitalization.
Referral Entry: Utilize both SRS portal and related applications to enter referral for services and/or supplies not provided in clinic.
Knowledge, Skills, and Abilities
Skills as a LicensedPracticalNurse or Licensed Vocational Nurse. Demonstrated proficiency in clinical skill and nursing judgment.
Ability to prepare patients for examinations, tests, or treatments and explain procedures.

Position Summary
The Licensed Vocational Nurse (LVN) is responsible for caring for patients in the ambulatory setting by assisting in the collection of subjective and objective data, recognize problems or abnormal conditions specific to the patient and perform a psychosocial assessment using active listening, verbal communication, and non-verbal communication. The LVN will contribute to the development of the patient plan of care and will implement, under the direction of a physician, AHP, or RN, only those parts of the plan of care that are within the LVN scope of practice.
* Placement in the pay range is based on multiple factors including, but not limited to, relevant years of experience and qualifications. Minimum Requirements
Experience
* Prior nursing or MA experience in pediatric environment required
* Must be comfortable working with and giving injections to babies and children of all ages
Education
* Current California LVN License with IV Certification required
* Current BLS for Healthcare Providers required
