Pay is $18.54/hr to $24.14/hr based on experience

Active CNA in Ohio through ODH's Nurse Aide Registry.

Middleburg is expanding with their advanced-practice provider coverage, and looking for compassionate, hard-working individuals to join our large group of advanced-practice providers. Applicants will be joining a fast-paced program and will be responsible for ambulatory care for all age-range patients.
The primary care department is ready to add a qualified Advanced Practice Provider to its team. The location would be Middleburg/West Market. This coverage will include both direct patient facing and virtual work schedule. It will include floating with a home site of Middleburg Heights. The provider in this role will work to the top of competency providing holistic care to children/adults in family and internal medicine. All candidates must be graduates of an accredited program to be considered. The ideal candidate has ambulatory experience, preferably in primary care and should be comfortable handling a wide range of responsibilities throughout the day including patient care, procedures and working with a team to accomplish team care. The standard hours for this role vary by location and need. Weekends, holidays, and call may be required for the role. Preferred qualifications: 1 year advanced practice experience in primary care.
Responsibilities:
Conducts thorough medical histories, performs complete physical examinations (where indicated), initiates appropriate lab, radiology tests or other special tests required for evaluation of illness, and scrutinizes lab data to establish a record of the patient's current health status and to develop a working diagnosis and treatment plan. Works in collaboration with the health care team.
Performs and interprets common laboratory, radiologic, cardiographic and other routine diagnostic procedures used to identify pathophysiologic processes as credentialed and privileged by the Medical Executive Committee and the Governing Body of the applicable hospitals.
Performs routine and specialized procedures as credentialed and privileged by the Medical Executive Committee and the Governing Body of the applicable hospitals.
May prescribe and monitor medications as allowed by license through applicable state board and Cleveland Clinic policies.
Educates patients and answers questions regarding their disease, treatments, related drug and treatment side effects and hazards.

Breastfeeding Medicine is ready to add a qualified Family/Adult Nurse Practitioner or Physician Assistant to its team within the pediatric ambulatory department at Middleburg Heights FHC and Independence FHC.
Working as an independent provider seeing both infants and mothers in the specialty area of breastfeeding. Collaboration with breast feeding medicine pediatric specialist on site as well as other pediatric providers.
Knowledge and experience with mothers /infants who are breastfeeding is required and IBLC preferred.
Responsibilities:
Conducts thorough medical histories, performs complete physical examinations (where indicated), initiates appropriate lab, radiology tests or other special tests required for evaluation of illness, and scrutinizes lab data on new and former clinic patients and hospital admissions to establish a record of the patient's current health status and to develop a working diagnosis and treatment plan.
