A caregiver who excels in this role will:
* Transport patients and their personal property using wheelchairs, carts and beds through the admission, discharge and transfer procedures.
* Transport patients with/without IVs and oxygen to and from Physical Therapy, Occupational Therapy, Radiology, Radiation Oncology, Physiology and other departments, including the morgue as necessary.
* Clean, sanitize and account for transportation equipment, wheelchairs and patient carts and store in designated areas.
* Provide wheelchairs for ACC, other areas and visitors, restock portable oxygen tanks and respond to all transport requests with a sense of urgency.
* Activate and complete tasks at the start or end location via phone system.
* Notify nursing caregivers, clinicians, technicians or reception upon arrival of the patient.
* Complete required logs and forms, including Critical Information.
* Verify correct patient identification through appropriate means (i.e. armband, chart, pager, DOB, etc.) for proper mode of transportation and standard precautions.
* Assistnursing caregivers in bedding or moving patients from bed into wheelchair or onto stretcher.
* Consult with nursing caregivers before lifting and transferring patients and obtain assistance when necessary.
* Complete transports efficiently and notify dispatchers when assignments are initiated, completed or when delays/cancellations are encountered.
* Understand and apply knowledge of basic patient transport concepts, infection control practices and proper body mechanics for lifting and moving patients.
Minimum qualifications for the ideal future caregiver include:
* High School Diploma or GED
* Demonstrated interest and aptitude
* Expressed genuine concern and care for patient welfare
Physical Requirements:

Take and record oral, rectal and axillary temperatures, pulse, respiration and blood pressure when ordered with appropriate completed/demonstrated skills competency. Assist in dressing and undressing patients as assigned. Assist in meal prep and feeding patients as assigned.
Extended Services: Assist in ambulation and exercise as instructed by the primary care nurse or therapist. Perform range of motion and other simple procedures as an extensional therapy service as ordered with appropriate completed/demonstrated skills competency.
Communication: Maintain good relationships with the patient, the patient's family, and staff. Reports any changes to the patient's medical and living conditions to the supervisor or the primary nurse.
Minimize Patient Risk: Meet safety needs of patient and using equipment safely and properly (foot stools, side rails, Oxygen, etc.). Provide proper care and observation of patient's skin to prevent breakdown of tissue over bony prominence.
Policies: Adhere to the organization's documentation and care procedures and standards of personal and professional conduct.
Operations: Attend in-service training and mandatory agency meetings.
HARD & SOFT SKILLS:
Compassionate communicator with a positive attitude.
Understanding of the hospice philosophy, comfortable providing specialized care to the terminally ill and secure with the issues of death/dying.
Patience is a virtue when working with patients, families, physicians, and coworkers.
Attention to detail is critical, as is being observant and following directions.
REQUIREMENTS:
Meets the training requirements of the State and has successfully completed a state established or other training program that meets the State's requirements of a competency evaluation program, or state licensure program.

Solon is expanding with their advanced-practice provider coverage, and looking for compassionate, hard-working individuals to join our large group of advanced-practice providers. Applicants will be joining a fast-paced program and will be responsible for ambulatory care for all age-range patients.
The primary care department is ready to add a qualified Advanced Practice Provider to its team. The location would be Solon /East Market. This coverage will include both direct patient facing and virtual work schedule. It will include floating with a home site of Solon. The provider in this role will work to the top of competency providing holistic care to children/adults in family and internal medicine. All candidates must be graduates of an accredited program to be considered. The ideal candidate has ambulatory experience, preferably in primary care and should be comfortable handling a wide range of responsibilities throughout the day including patient care, procedures and working with a team to accomplish team care.
