Responsibilities
Working under the supervision of and in collaboration with CPIhealth physicians, diagnose symptoms and implement treatment plans to reduce pain and restore function.
Perform medical histories, examine, and treat patients as part of a healthcare team.
Ensure safe and compliant clinical and surgical procedures are provided to patients utilizing the following:
Evaluation - elicit a detailed and accurate history; perform an appropriate physical examination; order, perform, and interpret appropriate diagnostic studies; define problems; develop management plans; and record and present data.
Monitoring - implement patient management plans, record progress notes, and participate in the provision of the continuity of care.
Patient Education - counsel and educate patients regarding issues of health care management, to include compliance with prescribed therapeutic regimens, interventional procedures, and ancillary services (i.e. physical therapy, bracing, TENS).
See new patients, as needed, accurately complete H&P, HPI, vitals, preliminary diagnosis and POC.
Follow up with existing patients in a timely manner to discuss improvements from a psychological, social, interventional, and opioid perspective.
Perform hospital consults on CPIhealth patients as requested by CPIhealth physician and per call schedule.
Provide appropriate professional referrals to physicians, therapists, and imaging.
Follow safe clinical procedures and make correct non-compliance issues in a timely and professional manner.
Document patient information in an accurate and timely manner.
Confer with physician on assessment, diagnosis, and plan of treatment.
Ensure timely follow up with patient care by making sure support staff are aware of the Plan of Treatment.
Maintain contact with referring and primary Physicians.
Participate in marketing activities.
Attend regularly scheduled company and peer meetings
Provide coverage and support to facilities not regularly scheduled.
Maintain professional credentials and take a leadership role in peer education.
Staff and peer training and motivation.
Performs other duties as assigned to support the mission, values, and strategies of CPIhealth.

Qualifications
Must be able to provide services and support for 1 - 3 members in your own home on an on-going basis
Must successfully complete PPCH's required training courses
Must be able to chart individual daily member activities and medications in a web-based system (training is provided)
Have professionalism and compassion
Able to focus on individualized care and support
Ability to teach others
Have good written and verbal communication skills
Have a valid driver's license and reliable transportation
Must clear background screening and reference checks
Be proficient with Microsoft Office, Google Office Suite, or similar software
Experience with intellectual or developmental disabilities is preferred, but not required

Are you the HomeHealthAid Caregiver we're looking for?
To thrive as a Caregiver / Personal Care Aide at Live Star Home Care, you'll need a blend of empathy, patience, and practical skills that empower you to provide exceptional support. Essential certifications such as CPR and First Aid, along with specialized training in Alzheimer's and dementia care, will be crucial in ensuring the safety and well-being of our clients. While a CNA or HHA certification is optional, any experience in personal care-like assisting with bathing, grooming, and mobility-will make a significant difference.
Communication skills are key, with bilingual abilities in English and Spanish or other languages being a fantastic bonus. You'll also need a knack for light housekeeping, meal preparation, and maintaining a clean living space. Being observant and attentive, you'll report any changes in your clients' conditions, always following care plans and agency policies.
Knowledge and skills required for the position are: CPR and First Aid Certification
Alzheimer's/Dementia Care Training
Medication Administration (QMAP)
CNA or HHA certification (optional)
Bilingual (English/Spanish or other languages) a plus
Assist clients with personal care needs such as bathing grooming, dressing, toileting and mobility.
Provide companionship and emotional support through conversation and engagement.
Help with light housekeeping laundry and maintaining a clean living space.
Prepare and assist with meals and snacks according to dietary preferences.
Support clients with transfers, ambulation and safe movement throughout their homes.
Provide medication reminders (non-medical).
Accompany clients to appointments and errands or recreational activities (as assigned).
Observe and report any changes in the client's physical or emotional condition to the supervisor.
Follow care plans, schedules and agency policies to ensure consistent and reliable service.
Ability to lift up to 25-50 lbs as needed for client support or transfers.
Minimum age of 18 years. Must be legally authorized to work in the U.S.
