Provides personal care services under the direction of the Registered Nurse. The HomeHealthAide is assigned to specific clients by the Staffing Coordinators and performs services for clients as necessary to maintain their personal safety, comfort and wellbeing.
Reports to: Field R and office nurse.
ESSENTIAL FUNCTIONS/AREAS OF ACCOUNTABILITY
1. Performs simple procedures as an extension of therapy services under the direction and supervision of the therapist.
a. Range of motion exercises
b. Assistance in ambulation or exercises
2. Performs personal care activities, including but not limited to:
a. Bathing
b. Shampooing
c. Skin care/nail care
d. Oral hygiene
e. Shaving
f. Dressing
3. Performs household services essential to health care at home, including but not limited to:
a. Meal preparation/feeding
b. Laundry
c. Light housekeeping
4. Remind client to take medication as ordered
5. Reports any changes in the client's condition and/or needs to the Registered Nurse.
6. Documents client's provider and completes the forms required for the client's records. Completes the appropriate records to document client's given and pertinent observations.
7. Promotes personal safety and a safe environment for clients by observing infection control practices, following agency guidelines, and reporting unsafe situations to the RN Supervisor.
8. Accompany client to medical appointments and grocery shopping.
9. Should be able to lift 50-75lbs weights, transfer patients from bed to chair, chair to bed and ambulate patients on a wheelchair and/or walker.
10. Demonstrates safe practice in the use of equipment. Does not use equipment until orientation has been provided. Notifies supervisor of educational needs.
11. Communicates effectively with all members of the interdisciplinary team through verbal reports, participation in staff meetings, and team conferences, as requested.
12. Maintains confidentiality in all aspects of the job.
13. Participates in required in-service programs to meet compliance requirements and indicate learning needs to agency supervisor.
14. Performs other related duties and responsibilities as deemed necessary.
Qualifications
QUALIFICATIONS
1. Successful completion of a formal certification training program and written skills test and have a HomeHealthAide License from DC Board of Nursing.
2. Be at least eighteen (18) years of age.
3. Minimum of three (3) months' work experience in a supervised setting.
4. Demonstrated ability to read, write, and follow a written Plan of Care.
5. Good interpersonal skills.
6. Current driver's license OR reliable transportation.

Maintain patient confidentiality/adheres to HIPAA requirements and agency policy and procedures manual.
Possess the ability to follow written and oral instructions.
Report abnormal findings in patient's conditions as observed and per care plan to RN.
Qualifications/Educational Requirements:
Graduate of an accredited High School or equivalent preferred.
Ability read, write and follow directions.
Must be a Certified Nursing Assistant(CNA) or Geriatric Nursing Assistant (GNA).
Demonstrate compassion, responsibility, and cheerful attitude.
A qualified homehealthaide is a person who has successfully completed:
A training and competency evaluation program as specified in this job description; or
A nurse aide training and competency evaluation program approved by the state as meeting the requirements of § 483.151 through § 483.154 of this chapter, and is currently listed in good standing on the state nurse aide registry; or
The requirements of a state licensure program that meets the provisions of the training and competency requirements.
