This position delivers professional nursing care to patients in accordance with the established plan of care and Company policies and procedures. Provides care consistent with all relevant professional standards and state and federal requirements
ESSENTIAL DUTIES:
Assess, plan and intervene using the nursing process to meet the patient's health care needs.
Observe and report any signs or symptoms indicative of changes in condition of the patient and family situation. Document such reports and observations in the clinical record and notify supervisor.
Perform assigned nursing procedures, including the administration of treatments and medications in the care of the patient, in accordance with the State Nurse Practice Act and the plan of care.
Teach the patients/caregivers appropriate self-care techniques.
Notify the Case Manager, physicians, and families of changes in the patient's condition.
Perform LPN and HomeHealthAide supervisory visits, in accordance with state regulations, to evaluate competency and provide appropriate instructions.
Maintain data collection, recording, and reporting systems to ensure proper service, uniform accounting, data collection, and measurement of outcomes.
Communicate potential risk management issues and report abuse or unsafe/illegal practice as required by law.
Attend and participate in multi-disciplinary patient case conferences, staff meetings and in-service education programs as assigned.
Work with patient, family and health care team members effectively to coordinate patient care.
Participate in the on-call schedule and weekends/after hour visits, as necessary.
Maintain a high degree of confidentiality at all times due to access to sensitive information.
Maintain regular, predictable, consistent attendance and is flexible to meet the needs of the agency.
Follow all Medicare, Medicaid, and HIPAA regulations and requirements.
Abide by all regulations, policies, procedures and standards.
Performs other duties as assigned.
POSITION REQUIREMENTS & COMPETENCIES:
Graduate from an accredited registered nursing program
Licensed as a RN in the state of practice
At least one (1) year clinical practical nursing experience preferably in intensive care, rehabilitation or medical surgical nursing
Homehealth experience preferred
Ability to make home visits and provide nursing care in the home environment
Ability to complete necessary documentation appropriately, legibly and timely
Interpersonal, verbal, and written communication skills necessary to complete job responsibilities
Must have reliable transportation, current driver's license and appropriate automobile insurance; or ability to take public transportation

Minimum Qualifications
Education:
High school diploma or equivalent required.
Experience:

Certifications and Licenses
Medication administration certification (as required by Pennsylvania regulations; may be obtained after hire).

Responsibilities and expectations:
Understand, perform and adhere to the information and precautions contained in the client Home Plan of Care.
Assist the client with personal care activities including: bathing, skin care, back rub, hair care, nail care, dressing and undressing, feeding, oral hygiene, shaving, grooming and bedtime preparation.
Assist the client to the bathroom or in use of urinal or bedpan. Keep incontinent clients clean and dry.
Assist the client with self-administration of their medications.

Assist with meal planning, purchase of food and meal preparation.
Assist with client's laundry and bed linens.

Required knowledge, skills and abilities:
Must be eighteen (18) years of age.
Must live within 25 miles of Great Falls, MT.
Minimum availability of 28 hours a week, preferred.
6 months of caregiving experience, preferred.
Valid certifications as a Certified Nursing Assistant (CNA) or equivalent qualification for PCAs are preferred.
Current CPR and First Aid certifications are preferred.
Demonstrate a sympathetic and positive attitude while caring for others.
Previous experience in private duty home care preferred.
Ability to regularly and predictably report to multiple locations on a daily basis.
Ability to listen and communicate clearly, fluently, and diplomatically - both orally and in writing.
Ability to remain flexible, resilient, calm and maintain a sense of humor in all situations.
Present a well-groomed image that reflects the professional image of the business.

Job Description
An individual who has successfully completed an approved C.N.A. course and examination. The individual is employed to provide patient care as assigned by the Registered Nurse who supervises this “unlicensed” member of the healthcare team.
Demonstrates the ability to deal with pressure to meet deadlines, to be accurate, and to handle constantly changing situations. Demonstrates the ability to deal with a variety of people, deal with stressful situations, and handle conflict.
Will perform all job duties or job tasks as assigned. Will follow and adhere to all requirements, regulations and procedures of any licensing board or agency. Must comply with all Benefis Health System's organization policies and procedures.
Education/License/Experience Requirements:
High School graduate or equivalent preferred.
Current Certified Nursing Assistant (CNA) certification in State of Montana for Long Term Care Facilities.
Current BLS Certification and maintain per policy.

As a HomeHealth Certified Nursing Assistant, here's a big-picture view of what you'll do:
Care for seniors and those with an illness, injury or chronic disease in their homes
Help with daily activities such as bathing, toileting, dressing, grooming, hygiene, nail/skin care, eating and nutritional intake
Assist with ambulation, transfers and range of motion exercises
Check vitals, measure intake/output, monitor blood glucose and document any changes
Work with a team of homehealth nurses and aides to ensure a patient's care plan is followed
Observe patient and notify supervisor of issues requiring a change in care, medication or medical equipment
A few must-haves for HomeHealth Certified Nursing Assistants:
High school diploma (or equivalent) and active Nursing Assistant Certification in Montana
CPR certification
Covid-19 vaccination card
Valid Montana driver's license, auto insurance and transportation
Compassionate nature, good communicator and ability to lift up to 50 lbs.
