Responsibilities:
Perform patient assessments and assist in developing individualized care plans under the supervision of a Registered Nurse (RN).
Administer medications, treatments, and therapies as prescribed by the physician.
Monitor patient progress and report any changes in condition to the supervising RN or physician.
Provide patient and family education on healthcare management, medication administration, and self-care techniques.
Collaborate with the interdisciplinary team to ensure coordinated and comprehensive care delivery.
Maintain accurate and up-to-date patient documentation in accordance with regulatory standards and organizational policies.
Provide emotional support and advocacy for patients and their families throughout the care continuum.
Qualifications:
Current and unrestricted RN license in the state of Pennsylvania.
Minimum of six months of clinical experience
Strong clinical assessment and critical thinking skills.
Excellent communication and interpersonal skills.
Ability to work independently and collaboratively within a multidisciplinary team.
Reliable transportation and willingness to travel to patient homes within the Doylestown area.

Position Overview
Registered Nurse (RN) is responsible for providing and documenting skilled nursing care in accordance with the developed care plan and physicians orders for each individual patient while adhering to confidentiality standards and professional boundaries at all times.
Essential Job Functions
• Responsible for the delivery and coordination of quality patient care in compliance with physician orders.
• Continuously observes and assesses patient condition and care needs and reports changes in condition to the supervisor and/or physician as appropriate.
• Documents all activities, assessments, nursing actions, responses and coordination of care in a timely manner whenever care is delivered.
• Develop, implement and update the nursing care plan.
• Takes appropriate nursing action based on assessment and achieves expected outcomes.
• Recognizes changes in patient needs and responses requiring intervention and implements care to prevent risk or reduce risk.
• Accepts responsibility for personal and professional accountability by complying with Aveanna policies, state and federal regulations, accrediting bodies and the Nurse Practice Act.
• Provide care utilizing infection control measures that protect both the staff and the patient according to OSHA standards.
• Educates the patient and family regarding the disease process, self-care techniques, and prevention strategies, and in meeting the patient's nursing needs.
• Maintain knowledge of competencies related to the nursing profession by participating in educational programs, continued education units, internal learning management skills and skill evaluations.
Requirements
• Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing
• Current, unrestricted state license as a Registered Nurse in the state of practice
• Current CPR certification
• Demonstrated proficiency in clinical assessments, documentation and compliance with nursing care and policies and procedures
Additional state specific requirements:
• South Carolina - One (1) year of pediatrics experience
• California - One (1) year of experience required working under current nursing license
• Louisiana - One (1) year of experience required working as a licensed nurse
• Continuing Education as required by state
Additional discipline specific requirements:
• Medical Oncology RN - 2 years Registered Nurse experience with cardiac monitoring and oncology experience
• Ambulatory RN - 2 years experience with vital assessment, plan of care development, and telephone triage
• Medical Surgical RN - 2 years Registered Nurse experience
• Peri-Operative Care RN - 2 years of Registered Nurse, preferably with oncology experience
Preferences
• Six (6) months of recent experience as a Licensed Nurse in a clinical care setting
• Home health experience

The primary purpose of your job is to provide a safe environment, give emotional and social support and attend to the resident's physical needs and comfort. Performs direct care to the residents as assigned, in accordance with the resident's care plan, and as directed by your Supervisor. Performs other duties as directed by representatives of the community. Follows all health, sanitary and infection control policies and maintains established standards of practice set forth by the community's administration and Nursing Policies and Procedures.
Education & Qualifications:
Current registry of certification to work as a Nursing Assistant in good standing within the applicable laws of the state in which you work, or otherwise possesses the requirements to work as a Nursing Assistant under the laws of that state.
Must be a high school graduate or equivalent.
Must be at least 18 years old (if 16 years old, must currently be in high school.)
Must be able to communicate in English and have the ability to follow verbal and written instructions.
Must possess and demonstrate the ability to carry out both verbal and written directions.
Must possess and demonstrate good interpersonal skills and attention to detail.
Ability to work with supervisors, co-workers and community staff in the performance of duties.
Ability to observe proper safety and sanitary techniques.
Ability to work hours as scheduled based on the requirements of the position/assignment.
Must not pose a direct threat to the health and safety of others in the workplace.
Experience:
Previous experience in long term care preferred, but not required.
