KEY RESPONSIBILITIES Provides care in compliance with the patient's established Plan of Care such as: Accountable for providing care that is within the scope of training Helps to maintain optimal hygiene and assists in maintaining a safe environment May plan and prepare nutritious meals based on approved diet Assists the patient with ambulation as tolerated Encourage patient independence Performs routine housekeeping tasks Reports any change in the patients mental or physical condition to the Clinical Manager or Director of Nursing Maintains an updated clinical record on the patient at all times, meeting required deadlines for documentation Communicates with FFHC nursing staff and any other FFHA caregiver as needed for continuity of patient care Participates in FFHC Quality Assurance, Performance Improvement program (QAPI) as requested Continually strives to improve his/her skills Attends and/or completes all training/ in-services as assigned Submits required personnel file documentation in a timely manner Adheres to FFHC compliance and ethics expectations Maintains patient and family confidentiality Maintains professional, supportive, and responsive interpersonal communication skills Performs other duties as assigned. ROLE QUALIFICATIONS EDUCATION & EXPERIENCE REQUIRED 18 Years of Age or older 86-hours of training under the direction of a registered nurse ( training provided at the time of hire ) Must be an eligible relative to the patient. (Patient must be 21 years of age or younger).
