We are seeking a skilled and compassionate Physician Assistant (PA) or Nurse Practitioner (NP) to join our inpatient PM&R/palliative care team. The ideal candidate will have experience in PM&R and will work collaboratively with the physicians, advanced practice providers and other healthcare professionals to deliver high-quality, patient-centered care.
Key Responsibilities:
Conduct comprehensive assessments including history, physical examinations, interpretation of diagnostic studies.
Manage chronic conditions,
Evaluate and triage new patient referrals and follow-up visits.
Order and interpret laboratory studies, diagnostic testing
Develop and implement evidence-based treatment plans with collaborating physicians
Provide patient education on disease prevention, lifestyle modifications, and medication management
Assist with hospital rounding, consults, and discharge planning (if applicable)
Participate in care coordination with other providers and specialties.
Document all patient encounters thoroughly and accurately in the Electronic Medical Record
Maintain compliance with organizational policies, regulatory guidelines, and best practice standards.
Weekend consults and night call as assigned
Qualifications:
Graduate of an accredited Physician Assistant or Nurse Practitioner Program
National Advanced Practice Registered Nurse Certification (APRN) OR national board certification as a Physician Assistant.
Current Physician Assistant or APRN license in the State of Florida.
Current CPR, BLS, and ACLS required
Preferred Skills:
Experience in Physical Medicine & Rehab or Palliative Care preferred.
Experience with Epic Electronic Medical Record

Responsibilities
Personal care functions including bathing, skin care, toileting, grooming, oral hygiene,and assist with dressing and undressing
Assist with feeding of residents
Measuring and recording intake and output
Weigh residents using upright, chair and bed scale
Assist in turning and positioning of residents
Proper transfer techniques
Demonstrate appropriate knowledge for safe use of medical equipment (cane, crutches, walkers, Hoyer lift, side rails)
Help keep resident's rooms clean and supplied
Make and change beds
Transport residents, supplies and equipment as needed
Ambulate residents who require minimal assistance
Perform skin assessment and notify RN of any abnormalities in skin integrity
Answer resident calls and take appropriate action
Collects and label specimens (urine, stool, sputum)
Immediately report any changes in clients' condition or incidents to the Nursing Supervisor
Maintains confidentiality in relation to all clients, healthcare staff and documentation
Maintains annual in service requirements in accordance with the facility and state regulations
QUALIFICATIONS:

· Assists residents in ambulation with or without self-help devices as instructed.
· Performs restorative procedures as instructed.
· Accurately measures and records heights and weights.
· Accurately measures and records temperatures, pulses, respirations and blood pressures (TPRs and B/P) as instructed.
· Empties urinary drainage bags and records amount in record as indicated; reports any irregularities to nurse.
· Ensures call lights are within reach of residents and answers call lights promptly.
· Ensures that residents who are unable to call for help are checked frequently.
· Ensures that residents' personal care needs are being met in accordance with residents' wishes.
· Reports all changes in residents' condition to supervisor as soon as practical.
