Under the direct supervision of a Registered Nurse/licensed Nurse, this Job will assist with routine and repetitive patient care activities in a nursing unit. This Job also performs specific clerical, organizational, and patient-focused activities. The patient-focused tasks will be supportive in nature.
Responsibilities:

Assists with the admission, transfer, and discharge of patients.
Completes pertinent documents.
Prepares rooms for admission by setting up IV pots, and adjusting bed to the appropriate level.

Safely transports stable patients as required.
Maintains security/confidentiality when transporting patients and interacting with family and visitors.
Cleans, procures and returns equipment and supplies.
Run errands for the unit as required.
Assist Licensed personnel in providing a safe and comfortable environment at all times; replenish patient room with water, linen, and towels, as appropriate.
Perform unit-specific patient care tasks in accordance with the skills checklist and documents on the flowsheet.
Identify obvious changes in the patient's condition and take responsibility for reporting those changes promptly to the nurse in charge.
Assist with the ordering and maintenance of stock items according to established inventory when needed.
Keeps unit supplies in an organized and orderly fashion.
Respond appropriately to emergencies.
Communicate effectively with other team members during and between shifts.
Responds to phone calls and patient call lights.
Appropriately adapt assigned patient assessment, treatment, and/or care methods to accommodate the unique physical, psychosocial, cultural, age-specific, and other developmental needs of each patient served.
Demonstrates competence to perform assigned patient care responsibilities in a manner that meets the population-specific and developmental needs of patients served by the department.

RESPONSIBILITIES
It is to be understood that the Nursing Assistant works by general, consistent, pre-arranged "Primary" group assignments as the basis for each shift worked, but must be able to accept changes and adapt to added assignment as the need arises. In addition, each Nursing Assistant must work with each other to assure safe practices for each resident and staff are followed.
1. Signs in per facility protocol.
2. Checks residents and makes rounds on assigned group noting significant issues for intervention. Begin planning time to assure all resident's needs are met throughout the shift.
3. Assists residents as indicated to provide A.M., P.M. and daily care to assure residents are clean, dry, toileted, continent, well-groomed and cared for in the manner they deserve. Follows restorative plan as outlined on the Interdisciplinary Care Plan. Provides for privacy and dignity at all times.
4. Assists residents to meals. Assists residents to toilet prior to meals. Assists resident to wash hands and face, brush teeth, put on glasses, get to dining room and in good position prior to meal.
5. Provides assistance and restorative eating as indicated. For those residents who are the most debilitated, consider their weakness in planning time for the whole group of residents.
6. After meal, assist resident to wash hands and face, return to their room and toilet as needed.
7. Assists in dining areas as assigned. Offers assistance to residents as needed. Offers substitutions for items not eaten well by any resident. Promotes atmosphere conducive to increased nutritional intake for each resident in the dining room.
8. For those residents unable to go to the dining room, assists in service, assisting and supervising meal service as assigned.
9. Assists in transporting used trays to the kitchen following appropriate infection control policies and procedures.
10. Assists residents to plan their time to enable them to be well cared for as well as attend their desired functions during the shift. (i.e. activities, visits with each other and their families, outings, meals, quiet times, etc.)
11. Answers call lights timely assisting with care as indicated. Keeps call lights available for the residents.
12. Encourages and motivates residents to participate in recreational activity of body and mind.
13. Provides and keeps fresh water and fluids available for the residents at all times. Offers fluids at least every 2 hours or each time in the room, when appropriate.
14. Timely serves and provides assistance with between meal nourishments as indicated.
15. Follows bath and whirlpool schedules as outlined for each resident. Do nail care, wash hair, shave, etc. to assure personal hygiene practices are always followed.
16. Checks all residents frequently and at least every 2 hours assuring the resident is repositioned, turned, toileted, clean, safe, etc. Provides intervention as needed to provide a safe clean environment.
17. Checks all residents daily for condition of skin, reporting findings to Licensed Staff assigned to unit.
18. Assists licensed staff when needed to position residents in need of treatments.
19. Keeps hand rolls and positioning devices in place as needed and as indicated for each resident.
20. Assists residents to bed for naps or night sleep as needed. Provides a quiet restful and safe atmosphere when residents are resting.
21. Takes and records vital parameters as assigned. Assists with weekly and monthly weights as assigned.
22. Cleans and properly arranges all bedroom furniture to include keeping the residents orderly, clean, and following infection control guidelines.
23. Follows Plan of Care.
24. Reports any changes in condition or any resident/family concerns to licensed nurse.

31. Keeps Licensed Staff informed of pertinent information timely. Writes pertinent information on the Group Assignment Sheet and Communication Books.
32. Provides information accurately and timely for information to be added to the residents Interdisciplinary Care Plan.
33. Cleans ancillary rooms and equipment as assigned. Assists other members of the staff to keep each area of the facility clean, orderly, safe, and a homelike atmosphere for the residents.
34. Assists other staff members as needed throughout the shift. Cooperates with other personnel to be sure residents are of prime importance to all members of the staff.
35. Assists new staff members in orientation as needed throughout the shift.
36. Maintains safe environment at all times, wiping up spills, working with housekeeping staff when available picking up articles as noted, etc.
37. Gives report to Licensed Staff and turn in completed Group Assignment sheet prior to leaving the unit to go off duty.
38. Performs other tasks as assigned.
39. Attends required staff in-services, meetings, orientation, and continuing education activities as required.
40. Is aware of and adheres to Patient's Bill of Rights and confidentiality of patient information including HIPAA regulations.
41. Interacts with patients and family members, co-workers, clinical and ancillary staff in a non-judgmental, supportive and calm manner.

II. Mission Statement

We strive to holistically meet their spiritual, social and clinical needs in the most peaceful, dignified and comforting manner possible.
III. ESSENTIAL JOB DUTIES
The Hospice Aide shall provide services established and delegated in the POC, record and notify the primary registered nurse of deviations according to standard practice including, but not limited to the following:
Perform simple one-step wound care if documentation of in-service for that specific procedure in the aide's personnel record. All procedures performed by the aide must be in compliance with current standards of nursing practice.
Provide assistance with mobility, transferring, walking, grooming, bathing, dressing or undressing, eating, toileting, and/or housekeeping needs. Some examples of assistance include:
Helping the patient with a bath, care of the mouth, skin and hair
Helping the patient to the bathroom or in using a bed pan or urinal
Helping the patient to dress and/or undress
Helping the patient in and out of bed, assisting with ambulation
Helping the patient with prescribed exercises which the patient and hospice aide have been taught by appropriate personnel
Performing such incidental household services essential to the patient's health care at home that are necessary to prevent or postpone institutionalization
Provide nutritional support
Reporting changes in the patient's condition and needs to a registered nurse
Provide other related supported tasks as specified by organizational policy.
Complete a clinical note for each visit, which must be incorporated into the record at least on a weekly basis
Obtain vital signs and report abnormal values to Registered Nurse
Restrictions: The Hospice Aide shall NOT:
Perform any intravenous procedures, procedures involving the use of Levine tubes or Foley catheters, or any other sterile or invasive procedures, other than rectal temperatures or enemas
Administer medications to any patient
IV. MINIMUM QUALIFICATIONS
Have current nursing assistant certification and have successfully completed a Hospice Aide competency evaluation; or,
Have successfully completed a Hospice Aide training program and have successfully completed a competency evaluation, or,
Have successfully completed a Hospice Aide competency evaluation; and
Exhibit maturity, a sympathetic attitude toward the patient, ability to provide care to the terminal patient, and ability to deal effectively with the demands of the job
Have the ability to read, write, and carry out directions promptly and accurately; and
When employed by more than one agency, inform all employers and coordinate duties to assure highest quality when providing services to the patients
Competency shall be evaluated by an RN prior to hospice aide performing patient care
Shall not have a finding of abuse, neglect or misappropriation placed against him/her on the Louisiana direct service worker (DSW) registry or the Louisiana Certified Nurse Aide (CNA) registry.
Note: The Hospice Aide competency evaluation is to be completed by a registered nurse prior to the Hospice Aide being assigned to provide patient care.

Has not been excluded from participating in Medicare and Medicaid programs.
Maintains eligibility to participate in Medicare and Medicaid programs throughout employment.
Capable of providing emergency care as needed for any patient.
