About the Position
We are looking for a Residential Care Associate II to join our team at our Johnson Creek SRTF in Milwaukie, Oregon! The Residential Care Associate II will provide support to the residents according to their individualized treatment plan with a focus on personal care services, environmental intervention, and limited group social and recreational treatment activities as assigned by utilizing an approved evidenced-based group curriculum. They will provide limited treatment services and personal care supports for clients that help them develop appropriate skills to increase or maintain their level of functioning. The Residential Care Associate II will administer prescribed medications, follow physician's orders for all medications and/or procedures, document on medical charts and count sheets according to established procedures, and responsibly assist clients with self-administration of medications within agency policies and procedures. They will transport clients to and from the program as necessary to engage and provide treatment services. They will maintain compliance with established productivity standards for documentation of services.
This position falls under the AFSCME bargaining unit. Wages, benefits, and working conditions are set in accordance with the collective bargaining agreement.
Training Program
With our MHACBO-accredited training program, you will earn CEUs towards applicable mental health certifications, while attending your required, paid training, and receiving qualified clinical supervision gaining on-the-job experience.

What You'll Make
$21.63 - $22.96 per hour DOE/Credentials. An additional $3.00 per hour night shift differential will apply during the work hours of 10:00pm - 6:00am.

What You'll Need
Must be eligible for Qualified Mental Health Associate registration through the Mental Health & Addiction Certification Board of Oregon (MHACBO). Eligibility requires 1 out of 3 Criteria below:
Bachelor's degree in Behavioral Science: Psychology, Counseling, Social Work, Human Development, or related field; OR minimum three (3) years of verifiable behavioral health experience working with individuals with behavioral health needs in a supervised setting; OR equivalent combination of behavioral science education in addition to satisfactory supplemental behavioral health experience and training
Will be required to register as a QMHA - R through MHACBO during the first week of employment.

We use a self-scheduling model created typically six weeks in advance, and we follow an every-third-weekend rotation and holiday rotation.
This position supports patients across the North Region, which includes Mass General Hospital and Salem Hospital.

* Identify eligible patients through participation in ED and inpatient huddles, rounds, and independent rounding.
* Gather and interpret clinical information to assess Home Hospital eligibility and safety.

The day-to-day duties of the DSP role requires:
Smart Phone
- The state requires all client visits to be tracked with GPS location verification at clock in/out. You will need a smart phone to access the timekeeping application once you arrive at the client's location.
Driving
- Have a valid license and car with current insurance. Some clients will need transportation.
Documenting
- Must create and enter specific progress notes after each shift. Also, a progress report is due once per quarter.
Scheduling
- Each DSP has a set schedule and must be able to maintain this with clients.
Reliability
- Because Family Relief Resources is not a facility, we need reliable employees who can work independently, show up for their shifts, and be excellent communicators with their client's families and with our agency.
Patient, Kind, and Positive
- Helping those with disabilities requires a person who can be patient in all situations, show kindness, and have a positive attitude toward the client and other members of the client's family even in challenging situations.
Flexibility
- Being able to adjust to different types of personalities and disabilities from day to day.
Minimum Job Requirements:
18 years or older
Pass a Criminal History Check.
Work a minimum of 15 hours per month.
Valid driver's license and insurance.
CPR and First Aid Certificate (within 3 months of hire)
State required online training courses (pre-hire and within 3 months of hire)
Continuing Education (24 hours per year)
Basic knowledge of technology, including the ability to use a smartphone for timekeeping, submitting daily and quarterly shift reports, communication with office and client family, and additional requirements as needed.
Has direct experience working with children and/or adults with varying levels of support needs (Including experience with intellectual and/or behavioral disabilities).
Understand requirements of maintaining confidentiality and safeguarding individual information.
Be literate and capable of understanding written and oral orders.
Be able to communicate with individuals, health care providers, case managers, and appropriate others.
Be able to respond to emergency situations when services are being delivered.

RESPONSIBILITIES of the Caregiver - $250 Signing Bonus
Provide for the personal needs and comfort of patients in their homes.
Perform personal care activities that assist the patient with ADL's - activities of daily living
Ambulation and Transferring Assistance as needed
Meal planning and preparation
Driving to appointments
Medication Administration
Perform light housekeeping activities including vacuuming, dusting, sweeping or mopping floors, doing dishes, changing bed linens, doing laundry and cleaning bathrooms, grocery shopping, taking client to doctors appointments.
REQUIREMENTS of the Caregiver - $250 Signing Bonus
A minimum 6 months of experience caring for a non-relative
Valid driver's license and reliable vehicle preferred
Must be able to pass criminal background check, DMV screening and professional reference checks
Must be 18 years or older

The starting salary for this position ranges from $14.00 - $15.00 per hour depending on experience and qualifications.

+ Helps patient maintain good personal hygiene by performing or supervising bathing, grooming, skin care, shaving, oral care, nail/foot care and other activities
+ Assists in feeding patients. Is able to communicate basic principles of nutrition, observe and record food and fluid intake when necessary. Safely positions patient for meals and feeds or assists in self feeding
+ Assists with patient toileting including use of bed pan/urinal, change and position catheter bags and bag change procedures on well-regulated ostomies
+ Provides necessary skills to safely assist the patient with patient mobility, exercises, positioning/turning, transfers and ambulation per Plan of Care and CenterWell HomeHealth policy
+ Provides necessary skills to appropriately report changes and document pertinent information and care rendered to patient to ensure continuity of care. Documents interactions with patients, caregivers, doctors and other staff members appropriately, legibly, thoroughly and in the amount of time allowed
+ Practice acceptable infection control principles. Provide a clean, safe and comfortable environment
+ Willingly assists with other household duties including light laundry, bed changing and bed making, light meal preparation, light housekeeping and shopping (if no other assistance is available and an MD order is present).

+ Must be in good standing on the HHA Registry (if applicable) and have completed HHA/CNA course to work for a Medicare certified agency.
