The Advanced Practice Provider for the Outpatient Behavioral Health Clinic provides quality and organized psychiatric services under the direction of a physician for identified clients and families. The scope of practice includes an emphasis on assessment, diagnosis, therapeutic treatment, medication management, and discharge planning needs. This position maintains accurate and up-to-date knowledge of all relevant regulations and standards regarding the provision of behavioral health services. POSITION ESSENTIAL FUNCTIONS: Client Care Assess, develop individualized plan for care, provide treatment (assessment, diagnosis, medication management, crisis intervention/management), evaluate the effectiveness of treatment, ensure quality services are delivered, and facilitate discharge planning/aftercare. Gather information from various sources and works with the case manager to monitor and evaluate the effectiveness of client's individualized plan related to client's condition(s), presentation/participation within the program, and supporting clients in reaching outcomes/goals. Manage time and caseload effectively. Complete all paperwork in a timely manner. Maintain appropriate boundaries with clients, ensure confidentiality is maintained, and adhere to all local, state, and federal privacy laws related to the practice of mental health and substance abuse.
