JOB SUMMARY:
A qualified Licensed Practical/Vocational Nurse administers one-on-one skilled nursing care to patients on an hourly (shift care) basis in their place of residence. This is performed in accordance with physician orders and plan of care under the direction and supervision of the Registered Nurse. Services are furnished in accordance with Agency policies.
QUALIFICATIONS:
Certified Home Health Aide (CHHA) Certificate provided by the State of California
Meets the training requirements in accordance with State and Federal laws.
At least 18 years of age.
Ability to read and follow written instructions and document care given.
Self-directing with the ability to work with little direct supervision.
Empathy for the needs of the ill, injured, frail, and impaired.
Possess and maintains current CPR certification.
Demonstrates tact, patience, and good personal hygiene.
Licensed driver with an automobile that is insured in accordance with Organization requirements and is in good working order.
RESPONSIBILITIES:

Performing range of motion and other simple procedures as an extensional therapy service as ordered (with appropriate completed/demonstrated skills competency).
Assisting patients in the self-administration of medication.
Doing patient's laundry, as appropriate.
Meeting safety needs of patients and using equipment safely and properly (foot stools, side rails, etc.).
Reporting on patient's condition and significant changes to the assigned nurse.
Adhering to the Organization's documentation and care procedures and standards of personal and professional conduct.

Benefit Planning (provide supervision and training of individual while accessing Medi-Cal benefits, applying for SSI, Medi-Cal, assist and train the individual how to make appointments, cancel appointments, attend appointments, immigration/citizen process, etc.)
Shopping Skills (provide supervision and support with shopping for groceries in the community. Personal attendant will support individual in making healthy food choices and assist with budgeting and price comparison)
Safety:
Report Special Incident (SIR) according to Regional Center and All Around Program Services' policy and procedures.
Ensure that the individual receives professional, quality services in a safe and meaning manner.
Assist with the transferring of the individual from wheelchair to vehicle, restroom, bed and vice/versa when, and if necessary.
Must have back pack and company ID badge with you at all times
Follow proper emergency procedures while maintain the safety of the individual at all times.
Practice safe driving while abiding to the California laws such as speed limits, pulling over to text or make/take a call.
Supervision and devote your time to the individual(s) at all times including while on various outings.
Programming:
Participate in development of the individual program plan (IPP), and or individual service plan (ISP) and or individual educational plan (IEP)
Monitors services and individuals progress in relationship to establish goals and objectives per individuals plans
Provide supervision and training as outlined in the individual's IPP, ISP, and or IEP.
Attends team meetings or other training as part of the continuation of education or any agency- wide training and in-service programs
Participates or provides information in individuals quarterly/semi-annual and annual meetings
Research and plan activities for each of your individual(s) according to their goals/objectives in the ISP, IEP and IPP.
Communication:
Communicate with supervisor during normal business hours regarding questions and concerns related to employment, hours, rates, individual's family, program, etc.

Must have a working cell phone at all times and it must be ON during work hours

This position performs resident/patient care activities and related services necessary in caring for the personal needs, safety, and comfort of residents/patients as assigned. 3. Performs food service functions including:

Responsibilities:
CSU Channel Islands seeks candidates for an Assistant or Associate Professor in Nursing to plan, develop and teach prelicensure and post-licensure Nursing courses in the classroom, clinical setting, and the campus simulation lab. Duties associated with full-time faculty responsibilities include but not limited to: advise students in the major; maintain accurate student records/clinical grades; coordinate clinical instruction with other faculty; coordinate students' experiences with clinical personnel; maintain communication between the University and clinical agencies including annual summation meetings; participate in curriculum development; and participate in curriculum excellence by remaining current in the subject matter and techniques of effective instruction through recent clinical experience and professional development.
Other Service oriented responsibilities include participation in Nursing Department committees; in CCNE accreditation and BRN mandated activities; development and review of department policies; participation in institutional shared governance and service to the University, the profession and broader community.
Engage in scholarly productivity including research, conference presentations, and/or publishing content related to the field of nursing and innovative developments.
Assignment may include day, evening, night, and weekend courses/clinical labs. Some nursing courses including pediatrics and psych/mental health are taught in the summer, so it is preferred that applicant be willing to work during the summer.
