    Job DescriptionOne Health is seeking an experienced fulltime Certified Home Health Aide (CHHA) to join our team of professionals. JOB SUMMARY: The Home Health Aide provides professional nursing services under the supervision of a Registered Nurse in compliance with the state's Nurse Practice Act, governmental laws and regulations, and Elite Health Care Systems policies and procedures. The Home Health Aide is a professional member of the interdisciplinary group that performs various services for a patient as necessary to meet the patients personal needs and to promote comfort. DUTIES AND RESPONSIBILITIES: Under the direction & ongoing supervision of the RN, provides essential personal care assistance to the patient: Bath, skin, and hair. Bed transfers as well as ambulation activities as ordered under the care plan Preferring such incidental household services as are essential to the patients well-being at home Preparing meals including special diet Take a patients temperature, pulse, respiration, and blood pressure if assigned by a registered nurse Assists with shopping if the patient has no other resources available Provides emotional support and compassionate care to the patient and family unit Demonstrates listening skills Offers encouragement when appropriate Maintains required competency and skills Documented initial and ongoing every 14 days supervision and evaluation by RN on each case assignment Participates in an annual performance review Report any changes in the patients condition immediately by calling the Hospice office and notifying the RN Case Manager. Practice Standard Precautions according to OSHA regulations and policy Attends at least twelve (12) hours of in-service education programs per year Maintain current certification as required by the State Maintains current knowledge and practice of infectious disease protocols Performs personal care needs after given a patient's assignment and attending to his/her requests promptly. Meeting safety needs of the patient and using equipment safely and properly (foot stools, side rails, Oxygen, etc.) Providing respite for patient's family/caregiver when on-site, as appropriate. Adhering to the organization's documentation and care procedures and standards of personal and professional conduct. QUALIFICATIONS: Minimum 1-year experience as a Home Health Aide or Hospice Aide. Must have Certification as CHHA. Understands the philosophy of Hospice concepts. Experience with the terminally ill patient. Ability to handle multiple priorities, documentation requirements and deadlines. Must have reliable automobile, valid drivers license, and the minimum state-required liability auto insurance. Able to travel to multiple locations in the course of the working day/week. Ability to assess and respond to the needs of patients, families, and caregivers and to function effectively in a nursing environment. Has excellent observation, verbal and written communication skills. Demonstrates autonomy, organization, assertiveness, flexibility, and cooperation in performing job responsibilities. Proficient computer skills required using specialized software, and equipment used in the delivery of patient care. Committed to improvement, clinical excellence, high-degree of patient and staff satisfaction. Demonstrates excellence in customer service and time management. Possession of current certification to practice as a Home Health Aide in the State of California.
