As a Foster Home Worker, you will be responsible for the case management functions of a caseload of respective foster homes. Who We Are At SAFY, our mission is Preserving Families & Securing Futures. What You'll Do
Clinical:
* Complete Facility logs to document monthly phone calls to all foster homes that do not have foster youth placed in the home.
* Complete Quarterly Environmental Audits for all assigned Foster Homes.
* Complete Foster Parent evaluations in conjunction with the assigned Case Coordinator.
* Complete Annual Interim paperwork for all assigned foster homes.
* Complete Home Study Addendums as required and collect all paperwork needed for foster home moves and/or the addition of new household members (Addendums are required for all moves into or out of the foster home; when new Household members enter the home, etc.) All Home Study Addendums are approved and signed off by the TD or Case Coordinator.
* Complete all Rules Investigations of assigned foster homes under the supervision of the Case Coordinator.
* Responsible for tracking Foster Parents training hours, along with tracking all paperwork needed by Foster Parents to remain in compliance with SAFY policies and state specific rules.
* Completion of the Large Family Assessment (the state required assessment if a Foster to Adopt home has 5 or more youth)
* Completion of the Individual Training Needs Assessment (ITNA) every 6 months.
* May conduct foster parent training
* May cover case management duties including carrying a caseload
* Will be included in on-call rotation
* Assist with auditing foster home files
* May facilitate foster parent support groups
* Assist recruiter with retention duties
Compliance/Reporting:
* Timely document outcome of home visits, support sessions, interaction with parties involved in the foster home. Document any incidents as required by policy, regulations and contract requirements.
* Compile profile using pre and post assessment tools for outcome measures. Report information to DOM/Finance department for billing and foster parent payroll as indicated.
* Review all foster home files pertaining to caseload as part of the quality improvement process.
* Be in compliance with monthly and quarterly requirements as defined annually by STARS standards.
* Inform youth and foster parents of company policies, procedures, rights and responsibilities. Enforce policies by writing corrective action plans, incident reports, and conducting rules investigations.
* Assist clinical staff with completion of surveys and tools, collect and organize necessary paperwork from foster parents, foster youth.
* Report any concerns/ incidents concerning assigned youth or foster parents immediately to clinical staff.
* Perform other duties as assigned or identified by TD.
* Plan schedule with supervisor and document activities on weekly log. Assist with planning and preparation for Divisional trainings, activities and special events. Meet with supervisor a minimum of once a month for staffing. Submit all required reports on time
Other:
* Establish and maintain a positive working relationship with all levels of SAFY staff by participating in joint meetings, and problem identification and resolution.
* Establish and maintain a supportive and positive working relationship with youth, foster parents, referring agencies, and other SAFY stake holders.
* Attend relevant in-house and external seminars, trainings, conferences and workshops to develop and enhance knowledge in the designated area as well as achieve relevant education hours per year to enhance professional and job related skills.
* Attends required meetings within division, region, and central office (team and/or community partners).
* Participate in assigned committees and chairs if so appointed. If appointed, maintains liaisons with state and national organizations.
* Meets regularly with supervisor to discuss issues and performance.
* Submit monthly/quarterly reports to immediate supervisor.
* Sensitive and responsive to the cultural differences of staff, SAFY youth, foster parents, referral sources and the public.
* May be on call 24 hours or on a rotating basis to address crisis with clients.
* Other duties as assigned by supervisor.

JOB OBJECTIVE: To work in partnership with families to promote healthy pregnancies, improve child health/development/school readiness, improve parent confidence and competence, and increase family connectedness to community and social supports. ESSENTIAL JOB DUTIES AND RESPONSIBILITIES:
Determine family's eligibility to receive Help Me Grow Home Visiting services.
Assure that every family has a completed family-directed plan with at least one family goal as it relates to any of the four goals of the Help Me Grow Home Visiting program within mandated timelines.
Assure that parents have been informed of their rights.
Assure that developmental screenings of the child(ren) have been completed. If a delay is suspected, then a referral for a developmental evaluation must be made with the consent of the parent.
If appropriate, referral and coordination of developmental evaluation and ongoing assessment in all five developmental domains, the family assessment, and hearing/vision/nutrition screenings to determine dual eligibility/re-determination for Part C services.

Facilitate and participate in the development, implementation, review, and monitoring of the Family Plan and their timelines.

Parent education using the Growing Great Kids curriculum.

How "Fill-In" Work Becomes Permanent

The Complete Picture: How Haven Really Works Understanding Client Matching (The Full Truth)
Client availability changes daily. Sometimes we have 5 clients ready immediately. If not, we're actively acquiring new clients and you can expect to get started within 1-2 weeks. Here's what determines your timeline:
Your Availability Matters:
6AM-8PM availability? First pick of everything
8AM-5PM availability? This is where 80% of care hours exist - you'll have good options
Morning-only or evening-only? Limited but possible
After 4PM only? You won't hit the 20-hour minimum (most clients need daytime care)
Your Preferences Impact Speed: The more specific your requirements (no pets, no smoking, limited drive radius, specific days only), the longer matching takes. This isn't punishment - it's math. We respect your boundaries completely, but fewer clients will match.
During Your Phone Interview, We'll Tell You:
"We have clients ready NOW" → Finish hiring fast, work fast
"Fill-in shifts available" → Start earning immediately while we match you permanently
"No current matches yet" → 1-2 week wait while we acquire new clients (we'll ask if that works for you)
Why Delays Happen (The Reality)
We're actively getting new clients daily, but it's not instant:
Clients need assessments
Insurance requires approval
Families need coordination meetings
Sometimes clients delay their own start dates
Bottom line: More flexibility = faster placement. Need guaranteed immediate hours? This role isn't structured for that. Can wait 1-3 weeks for the right match that respects your preferences? That's our specialty.
How "Fill-In" Work Becomes Permanent
Fill-in shifts are always available because caregivers call off unexpectedly. When you take a fill-in shift, two things can happen:
It's covering a call-off → One-time shift
It's an unstaffed visit slot → If the dynamic works between you and the client, that time slot becomes yours permanently
This is how you can build your permanent schedule faster during your first 2 weeks.

Week 1-2: Orientation Period

Our Care Coordination Team is your main support system:
Call them with ANY questions about clients, schedules, or concerns
They're available during business hours and for urgent matters
Open-door policy - we want to hear feedback, concerns, or problems
Client Match Not Working? Tell us immediately. We respect that sometimes personalities don't mesh, unexpected issues arise, or preferences change. We'd rather reassign you than lose you. Be completely open with our care coordination team.
No Growth Pathway: We're being honest - there's no ladder to climb here. This is a caregiving role, and it stays a caregiving role. What grows is your relationships with clients and your weekly paycheck through referral bonuses.

State Testing Requirement (Important Compliance Note)
All home health aides in Ohio must pass a state-compliant written and skills test at 80% or higher. This is a regulatory requirement, not our policy.
If you've already taken this test at another agency: Provide those results and you won't need to retest (as long as you scored 80%+)
If you haven't tested yet: We administer it as part of your hiring process
Client Types You'll Serve
Primarily elderly clients needing assistance with daily living
Some disabled clients requiring personal care support
Post-surgical or rehabilitation clients needing temporary assistance

You will NOT: Administer medications, perform wound care, or handle complex medical procedures
The Money Math (Full Breakdown)
Base Pay: $16/hour for all client care time AND travel time between clients
$100 Referral Bonus (per caregiver): When your referral completes 30 days with good standing
$100 Referral Bonus (per insurance client): When you refer a client who becomes a Haven client through their insurance
$75-$1,500 Referral Bonus (per private pay client): Amount varies based on how many weekly hours the client receives from Haven. More client hours = higher your bonus.
$19/hour Premium: When working with private pay clients (instead of standard $16/hour)
10% Holiday Premium: Earn $17.60/hour when you choose to work on holidays
