Responsibilities and Qualifications:
As a Registered Nurse RN you will educate patients and their caregivers on their conditions and the steps needed to ensure proper recovery.
Additional responsibilities of our Registered Nurse include:
Provide admission, case management, and follow-up skilled nursing visits for home health patients. • Confer with the physician and patient in developing a plan of care based on patient's medical condition and assessment findings at the time of the Start of Care and with every re-certification
Observe and assess patient's overall health and community status; train/educate patient /family on disease condition and treatment plan; render hands on care as needed and do management and evaluation when appropriate.
Coordinate with other disciplines involved in case to ensure that quality services are provided and there is continuity of care among providers
Update physician on patient status routinely and as needed when patient has a significant change in condition
Work with families and get them involved in the plan of care in order to achieve maximum resolution to patient needs/problems.
Create plan of care for home health aides and supervise them routinely to ensure that they are following the plan of care.
Perform discharge planning and provide patient with the necessary information they require in order for them to be able to continue to manage on their own after home health services are discontinued.
Required Skills
Associates of Science degree in Nursing
Valid RN license in state of employment
Valid driver's license
Able and willing to travel

Infusion, ostomy, wound vac, wound care, foley replacement and care, nephrostomy tube care, GT/JT/PEG care, medication reconciliation and instruction, and disease management teaching.

Job Description
Home Health Advantage is an established and growing home healthcare company that has been servicing patients in Chicago Land areas including, South, Southwest, North and Northwest Suburbs of Chicago (McHenry, Winnebago, Boone, Grundy, Kankakee, Lake, Grundy, Kankakee, Kane, Will, DuPage, and Cook Counties) seeking a high-energy, self-initiating, and caring RN clinician who enjoys onboarding new home health nurses, performing competency in the field and monitoring infection control practices. Must be an RN with strong clinical skills such as infusion (IVP, TPN, PICC and portacath care), wound care and wound vac, OASIS and comprehesnive assessment skills, ostomy care, urinary catheter insertions, clinical bag technique, pleurix care, nephrosotmy tube care, and PEG/GT feeding care. Your organizational skills and strong understanding of home health, geriatric care, and previous experience in home health will be required. Must be willing to travel to all geographic areas in order to support our clinical team members. Would consider a part time candidate with a minimum of 2 days per week but prefer full time.
Qualifications:
Valid state issued Registered Nurse License
A working knowledge of Medicare home health requirements
Strong clinical knowledge and background

As a Registered Nurse, here's a big-picture view of what you'll do:
Care for patients with a variety of illnesses, injuries and diseases in facilities such as clinics, medical offices, assisted living facilities, nursing homes, hospitals and schools
Work with a team of physicians and oversee LPNs, CNAs and aides providing patient care
Examine patients, conduct assessments, document condition and report changes to physician

QUALIFICATIONS:
1. Graduate of a state approved school of practical (vocational) nursing and currently licensed in the state(s) in which practicing.
2. Minimum of one (1) year experience in nursing, preferred.
3. Acceptance of philosophy and goals of this Agency.
4. Ability to exercise initiative and independent judgment.
RESPONSIBILITIES:
1. Understands and adheres to established policies and procedures.
2. Implements the nursing care plan for each patient.
3. Provides nursing services, treatments and diagnostic and preventive procedures as assigned.
4. Initiates preventive and rehabilitative nursing procedures as appropriate.
5. Observes signs and symptoms and reports to the physician and RN reactions to treatments, including drugs and changes in the patient's physical or emotional condition.
6. Teaches and counsels the patient and family/significant others regarding the nursing care needs and other related problems of the patient at home.
7. Evaluates with registered nurse the effectiveness of the LPN's/LVN's nursing service to the patient and family under the guidance of the registered nurse.
8. Maintains accurate and complete records of observations, treatments and care of patient.
9. Participates in medical record audit as assigned.
10. Attends staff meetings, patient care conferences and in-services as scheduled.
