Responsibilities:

This is a Full Time position at 40 hours per week located at the McHenry clinic currently, but come October 2026 it will be based at the Huntley clinic. Schedule is Monday through Friday, 8:30am-5:00pm. Surgery scheduling experience is preferred.

Description
This is a Part Time position at 20 hours per week located on the Lake Forest Hospital campus for the GI clinic. Schedule is as follows: Monday and Wednesday, 7:30am-4:00pm and Friday, 7:30am-12:00pm. Surgery scheduling experience is preferred.

The Patient Procedure Scheduler is primarily responsible for scheduling surgical and pre-operative procedures and maintaining schedules related to these procedures. Scheduling includes registration, verifying insurance information, obtaining pre-certifications, instructing patients on appropriate procedures. This role may also be cross-trained and utilized as a Patient Service Representative, thereby assuming the responsibilities associated with the check-in/out functions, or as a Patient Liaison, thereby assuming the responsibilities associated with provider templates and patient correspondence.

Collaborates and participates in the OR daily huddle with Pavilion Medical Director/Manager for final approval of the next day surgical schedule.

Occupational Risk for Exposure to TB:
There is an occupational risk for exposure to TB, as this position involves extensive community and patient contact. Employees in this role are required to comply with annual TB testing as part of workplace health and safety protocols. The only acceptable exceptions to this requirement are documented medical contraindications or religious objections. Position Summary:
We are seeking a dedicated Patient Care Coordinator who combines clinical/coordination competence with a strong interest in business development and referral partner engagement. This role will coordinate patient care and simultaneously develop relationships with referral sources (hospitals, nursing homes, physician offices, assisted living) to support our growth and service mission.
Key Responsibilities:
Coordinate patient admissions, transitions and ongoing care plans in collaboration with clinical teams.
Serve as a point of contact for patients, families, and internal care teams to ensure seamless service delivery.
Establish and maintain professional relationships with referral sources including hospitals, skilled nursing facilities, assisted living communities, physician practices and community organizations.
Educate referral sources about hospice services, company philosophy, eligibility criteria.
Assist with marketing/promotional initiatives and in-services to referral partners and community contacts.
Participate as needed in evening/weekend/after-hours admission calls and on-call coverage.
Qualifications:
Bachelor's degree preferred; combination of healthcare experience and business-development or sales experience also acceptable.
1-5 years of relevant experience in hospice/home health/skilled nursing facility environment, or clinical coordination + outreach.
Excellent verbal and written communication and relationship-building skills.
Ability to work independently and with a team
Knowledge of hospice care processes, eligibility, and home care environment preferred.
Comfort with CRM systems, tracking metrics, and generating growth reports.
Valid driver's license and reliable transportation (role includes field visits).
