The Rehabilitation Registered Nurse is responsible for assisting patients in attaining and maintaining Amergisum function due to chronic illness and/or a debilitating injury. The Rehabilitation Registered Nurse assists patients in the recovery process, as well as creating a recovery plan with the ultimate goal of achieving an independent lifestyle as much as possible.
Minimum Requirements:
Current Registered Nurse License for the state in which the nurse practices
One year prior professional nursing experience in a rehabilitation setting preferred
CRRN certification may be required per state/client contract
Current CPR if applicable
TB questionnaire, PPD or chest x-ray if applicable
Current Health certificate (per contract or state regulation)
Must meet all federal, state and local requirements
Must be at least 18 years of age

Work Environment
Nurse Corps Officers may serve at any one of more than 250 Navy and medical facilities around the globe, from Hawaii to Japan, Germany to Guam, and Washington, D.C., to Washington state. As a Navy Nurse, you may work at one of the highly acclaimed National Naval Medical Centers in Bethesda, MD, Portsmouth, VA, or San Diego, CA. Training & Advancement
Upon commissioning, Nurses who are new to the Navy are required to attend Officer Development School (ODS) in Newport, RI. ODS is a five-week program designed to introduce you to Navy culture. Nurses commissioned through a school ROTC program may not need to attend ODS.
Officer training is complete, you will learn the ins and outs of life as a Navy Nurse before receiving your first posting. Promotion opportunities are regularly available but competitive and based on performance.
The Navy provides Nurse Corps Officers the opportunity to specialize based on manning needs. Specialization fields include:
Nurse Anesthetists - Administer general and regional anesthesia, monitor patients receiving anesthesia, and assist in instructing medical trainees and other Officers
Primary Care Nurse Practitioners - Provide comprehensive health care and health maintenance for service members and their families
Medical-Surgical Nurses - Assess, plan and implement direct nursing care of patients on an assigned unit, and assume charge nurse responsibilities
Perioperative Nurses - Plan, implement and evaluate nursing care of surgery patients
Critical Care Nurses - Provide highly skilled, specialized nursing care to critical patients, including en route care, and train personnel in critical care nursing procedures

If you're a nursing student opting to serve full-time in the Navy, you may get up to $34,000 to help pay your way through nursing school through the Nurse Candidate Program (NCP).
For Graduate Students:
If you're a graduate student enrolled in a postgraduate nursing program in certain nursing specialties and opting to serve part-time in the Navy Reserve, you may qualify for up to $50,000 in nursing school loan repayment assistance.
For Practicing Nurses:
If you're a practicing nurse opting to serve part-time in the Navy Reserve, you may qualify for an immediate, one-time sign-on bonus of up to $30,000. Depending on your specialty, you may have the option of choosing between a sign-on bonus, nursing school loan repayment assistance or specialty pay.

Qualifications & Requirements
A degree from a four-year college or university is a minimum educational requirement to become a Commissioned Officer. You must also attend Officer Training. There may be exceptions to the degree requirements based on extensive service experience. Additional qualifications include:
U.S. Citizen between the ages of 18 and 41
Currently licensed and practicing nursing in the U.S. (new graduates must obtain a license within one year of beginning Active Duty service)
In good standing (as a student or graduate) with a CCNE-accredited U.S. education program granting a Bachelor of Science degree
Willing to serve a minimum of three years Active Duty
In good physical condition and able to pass a full medical examination
General qualifications may vary depending upon whether you're currently serving, whether you've served before or whether you've never served before.
Part-Time Opportunities
When serving part-time as a Navy Reserve Nurse, your duties are carried out during your scheduled drilling and training periods. During monthly drilling, Nurse Corps Officers in the Navy Reserve typically work at a location close to their homes.
For annual training, you may serve anywhere in the world, including locations in the U.S., at bases overseas, or in areas where humanitarian needs are great.

Most of what you do in the Navy Reserve is considered training. The basic Navy Reserve commitment involves training a minimum of one weekend a month (referred to as drilling) and two weeks a year (referred to as Annual Training) - or the equivalent of that.
Nurses in the Navy Reserve serve in an Officer role. Before receiving the ongoing professional training that comes with this job, initial training requirements must first be met.
For current or former Navy Officers (NAVET): Prior experience satisfies the initial leadership training requirement - so you will not need to go through Officer Training again.
For current or former Officers of military branches other than the Navy (OSVET), as well as for Officer candidates without prior military experience: Officer Candidates are required to attend Officer Development School (ODS) in Newport, RI. ODS is a five-week program that provides a comprehensive and intense introduction to the responsibilities of Navy Staff Corps Officers. Officers who previously held a commission in another United States Military Service, National Oceanic and Atmospheric Administration, Public Health Service, or United States Coast Guard are exempt from attending ODS or LDO/CWO Academy.
With flexible training options, Nurses in the Navy Reserve can comfortably balance civilian and military schedules. Responsibilities:
Provide overall direction and clinical management for home care services, ensuring compliance with all state and federal regulations.
Take, review, and convert home care referrals; collaborate with office staff to ensure cases are appropriately authorized for services.
Conduct and supervise client care, including assessments, recertifications, care plan development, coordination of services, discharge planning, and periodic evaluation of client care.
Receive, document, and assist in resolving complaints.
Oversee HomeHealth Aide requirements, including in-service training and competency evaluations.
Partner with the Branch Director on the Quality Assurance and Performance Improvement (QAPI) program.
Conduct recertification, supervisory, and other home visits as needed.
Support office staff with recruitment, new hire processing, and scheduling issues.
Collaborate with office staff to implement a recruitment program ensuring staff growth and adequate coverage of existing cases.
Requirements/Qualifications:
Current unencumbered RN License in the state of Colorado
At least 3 years of current nursing experience as an RN
At least 1 year of experience in homehealth care preferred
Previous management / supervisory experience is preferred
Knowledge of homehealth care regulations preferred
Bachelor's degree in nursing preferred

RESPONSIBILITIES
Completes an initial assessment of patient and family to determine home care needs. Provides a complete physical assessment and history of current and previous illness(es).
Regularly re-evaluates patient nursing needs.
Initiates the plan of care and makes necessary revisions as patient status and needs change.
Uses health assessment data to determine nursing diagnosis.
Develops a care plan, which establishes goals based on nursing diagnosis and incorporates therapeutic, preventive, and rehabilitative nursing actions. Includes the patient and the family in the planning process.
Initiates appropriate preventive and rehabilitative nursing procedures. Administers medications and treatments as prescribed by the physician. Provides direct patient care as defined in the State Nurse Practice Act.
Counsels the patient and family in meeting nursing and related needs.
Provides health care instructions to the patient as appropriate per assessment and plan of care.
Identifies discharge planning needs as part of the care plan development and implements prior to discharge of the patient.
Acts as Case Manager when assigned by Clinical Supervisor and assumes responsibility to coordinate patient care for assigned caseload.
Prepares clinical notes and updates the primary physician when necessary and at least every 60 days.
Communicates with the physician regarding the patient's needs and reports any changes in the patient's condition; obtains/receives physician's orders as required.
Communicates with community health related persons to coordinate the care plan.
Additional Duties:
Participates in on-call duties as defined by the on-call policy.
Ensures that arrangements for equipment and other necessary items and services are available.
Instructs, supervises and evaluates homehealth aide care provided every two (2) weeks.
QUALIFICATIONS:
Graduate of an accredited school of nursing.
One (1) to two (2) years of recent acute care experience in an institutional setting.
Current licensure in Colorado and CPR certification.
Bachelor's degree, with one (1) year of homehealth care experience preferred.
Management experience not required.
Responsible for supervising homehealth aides.
Excellent observation, verbal and written communication skills, problem solving skills, basic math skills; nursing skills per competency checklist.
Prolonged or considerable walking or standing. Able to lift, position or transfer patients. Able to lift supplies and equipment. Considerable reaching, stooping, bending, kneeling or crouching. Visual acuity and hearing to perform required nursing skills.
Licensed driver with automobile that is in good working order and insured in accordance with the organization requirements.

Our Weekend Baylor's are Registered Nurses (RN) and are responsible for partnering with the Clinical Team to administer and coordinate skilled nursing care to patients requiring home care services. Services are provided in accordance with physician's orders, under the direction and supervision of the Clinical Director, and in compliance with applicable laws, regulations, and policies of the organization.
*The Weekend Baylor RN's schedule is every weekend (Saturday & Sunday) from 7:00AM - 8:00PM, as well four (4) hours on Mondays to conduct follow-ups with providers and/or complete documentation. This is a full-time, exempt (salary) position.
WHAT YOU'LL DO
Clinical Care:
Provides direct care and case management of assigned patients, including initial assessment and development of plan of care, as well as oversight and administration of prescribed medications and treatments. Maintains and updates schedules of all disciplines in patient's home.
Consistently assesses and evaluates patient's status and goals and revises the plan of care as necessary. Communicates adjustments to plan of care to all team members in the appropriate time frame to support consistent delivery of care. Complete authorization modifications in a timely manner, where needed.
Initiates and applies appropriate preventative, therapeutic, and rehabilitative procedures and maintains communication regarding patient care with the physician, supervisory personnel, and other professional staff involved in patient care.
Consistently monitors and manages patient medications; maintains an accurate medication profile; coordinates prescription refills with physicians and pharmacies and picks up medication refills as needed.
Teaches the patient and family/caregiver self-care techniques as appropriate. Provides medication, diet, and other instructions as ordered by the physician. Recognizes and utilizes opportunities for health counseling with patients and families/caregivers.
Provides and maintains a safe environment for the patient. Reports unsafe conditions, complaints, and incidents on behalf of patient as appropriate.
Observes and supervises HomeHealth Aide & LPN to evaluate performance and quality of care.
Accepts responsibility of assignment to perform a specialized procedure, such as IV therapy, when qualified with appropriate training and proven competency in the delivery of the therapy.
Uses supplies and equipment effectively and efficiently. Maintains equipment per agency policy.
Fulfills the obligation of assigned patient case load including tracking of patient status when transferred, following up with other healthcare facilities providing care to patients, resuming
care of patients when appropriate.
Adheres to HIPAA laws and maintains patient confidentiality always.
Other duties assigned by the case manager/clinical management, i.e., performing field supervisions of care team members as required by agency policies.
Documentation:
Completes and submits accurate, timely clinical notes including all OASIS documents, visit authorization requests, physician orders, and all other clinical documentation regarding patient's condition and care provided in accordance with Innovive company policies.
Communication and Meetings:
Communicates with physicians and other agencies providing nursing or related services on a consistent basis to ensure continuity of care and implementation of a comprehensive care plan. Acts as part of the interdisciplinary care team.
Participates in scheduled clinical and internal/external case management meetings. Attends vendor and referral meetings as needed and assigned, or as patient conditions require. Documents all meetings and outcomes for patient medical record.
Coordinates with pharmacies, insurance companies and other service providers as needed to facilitate timely and appropriate provision of patient care.
Performs other duties as assigned.
WHAT YOU HAVE:
Graduate of an accredited school of professional nursing
Is currently licensed as an RN through the State Board of Nursing and meets one of the following criteria:
RN with a bachelor's degree in nursing and one year of related working experience
RN with a diploma or associate's degree with two years related work experience
Has passed the National Council Licensure Examination (NCLEX)
Complies with accepted professional standards and principles
Must be a licensed driver with an automobile that is insured in accordance with state and/or organization requirements and is in good working order, or other reliable means of transportation
Possesses and maintains CPR certification
Has presented a pre-employment physician's health clearance including negative TB skin test and/or CXR and other tests as required by organization policy
Has satisfactory references from nursing school, previous (or current) employers and/or professional peers
Preferred Education and Experience:
Prior experience caring for mental health patients
1-2 years' experience in a homehealth environment
