The Registered Nurse creates & develops patient care plans to direct the safe and appropriate administration of medication in the home or alternate site setting. Working with Home Infusion staff the Registered Nurse helps identify patients capable of successful self-administration, educates patients & caregivers on proper administration techniques & methods, coordinates discharge activities, administers or supervises administration of medication, provides IV site dressing care and phlebotomy services as ordered.
Job Description:
Primary Responsibilities:
1. Practices in accordance with the state and federal laws, rules, and regulations, including but not limited to Massachusetts Board of Registration in Nursing, Department of Public Health, the Food and Drug Administration, the Drug Enforcement Administration, and the United States Pharmacopeia.
2. Assesses patient needs and capabilities for acceptance onto services for ordered therapies through physical assessment, chart review, patient interview.
3. Provides education with patient & caregiver as a condition of acceptance onto home infusion services (in person, via prerecorded videos or teleconference).
4. Collects necessary authorizations, paperwork, signatures when needed to support patient billing functions.
5. Effectively communicates with all members of patient discharge team (hospital based & pharmacy based).
6. Able to effectively manage patient census & demonstrate flexibility in scheduling to meet time sensitive patient needs.
7. Adheres to policies & procedures for medication handling, preparation, administration, disposal.
8. Advocates patient needs to prescribers or other clinicians through written or verbal communication.
9. Create patient specific care plan, documents patient progress towards goals.
10. Maintains accurate listing of all medications, supplements & diagnoses patient has.
11. Create discharge plan for patient upon completions of therapy as ordered, or if abrupt discontinuation is required.
13. Participates in continuous quality control exercises, policy & procedure review, performance improvement tasks.
14. Take part in clinical on-call rotation for after-hours patient support.
Required Qualifications:
1. Current licensure from the Massachusetts Board of Registration in Nursing.
2. Flexibility with hours worked to provide support evenings/weekends/holidays.
3. Current BLS certification
4. 2 years of Infusion related experience required.
5. Valid Driver's license & auto insurance
Preferred Qualifications:
1. CRNI certification
2. Current ACLS certification

The Director of the Behavioral Health (BH) Transfer Center provides senior-level leadership for a centralized, 24/7 behavioral health access and admissions operation supporting multiple facilities across several states. This role is accountable for operational performance, clinical integrity, regulatory compliance, and relationship management across a multi-state healthcare system.
The ideal candidate is an experienced Behavioral Health leader at the Director level, highly operationally focused, data-driven, and comfortable leading remote clinical and non-clinical teams. This leader serves as the subject matter expert for BH transfer operations and works closely with executive leadership, hospital CEOs, and clinical teams to ensure safe, timely, and compliant patient transfers.

The Behavioral Health Transfer Center functions as a centralized, 24/7 hub responsible for the timely and appropriate placement of patients requiring acute behavioral health care. The center streamlines the transfer process by conducting clinical assessments, confirming bed availability, and coordinating transportation to ensure patients are placed in the appropriate level of care without delay across 18 facilities.
Staffed by a multidisciplinary team of 31 experienced behavioral health professionals-including licensed clinicians, nurses, EMTs, and case managers-the Transfer Center serves as the primary liaison for referring facilities and manages more than 6,000 patient transfers each month. The mission is to reduce wait times, eliminate gaps in care, and ensure safe, seamless transitions for individuals experiencing mental health or substance use crises. This role offers the opportunity to work remotely within a high-volume, fast-paced, and purpose-driven environment focused on clinical excellence and operational efficiency.
Key Responsibilities
Provide overall leadership and accountability for Behavioral Health Transfer Center operations, including remote clinical (RNs/licensed clinicians) and non-clinical staff.
Drive operational excellence through metrics, KPIs, service-level management, and continuous process improvement.
Ensure appropriate clinical decision-making and level-of-care recommendations for behavioral health patients across all age populations.
Partner with hospital CEOs, Directors of Nursing, Directors of Assessment, and executive leadership to ensure consistent admissions and transfer processes across all facilities.

Responsibilities
The position is 100% remote with up to 75% travel. The Surveyor will serve as a team member or team leader on various types of surveys (i.e. re-certification, comparative, complaint investigation, and revisits) for long-term care and non-long-term care surveys, which can include; ambulatory surgical centers (ASC), Intermediate Care Facilities for Individuals with Intellectual Disabilities (ICFs/IID), end-stage renal disease (ESRD), Psychiatric Residential Treatment Facility (PRTF), hospital, critical access hospital, and hospice facilities. The responsibility of the Surveyor is to apply approved survey protocols for conducting on-site surveys. Specific activities include the following:
Conduct on-site surveys of Medicare-Medicaid certified health care facilities to determine compliance with Federal regulations.
Surveys require preparations such as off-site preparation, tour of the facility, observation of care, record review, interviews of staff and families, etc. Determine if care, treatment, and services are consistent with Federal requirements.
Evaluate many aspects of the healthcare facility system from the quality of care to the consumers' rights and physical plant.
Communicate the non-compliance issues with key personnel throughout the site review and during the exit conference to assure an understanding of the deficiencies cited.
Determine if a plan of correction is acceptable.
Conduct follow-up visits and monitoring surveys to ascertain if facilities' plans of correction to resolve the deficient practice have been implemented.
Training: Assist with facility and bureau training. Participate in work groups as needed
Requirements
Must have an Associate or Bachelor's degree in nursing
Must have successfully completed CMS's Long-Term Care Basic Training and passed the Surveyor Minimum Qualifications Test (SMQT).
Must have At least two (2) years of recent experience working as a surveyor accrediting or certifying facilities that serve the residents of long-term care facilities.
Must maintain current licensure to practice as a Registered Nurse (RN).
Demonstrated a history of independent decision-making skills to direct and effectively manage the survey process.
Ability to set priorities independently and collectively in performing survey tasks.

The Role

Job Summary
The Patient Care Coordinator is responsible for the day-to-day management of the cardiac remote monitoring program for specific cardiology clinic customers of CRMS by DocGo. Duties include reviewing daily alerts and transmissions of all patients enrolled in remote monitoring websites, documentation, interpretation, triaging and communicating with patients, team lead, clinical staff and the physicians to ensure appropriate coordination of care. Over the last three years, we've experienced tremendous growth and are looking for a Registered Nurse excited to help lead us into the future. As an RN in our new team, you will be the hub of our care team structure and model of care. You will collaborate with our PCPs, Medical Directors, Technology Team, partners, and of course our patients as we work to build a company that delivers the best quality geriatric care available. What You'll Do:
Collaborate with a dedicated team of Primary Care Providers (PCPs), Medical Directors, and Medical Assistants supporting a patient panel.
Triage inbound communications, including calls, faxes, emails and instant messages from teammates, patients, and partners. You will aim for "one touch" call resolution, escalate to the PCPs when necessary, and route sub-clinical tasks to our Medical Assistants.
Act as a support system and escalation point for LVNs/LPNs/MAs within the team, enhancing our team's efficiency and effectiveness.
Engage in comprehensive chart preparation before patient appointments.
Oversee post-appointment processes, including diagnostic orders, referrals, medication orders, and provide crucial patient education on care plan adjustments when needed.
Participate in daily team huddles, offering guidance and fostering efficient teamwork in support of patients & partners.
Conduct virtual patient assessments, developing personalized care plans and monitoring health outcomes.
Promote patient health through education on preventive care and self-management strategies consistent with the patient's care plan and wishes.
Participate in crisis intervention and palliative care discussions, focusing on patient comfort and quality of life.
Engage in ongoing quality improvement initiatives and contribute to research efforts aimed at enhancing geriatric care.
What We're Looking For:
A current unrestricted RN license in CA (preferably AZ and NV as well) with at least 2 years of clinical experience, preferably in geriatrics or related fields.
Strong experience in remote roles, showcasing proficiency with technology and digital communication platforms.
A track record of high accountability, exceptional communication, and the ability to thrive in a fast-paced environment.
Experience with geriatric patients, particularly in settings like senior living communities or skilled nursing facilities.
Familiarity with chronic care management and a solid understanding of geriatric-related diseases and treatments.
A collaborative mindset, valuing the contributions of a multidisciplinary team and committed to improving patient outcomes.

How you'll change lives
As a Registered Nurse (RN) at US Renal Care, you'll be an integral part of a cross-functional team, working under the supervision of a Charge Nurse and assisting and supervising Patient Care Technicians (PCTs) to care for patients living with kidney disease.
What you'll be doing
Patient care. You'll work directly with patients to provide safe, comfortable, and high-quality dialysis treatment. Responsibilities include completing and documenting physical assessments before and after treatments and monitoring patients during treatment. You'll ensure quality renal care by completing monthly reviews of patient medication profiles and administering medications according to physician orders.
Teamwork. You'll be a critical member of an interdisciplinary team to provide all required patient assessment and care planning activities. You'll also make sure center staffing requirements are met at all times under the direction of the Administrator and according to state regulations for staffing ratios.
Safety & Quality. Working under the supervision of a Charge Nurse, you'll adhere to all company policies and procedures and state and federal laws and regulations. You'll participate in all required staff meetings and continuing education offerings.
