POSITION QUALIFICATIONS
The Clinical Liaison should be a Licensed Vocational Nurse, Registered Nurse or Respiratory Therapist. A Bachelor of Science in Nursing (BSN) is preferred. Complete Minimum Data Sets for Prospective Payment (Medicare and HMO) residents as per CMS Assessment Schedule. 5, 14, 30, 60, 90 Day and significant change as appropriate.
Complete CATS ((Care Area Triggers), CAAs (Care Area Assessments) and care plans within 21 days of admission and for all comprehensive assessments (I.e. significant change, annual).
Ensure all areas that triggered on the CAA are included on the resident's plan of care, using CMS Minimum Data Set guideline protocols to determine whether to proceed or not to proceed regarding care plan issues.
Understand billing for the MDS process and coordinate this with the Business Office Manager, as needed.

+ Deliver and document homehealthnursing interventions as agreed upon in participant care plans, including but not limited to maintaining a healthy and safe environment, promptly responding to physician orders, and accurately administering medications and performing ordered tests and treatments
+ Instruct participants, their families, and caregivers regarding the disease process, self-care techniques, and prevention strategies related to their medical conditions
+ Track and monitor homehealth scheduling in collaboration with the HomeHealth Manager or scheduling point of contact
+ Collaborate with the Physician and other IDT members to assist in smooth care transitions between settings (hospitals, skilled nursing facilities, home, etc.)
**Job requirements include:**
+ Graduate of an accredited school of nursing
+ Unencumbered RN License required
+ Minimum of one (1) year of nursing experience in a clinical setting with a frail or elderly population; homehealth agency experience preferred
+ Reliable means of transportation
+ Must obtain CPR and First Aid certification within 180 days of hire and maintain certification thereafter; CPR and First Aid certification upon hire preferred
