The Philadelphia Model: Addressing serious mental illness (SMI) in long-term care (LTC) will provide highly structured therapeutic residential behavioral healthcare and treatment for frail adults who are eligible for skilled nursing level of care who also have serious mental illness and who have reached maximum benefit from the mental health resources available elsewhere in the community or hospital. The program philosophy is to create a welcoming, safe, and supportive environment where frail older adults with SMI that may include other complex behavioral health needs and/or prior criminal history, and need long-term skilled nursing services- can receive the care and services needed to address their holistic needs free of stigma. A multidisciplinary team will provide both long-term care and behavioral health services in this integrated program.

GENERAL OVERVIEW OF KEY ROLES & RESPONSIBILITIES:
Under the direction and supervision of the Program Director and the Compliance Regulatory Nurse, in collaboration with the Director of Nursing, the Registered Nurse will demonstrate a commitment in the assessment and management of residents with psychiatric, mental health and/or substance abuse diagnosis. The Registered Nurse assesses, plans, implements, and evaluates behavioral healthnursing care provided to residents and their families within the context of the program's philosophy and goals. Will be expected to apply all nursing service policies when rendering nursing care and providing health education to patients and families.
Recognizing that nursing care is one portion of health care, the Registered Nurse is expected to function as a member of a multidisciplinary team, interacting collegially with staff, physicians, consultants, and interdepartmental personnel.
ESSENTIAL & CORE FUNCTIONS:
•Practices psychiatric-mental healthnursing in accordance with the philosophies, goals, and objectives of both Behavioral Medicine and the assigned unit.
•Provides quality behavioral healthnursing care to all residents as defined by ANA Standards of Psychiatric Mental Health Clinical Nursing Practice, ANA Standards of Professional Performance, and long-term care regulations. Models ANA Standards.
•Intervene with residents by providing health promotion and health maintenance, intake screening and evaluation, milieu therapy, therapeutic use of self, self-care activities and bio-psychosocial interventions, health education, crisis intervention, individual and group counseling, and resident advocacy.
•Contribute to the development and evaluation of a goal-oriented care plan continuously based on nursing assessments and nursing interventions with residents and resident representatives.
•Completes daily chart reviews.
•Reviews Care Plan and updates information as needed.
•Implements and delegates behavioral nursing interventions that are consistent with the care plan and the medical regime.
•Implements Nursing policies and procedures.
•Supervises and directs unit staff, designating and reviewing assignments of Behavioral Health Techs, on appropriate assignment sheet.
•Assumes the charge role when designated.
•Participates in Performance improvement and Quality Improvement activities.

•Acquires and maintains current knowledge of psychiatric-mental healthnursing practice.

EDUCATION & EXPERIENCE:
1.Graduate from a NLN accredited school of professional nursing, required. Baccalaureate degree, preferred.
2.Current Commonwealth of Pennsylvania Registered Nurse licensure and CPR Certification required.
3.Three (3) - Five (S) years of professional nursing practice in behavioral health required.
4.Direct Psychiatric or addictions nursing experience, desired.

Responsibilities:
Under the direction of the Clinical Research Study Site Leader, this resource will be responsible for supporting mostly Medical Oncology interventional studies (phase I through phase III) - Solid Tumor, Lung, Head and Neck, Melanoma and Neurology. Studies are mostly drug and Biologic (very few device studies). Studies often involve radiation and surgery.
Proactively manages, coordinates, and delivers high-quality, compassionate hands-on competent oncology nursing care and coordinates with the assigned clinical team, investigators, and ancillary departments (such as pathology, radiology, clinical laboratory, surgery, infusion services) as per protocol requirements and ensures adherence to the treatment plan and Good Clinical Practice (GCP) guidelines.
Assist investigators (physicians) to prepare and implement new clinical trials, screens and enrolls study participants, and provides protocol-related clinical management to those participants while on study.
This is NOT a typical nursing role that is task-based, and tasks dictated by physicians. The Nurse has to be a critical thinker, to prioritize, and be "appropriately assertive" and comfortable pushing back on physicians to "save them from themselves" - looking at study protocol and see are orders all correct as it relates to medical record, is anything missing, does the patient have what they need?
The role will follow group assignments. The Physicians have designated clinic days. These are the days the Nurses will see more patients. On non-clinic days, the Nurse will see less patients and perform administrative tasks including patient documentation, patient phone calls, making sure orders are correct (ensuring nothing is missing) and perform project management related activities.
Collaborate with Protocol Activation office to initiate and activate all new clinical trial protocols assigned utilizing Oncore Study-start up task list.
Reviews patient's charts and medical history to confirm protocol eligibility and obtains source documents (i.e., medical record documentation) as needed.
Demonstrate basic knowledge to accomplish core tasks such as recording and reporting Serious Adverse Events (SAE) in a timely manner, accessing protocol attachments via OnCoreTM, enter charts into Medical Records Database, formatting, and printing patient study calendars, etc.
Required Skills:
Graduate from an accredited school of nursing with a license to practice in the State of New Jersey.
Bachelor of Science in Nursing (BSN) Degree.
2 or more years of nursing experience working in a hospital setting.
1 or more years of experience working in an Oncology specialty area in a hospital or clinic setting.
Experience working in a clinical research/trial participant facing role is a big plus (but this is not a requirement - resource will be trained in clinical research).
Must be a critical thinker, prioritize, and be "appropriately assertive" and comfortable pushing back on physicians stressing the importance of accurate patient information relative to the study protocol.
Communication Skills: The Nurse is the point person for communication with other departments. Able to communicate and articulate, translate complicated information back to a patient in a way they can understand (very important part of this job).
Document Management Skills: comfortable editing in Microsoft Word, basic reporting in Excel, converting PDFs or Word Documents in Adobe. Comfortable with PowerPoint to provide education with other Nurses. MS Office, Teams, and SharePoint.
Detail oriented but not to the point of stagnation.
Must be able to thrive in a fast-paced, Academic, diverse environment.

About the Role
At Complete Homecare, we provide 1:1 care in the home, 1:1 staffing at school, school nurse staffing. We are available to assist you with your clinical questions 24/7. We are currently hiring in Mercer, Burlington, Camden, Gloucester, Atlantic, Ocean, Cumberland and Salem counties. Seeking a minimum of one year homecare experience. The ADULT MEDICAL SURGICAL REGISTERED NURSE (RN) is a healthcare provider who through education and experience possesses a distinct body of knowledge and skills relative to the care of the adult patient across the lifespan who is experiencing general medical conditions or general surgical interventions. EDUCATION
Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing.
LICENSURE
Current and unrestricted RN nursing license in the state of practice.
EXPERIENCE
One year of experience as an RN in an Adult Medical Surgical Unit in the last three years.
CREDENTIALS
Current CPR at least to the BLS level; and other health and screening tests as required by specific facilities and/or regulatory agencies.
ENVIRONMENTAL WORKING CONDITIONS
Various client facility settings; possible exposure to blood, bodily fluids, and other potentially infectious materials.
REPRESENTATIVE DUTIES AND RESPONSIBILITIES
Complies with ATC policies/procedures.
Complies with client facility nursing policies/procedures.
Conducts individualized patient assessment, prioritizing collection of data based on individual needs and condition.
Conducts ongoing nursing assessment as dictated by the patient's condition and in accordance with client facility's protocols.
Collaborates with other team members in the development, implementation, and evaluation of an individualized plan of care.
Performs appropriate treatments and therapies as ordered by physician in an appropriate and timely fashion.
Provides individualized patient and family teaching.
Documents patient assessments, findings, and psychosocial responses to nursing interventions
Initiates emergency measures according to adult and client resuscitation protocols.
Maintains confidentiality related to patient, family, client facility and staff in accordance with the Health Insurance Portability and Accountability Act (HIPAA).
Provides patient care in a non-judgmental, non-discriminatory manner that considers cultural diversity and age appropriateness so that autonomy, rights, and dignity are preserved.
Advocates patient rights with respect to advance directives and organ donation.
Reports patient condition to appropriate personnel during each shift, as needed based on individualized patient assessment.
Maintains competency by participating in continuing education programs and meets state specific requirements.
Complies with accepted ethical conduct and professional Standards of Nursing Practice as set forth by the American Nurses Association or equivalent national organization.
Demonstrates ability to delegate effectively and appropriately.
