PACE is a comprehensive health care program that provides an integrated approach with medical and supportive services designed to help individuals meet their health care need in the community. With a robust interdisciplinary team of 11 each PACE participant received wrap-around care designed to meet each person's unique needs.
The Position:
The Home Care Manager for PACE supervises the PACE home care staff to ensure regulatory compliance and quality, while maintaining an optimal level of independence for participants. Provides technical, organizational, and interpersonal skills necessary to coordinate the scheduling, communication, and documentation of all aspects of the plan efficiently and effectively for home care. Promotes and maintains health of participants in the community setting through teaching, counseling, and appropriate preventative and or skilled services. Responsible for assessment of health and home safety needs, contributing to the plan of care, initial implementation of nursing care plans and providing nursing care, as well as periodic re-evaluation of individual and family needs. Demonstrates the knowledge and skills necessary to assess, plan care for and provide service to frail elderly participants according to assigned responsibilities and PACE Program standards.
Requirements:
- Registered Nurse with current Colorado licensure; graduate from an accredited school of nursing
- Bachelor's degree preferred
- At least 3 years of recent experience in a home care setting preferred
- Management experience preferred
- Colorado driver's license, adequate automobile insurance and unlimited access to reliable transportation
Pay & Benefits:
- Hiring range starting at $33.33-$40.58 per hour
