The Position:
The Home Care Manager for PACE supervises the PACE home care staff to ensure regulatory compliance and quality, while maintaining an optimal level of independence for participants. Provides technical, organizational, and interpersonal skills necessary to coordinate the scheduling, communication, and documentation of all aspects of the plan efficiently and effectively for home care. Promotes and maintains health of participants in the community setting through teaching, counseling, and appropriate preventative and or skilled services. JOB SUMMARY:

EDUCATION OR TRAINING REQUIREMENT: High School diploma or equivalent.
EXPERIENCE REQUIREMENT: Minimum 2 years experience working with individuals with disabilities and/or
related field; 1 year of supervisory experience preferred; Equivalent field experience may be substituted for supervisory experience with Executive Director’s approval.

SUPERVISORY RESPONSIBILITY: Supervises in-home, direct care staff
ESSENTIAL JOB DUTIES AND RESPONSIBILITIES:
I. Training/Program Responsibilities
A. Implements activities as coordinated by Consumer Support team which reflect training objectives in each client’s
individual service plan.
B. Engages residents in activities of choice, which reinforces ISP goals.

I. Coordinate residents’ outings for community involvement and shopping for their personal supplies.

II. Responsibilities Relating to Health and Safety

C. Conducts evacuation drills, both fire and severe weather; records results of each drill.
D. Follow program plan for meeting all potential emergencies and disasters, such as medical, fire, severe weather

IV. Administrative Responsibilities

B. Complies with the organization policies and procedures governing the use of behavior management programs for
controlling maladaptive or problem behavior.
C. Complies with policies and procedures to ensure that confidentiality requirements are upheld.
D. Supports and protects the fundamental human, civil, constitutional and statutory rights of clients and families as
defined by agency.
E. Demonstrates competence in handling medical emergencies in accordance with UCP policies and procedures.
F. Maintains accurate and complete records of all resident’s personal spending monies in accordance with rules and
regulations; completes monthly reconciliation and submits report to Regional Director of Community Living Services

VI. Physical Health Service Responsibilities

certification, physical, etc.
