Assists with the development, writing, and revision of the plan of care in collaboration with the Agency staff and the physician Consults with the Nurse Case Manager, Director of Professional Services, VP of Clinical Services, or designee regarding discharge planning with the client and caregiver. Notify the Agency of imminent discharge so proper notification can be mailed to the physician. Maintains appropriate clinical records in a prompt and comprehensive manner, documenting client limitations and progress. Develops and instructs Agency staff and caregivers in maintenance therapy programs as approved by the physician. Assumes responsibility for equipment signed out for client use. Evaluates designated OASIS items upon discharge from the services for coordination of care among other disciplines. Qualifications Holds a current Physical Therapy License. Has graduated from an approved physical therapy program. One-year clinical experience. Has current CPR certification. Has excellent communication, organizational, and human relations skills. Is able to function with minimal supervision. Is able to manage multiple projects/changing priorities. Is able to employ critical thinking skills

Completes initial home assessments of clients, caregivers and home environments for admission to and continued services to determine eligibility and needs according to DMAS and/or Agency regulations Prepares written plans of care for administration of care to the client. Conducts supervisory visits to the client's home to ensure quality and appropriateness of services as well as the health, safety and welfare of the client. Provides any necessary supervision to the aide and client and records all significant documentation in the client's file accordingly. Observes, evaluates and documents the adequacy and appropriateness of care based upon the recipient's functional status and medical/social needs. Submits the needed documentation requesting increases/decreases in hours of care provided to the member. Discusses the client/family's satisfaction of services during scheduled supervisory visits. Reviews aide records during the supervisory visits in accordance with regulatory and organizational policies. Maintains timely/comprehensive clinical records, reports and DMAS forms per agency guidelines. Corrects documentation as requested by Clinical Supervisor. Coordinates care for clients with staffing coordinators and other outside agencies/organizations as needed. Arranges necessary referrals to physicians, skilled services and other community agencies in collaboration with other staff disciplines as needed. Maintains ongoing communication with Paraprofessional staff regarding changes in client conditions or plan of care and evaluates for effectiveness. Serves as advocate for clients/caregivers and reports any issues/concerns or needed changes to the Agency and/or support staff. Participates in clinical meetings and completes in-services as directed. Maintains schedule for required visits and completes these visits according to regulatory and organizational criteria. Submits monthly Level of Care reviews per DMAS guidelines.
