Maat Program Overview The Maat Program is HCNs Afri-centric clinical and community wellness initiative designed to support children, youth, and families in San Francisco with culturally grounded mental health services, case coordination, healing-centered engagement, and community connection. Rooted in the principles of balance, truth, order, reciprocity, and harmony, Maat integrates Black cultural knowledge, trauma-informed care, and community-defined best practices to honor the lived experiences of families impacted by generational trauma, racism, displacement, and poverty. The program provides clinical servicesincluding therapy, family work, and emotional wellness supportdelivered in community settings, homes, schools, and transitional or permanent supportive housing sites. Maat staff work closely with families to address emotional, behavioral, and relational challenges, while strengthening protective factors, enhancing resilience, and supporting overall wellbeing. With an emphasis on trust, cultural affirmation, and genuine partnership, the Maat Program uplifts families through holistic healing, community connection, and resource access. About the Position The Program Manager for the Maat Program provides strategic, clinical, and operational leadership for this community-based mental health program. This role oversees program implementation, supervises staff, ensures compliance with Medi-Cal and EPSDT requirements, and maintains high-quality clinical services rooted in community connection, cultural humility, and healing justice. The Program Manager works closely with HCN leadership, community partners, and funders to ensure that services are delivered effectively, ethically, and in alignment with contract obligations and culturally responsive practices. Key Responsibilities Program Leadership & Management Provide day-to-day program management to ensure the delivery of culturally responsive, high-quality services to Black/African American children, youth, and families. Develop, refine, and oversee program workflows, policies, and procedures in alignment with HCNs mission and the Maat Afri-centric framework. Manage program deliverables, budgets, and reporting associated with government contracts and grant-funded services. Support with, collaborate with HCN departments (Finance, Evaluation, Compliance, HR) to ensure accurate billing, quality data reporting, and contract compliance. Strengthen and expand community partnerships to increase access, visibility, and impact of Maat services. Provide support to community-based clinicians by assisting with group facilitation, and, as needed, independently lead or conduct group sessions at various community sites. Clinical Oversight & Supervision Provide clinical guidance and support to therapists, clinicians, and community-based staff supplemental to established individual and group supervision that clinicians currently receive. Support high-quality clinical documentation aligned with Medi-Cal and EPSDT standards, including timely progress notes, treatment plans, assessments, and billing. Promote culturally grounded, trauma-informed, and strengths-based practices rooted in Black mental health and community wellness. Staff Support & Development Supervise and support a team working independently across San Francisco in community-based settings. Monitor level of effort (productivity), caseloads, and service delivery to ensure balance between program expectations and staff wellbeing. Address burnout and morale through reflective supervision, coaching, and structured staff support systems. Facilitate staff training, mentorship, and professional development opportunities. Community Engagement & Collaboration Build and maintain strong relationships with families, schools, community partners, housing providers, and culturally specific organizations. Represent the Maat Program at meetings, collaborative spaces, and stakeholder convenings. Collaborate with HCN leadership to respond to emerging community needs and implement culturally relevant services. Compliance, Billing & Quality Assurance Ensure adherence to Medi-Cal, EPSDT, BBS, and San Francisco Department of Public Health documentation and service standards. Monitor program performance indicators, evaluation data, and audit readiness. Ensure accurate, timely, and ethical billing practices. Implement continuous quality improvement strategies based on program data and feedback. If eligible, when necessary, will assume and maintain an individual caseload in alignment with programmatic needs and performance expectations. Qualifications Required Minimum 3 years of experience within a community mental health setting ( case management, mental health, education setting, clinics, shelters, in patient and or outpatient facilities etc) . Experience with Medi-Cal and EPSDT billing, documentation, and service standards. Demonstrated experience supervising clinical or community-based staff who work independently in field settings. Ability to monitor and support level of effort (productivity), caseload management, and staff wellness. Deep experience working with BIPOC families, particularly Black/African American children and families impacted by trauma, poverty, racism, and community violence. Commitment to culturally responsive, trauma-informed, healing-centered, and Afri-centric approaches. Preferred Qualified to provide clinical supervision (meeting BBS supervisory requirements). Licensed clinician in California (LCSW, LMFT, LPCC, or Psychologist) in good standing. Knowledge of government contracts and funder requirements within nonprofit mental health programs. Experience using Afri-centric, Indigenous, or liberation-based healing frameworks. Familiarity with San Franciscos behavioral health systems, including SFDPH, CBO partnerships, and school- or community-based service models. Experience with program evaluation, outcomes tracking, and continuous quality improvement. Work Environment & Physical Requirements Ability to work in both office and community settings across San Francisco. Frequent travel to schools, community sites, family homes, and partner organizations. Occasional evening or weekend hours for community events or family needs. JABALI YOUTH SUD OUTPATIENT PROGRAM Jabali Youth SUD Outpatient Program is a culturally responsive and family-centered substance use treatment program for youth in San Francisco. Utilizing Evidence-based and locally innovative interventions, this program will provide co-occurring or SUD standalone services for youth who are at risk or struggling with drug, alcohol, and other substances. Jabali offers a continuum of services depending on the level of care needed: Early Intervention for youth who are at risk of developing a Substance Use Disorder (SUD). Outpatient Level 1 for youth who are actively using substances and meet criteria for SUD diagnosis, but who present with NO acute intoxication and/or withdrawal potential. Intensive Outpatient for youth who are actively using substances, meet criteria for SUD diagnosis, and who present with acute intoxication and/or withdrawal potential. This program serves all ethnicities and populations within San Francisco with focused expertise to meet the needs of youth under the age of 18 years old who are Medi-Cal beneficiaries. POSITION SUMMARY: The Substance Use Counselor, also known as Alcohol & Other Drugs (AOD) Counselor, will be working alongside and in partnership with the Programs Licensed Healing Arts Practitioners (LPHA) and Case Managers. The Substance Use Counselor is the primary direct service provider in the program. Their direct service tasks include individual and group counseling, contributing to assessment and treatment planning, screening referrals, case management and care coordination, patient education, and collateral services. Additionally, they may support with outreach and engagement activities The duties of the AOD Counselor will be primarily overseen by the LPHA. DUTIES AND RESPONSIBILITIES: Intervention and Counseling Offer one-on-one counseling or group counseling sessions for clients dealing with substance use issues. Offer collateral support to family and significant persons, as directed by LPHA. Provide emotional support, coping strategies, and behavioral management techniques. Support clients in addressing personal or social problems that may impact their substance use. Assessment and Treatment Planning Contributes to clinical assessment by gathering information as directed by the LPHA, who will be leading the intake and assessment process. May contribute to screening and client intakes as appropriate. Contributes to the development of strength-based Treatment Plans of Care for each client and consistently monitor progress. Will continuously re-assess and monitor clients well-being and functioning and collaborate with LPHA in updating assessment and treatment plan as needed. Client Education: Educate clients and their families about substance use disorders, mental health disorders, treatment options, and coping strategies. Promote wellness and help clients develop long-term strategies for managing their substance use and mental health. Provide client education in individual or group settings on a topic collaboratively designed with LPHA under the direction of the Clinical Director. Case Management and Care Coordination: As directed by LPHA, coordinate with other providers to offer holistic care. Facilitate referrals through effective networking. In addition to external networking, will provide information to internal programs that would like to refer clients to SUD treatment services. In collaboration with LPHA, will consult on potential internal referrals maintaining clients confidentiality and privacy. Professional Development: Engage in continuing education, professional training, and supervision to stay current with best practices and new substance use and mental health treatments. Participate in workshops, seminars, and conferences related to substance use and mental health care. REQUIRED QUALIFICATIONS A minimum of 2.5 years working with at-risk youth and their families. Must be 1) certified or registered by an organization that is recognized by the Department of Health Care Services and accredited with the National Commission for Certifying Agencies (CAADE, CADTP, or CCAPP), and 2) meets all California State education, training, and work experience requirements set forth in the Counselor Certification Regulations, Cal. Code Regs., tit. 9, Div. 4, chapter 8. Prior experience working with individuals dealing with substance use and co-occurring mental health disorders, ideally in a clinical, outpatient, or hospital setting.
