This role operates in a Home and Community-Based Services (HCBS) Setting, defined as medical, social, and supportive services for Iowans with functional, cognitive, and other physical or mental health needs (Iowa Health & Human Services). Collaborate with the HCBS Supervisor and HCBS Programmer to develop goals for the individual served.
Provide services to individuals based on their unique goals and behavioral care plan. This includes:
o Teaching individuals how to accomplish their goal (rather than completing a task for them).
o Using behavioral strategies that support individuals and address any behaviors that do not lead to their success.
o Motivate and encourage individuals to participate in their behavioral care plans and reach their goals.
o Participate in recreational, community, and social activities with those you serve, honoring the choices of the individual.
o Display and understanding of when it is appropriate to apply rights-restrictions, only as determined by the interdisciplinary team and when indicated in the individuals behavioral care plan.
Provide complete, consistent, and accurate documentation of the individuals progress.
Provide safe environments and instruction to individuals served regarding the practices that will help them reach their goals.
Provide care to individuals that help them present their best selves to the community. This may include helping with hygienic needs, choosing appropriate clothing, administering medication, and ensuring they have food options suitable for their dietary needs.
Be an advocate for individuals receiving services to have as much control over their own lives as possible. All individuals should be treated with respect and should feel empowered to live their life as independently as physically possible.

Schedule:
* Part-Time Days; Primarily looking for candidates to work hours between 10:00am - 6:30pm, or 11:00am - 7:30pm. Our Child Development Center is open Monday through Friday 5:45am - 7:30pm. No weekends or holidays!

* Child Abuse Mandatory Reporter Training is required within 6 months of hire and every 3 years thereafter.
* One to two years previous work experience with children and/or schooling in Early Childhood Education is highly desirable.

Looking to expand program with additional Critical Care providers
12-hour schedule
204 bed facility with full range of specialist and subspecialist
12 bed open ICU with plan to expand to 16 beds closed ICU
Fully integrated system with clinic and hospitals on Epic EMR to improve care coordination
Responsibilities
Collaboratively manage critically ill patients in the ICU, trained to perform essential procedures such as central lines, intubations, arterial lines, and dialysis catheter insertions
Actively engage in daily multidisciplinary rounds in the ICU
Collaborate daily with the intensivists, hospitalists, and other consulting physicians to coordinate care and cleary communicate goals and patient treatment plan with the intensive care unit nurses to facilitate care
Work within the critical care team and ensure coordination with attending, subspecialists, and nursing team.
Maintaining a high standard of continuity of care in handoff communication between shifts among providers and physicians
Average census: 9-10 patients per shift
Ability to Work as a team member to improve the health of the people and the communities we serve
Qualifications
Prior ICU and/or Critical Care either as advance practitioner and/or nurse preferred but with completion of the acute care program
Demonstrate and practice aligned with our FOCUS valuse in accordance with UPH standards
Hold an unrestricted medical license in the state of IA
Hold current and unrestricted state and federal authority to prescribe and administer medications as necessary or appropriate to the Advanced Practitioners field of practice
