Established in 2001, MSHN Enterprises is a supervised group home setting for individuals enduring severe and persistent mental illness. We provide therapeutic psycho-social rehabilitation groups, community outings, medication management, as well as individual therapy. MSHN also offers enhanced and medically fragile housing services to those individuals who have complex behavioral issues and/or medical issues in addition to mental illness. These services include treatment and nursing and/or physician professionals. We are not just a group home, we are a full-service supportive living program.
RESIDENT SAFETY:
Ensure residents are safe and not in the way of danger at any time. Proper Supervision of residents at all times and performing necessary checks. Never leave any residents by themselves or in a situation where they would be unsupervised. De-escalate residents to maintain group safety and help them regain self-control. Immediately report any unsafe environmental circumstances. Read and be familiar with fire and other natural disaster evacuation plans.
PROFESSIONALISM:
Be receptive to supervisory feedback, recognize your own weaknesses, and identify ways to strengthen them. Maintain all necessary training and seek out additional information that will aid in personal development. Maintain current knowledge on psychiatric disorders and mental health disorders. Refrain from gossip and any type of negative ongoing attitudes. Encourage staff members to refrain from gossip and maintain a positive, professional attitude.
TIME AND ATTENDANCE:
Be on time and ready to work for your shift. When unable to attend work due to illness, contact your supervisor with as much notice as possible. If you have a medical condition that causes you to be absent repeatedly, you may notify Human Resources directly to complete any necessary paperwork. Come to work dressed professionally, ready to do your best. When requesting time off, requests need to be made 2 weeks in writing in advance to your supervisor.
TEAMWORK IS KEY:
Every position at MSHN Enterprises is an important part of our success. We are all part of the team that has the same goal of taking care of our clients to ready them for independent living. At any time if you feel as though you have a conflict/issue with another employee, talk with that employee first. If it cannot be resolved, follow the chain of command to resolve it.
MEDICATION ASSISTANCE:
Adhere to policy and procedure of Medication Assistance as overviewed in Medication Assistance documentation. Ensuring that the correct medication and the correct resident on the correct schedule are being done and appropriately initialing the MAR while medication assistance is going on. Sending the correct medications with residents as they go on outings or pass and notifying ED and/or LPN of any discrepancies in medication or if the medication is running low for some reason. Appropriately document any PRN medications as they are needed.
RESIDENT INTERACTION:
Conduct Psycho-Social Rehabilitation discussions and activities. Respect the residents' right to privacy and dignity. Provide positive encouragement as you educate them in order to help them accomplish the necessary skills for independent living. Always model appropriate behavior utilizing yourself as the standard for residents. As needed assist or supervise residents with personal hygiene needs and appropriate clothing choices. DOCUMENTATION AND COMMUNICATION:
Always complete accurate, thorough documentation in a timely manner. When completing incident forms, do not leave any blanks or leave out significant information. Ensure all checklists and deep cleaning logs are completed on time. Do not be afraid to ask questions. Complete the intershift communication book and verbally communicate pertinent information to the next shift. At the beginning of shift; check the staff communication book for new memos and initial any memos after you have read them. Report all incidents immediately following the proper chain of command.
ENVIRONMENTAL CLEANLINESS:
Start your shift by completing a walk-through evaluation and addressing any issues. Refer to the daily rounds checklist to ensure everything has been covered. Assist and supervise residents in the completion of their chores and/or any cleaning jobs as they arise. Check your shift-specific checklists and complete them in a timely manner.
FOOD PREPARATION:
Prepare meals as dictated by the weekly menu or make appropriate substitutions as necessary with proper clearance from your supervisor. Check the menu and takeout food to thaw for the next day's meals. Appropriately label (time/date, what it is) and store any/all left over food items.
Requirements

Summary
The Advanced Practice Provider (APP) works independently, and in collaboration with the supervising physician to deliver excellent primary and chronic care management to a geriatric and palliative care population within long term care facility settings. The APP provides services within the APP scope of practice including the management of symptoms or disease processes related to acute and chronic conditions, assisting with communication and coordination of care, timely disease-specific education, as well as developing treatment plans that align with the patient's goals throughout the disease process. The APP would provide recommendations for and/or orders including pharmaceutical interventions/medications, laboratory, and diagnostic services as needed, and will work closely with the facility staff as well as patient and caregivers to maintain close engagement for optimal management. Assessment of needs may include utilization of additional IDT resources including social work or chaplaincy as needed. Due to the variety of variables that may impact navigation of care for this population, the APP must be flexible, demonstrate initiative, organization, and excellent professional communication with ability to build rapport rapidly with patients, their caregivers and facility staff for high satisfaction.
As a newer service line of care, the APP will exhibit positivity, autonomy, problem solving capabilities, and ambition to build and grow the geriatric - palliative care facility-based service line through excellent evidence-based care that is holistic in nature and provides optimal outcomes.
ESSENTIAL DUTIES AND RESPONSIBILITIES
Under the direction of the designated supervising physician, diagnoses medical conditions, plans and implements interventions, and evaluates patient response.
Assembles and documents complete patient medical database including health history and physical examination.
Documents assessments, orders, interventions, results and planning in the medical record.
Communicates findings with supervising physician, external medical team involvement, and IDT (interdisciplinary team) members as appropriate.
Provides information, education, and support to patients and families, as well as facility staff
Facilitates communication between patients, family members, and the IDT.
Fosters teamwork and serves as a resource within the IDT.
Promotes and practices economical and effective primary geriatric - palliative care.
Assesses eligibility for hospice services
Exhibits behaviors that reflect commitment to the philosophy and vision of Alive
Participates in team meetings as required
Potential to serve as a mentor and preceptor to nursing students, medical students, medical residents and fellows
Assists in orienting new providers to the Alive team as needed
Maintains all State and National certification (discipline specific), including active and unrestricted DEA, and demonstrates continued competency in the management of patient care
Participates in required continuing education to maintain credentialing and compliance for state licensure and national certification;. Remains current on research findings and integrates into practice.

Requirements
EDUCATION and/or EXPERIENCE
Graduation from an accredited APP degree program (ex: NP, CNS, PA); Masters level degree preferred. Experience in geriatrics, hospice or palliative care preferred. Experience in community based chronic disease management preferred.
CERTIFICATES, LICENSES, REGISTRATIONS
Current/Active and unrestricted Tennessee licensure, and maintenance of national certification in associated discipline (ex: NP/CNS/PA) along with ability to maintain unrestricted DEA for prescribing practices. If required to drive to carry out the duties of this position: current driver's license and automobile insurance as required by Tennessee State Law.

Responsibilities
Complete opening and closing park procedures, including, but not limited to: Check-in and retail facilities; completion and recording of daily inspections of courses, gear, and grounds; and reservation management.
Monitor climbers and axe throwers; coaching and providing feedback as necessary. This includes training and leading guests to complete check-in, briefing, harnessing, aerial trails, axe throwing, and special events.
Actively engage with guests to provide high-quality, memorable outdoor experiences. Provide excellent customer service in response to identified and expressed guest needs.
Acts as the first level of response for customer complaints, course assists, and emergency situations, informing and involving the supervisor and management teams as needed. Complete necessary supporting paperwork in a timely manner.
Assist with park maintenance, groundskeeping, retail sales, and marketing as needed.
Follow and enforce Outdoor Venture Group's policies and procedures, including those related to medical and disciplinary situations, facility and equipment use, and emergencies. Ideal candidate models desired guest behaviors.
Communicate personal, staff, guest, course, and supply needs to the management team in a timely manner, in addition to regular operational updates.

Qualifications
The Adventure Park operates in a natural outdoor environment. You must be comfortable working at height in all seasonal weather conditions as required.
Candidates must be at least 18 years old, able to lift 50 lbs.
Climbing experience is preferred, but not required.
Previous experience providing high-quality customer service is preferred.
Experience working with large groups in a professional setting is preferred.
Additional experience in adventure tourism is preferred.
Schedule
This is an hourly position, 20-40 hours per week. Available positions can be seasonal, part-time, or full time depending on operational needs and candidate availability.
High preference is given to candidates that are able to commit to the full operating season, currently scheduled to run from March 1 - December 1, 2026. Candidates that are available for the full season will be considered first for any extra winter, off-season hours that may become available.
Candidates should have a flexible schedule with both weekday and weekend availability.
Pay Range
While the exact pay offered reflects a candidate's qualifications and experience, Park Monitors are paid at a rate $16+ per hour.
