Job Duties:
Coordinates hospice care by working with other team members as well as with the patients/family members to ensure needs are met.
Evaluate the effectiveness of hospice services to the patient and family on an ongoing basis.
Perform evaluations for hospice care along with informational visits as needed.
Utilize the nursing process to provide professional nursing care.
Submit patient visit schedule weekly.
Provide guidance and supervision to the LPN and supervises the LPN a minimum of every 30 days.
Prepare the care plan for the Hospice Aide/Home Attendant.
Requirements
Must be a graduate from an accredited School of Nursing
Must be licensed in the State of Virginia as a Registered Nurse
Hospice/Home care experience preferred (extensive training provided)

As a Home Health Registered Nurse, here's a big-picture view of what you'll do:
Provide home-based care to patients with illnesses, injuries and chronic diseases-often after discharge from a hospital, rehab or nursing facility
Work with a team of physicians and oversee LPNs, CNAs and aides providing patient care
Examine patients, do assessments, document progress and report changes to their physician
Educate patients on their plan of care, prescribed medication, therapy, diet and exercise
Coach family members on the patient's plan of care, medication and home safety
Ensure goals are met and coordinate discharge from services
A few must-haves for Home Health Registered Nurses:
Graduate of an accredited nursing program and active RN license in Virginia
Minimum of 1year of nursing experience, ideally in home healthcare
CPR certification, TB Skin Test
Knowledge of state and federal home health regulations
Valid state-issued driver's license, automobile insurance, reliable transportation and willingness to travel
Clinical proficiency in nursing specialty, strong communication skills and compassion

Hospice Registered Nurse
Registered health nurse job at CareGivers Home Care
Salary Range 85k - 120k *More earning Potential along with benefits*
Coordinates hospice care by working with other team members as well as with the patients/family members to ensure needs are met.
Evaluate the effectiveness of hospice services to the patient and family on an ongoing basis.
Perform evaluations for hospice care along with informational visits as needed.
Utilize the nursing process to provide professional nursing care.
Consult with the attending physician and Hospice Medical Director concerning changes in patients' condition requiring order changes and/or changes in the plan of care.
Coordinate patient services with other members of the IDG as well as contracted services, when needed.
Submit complete, accurate and relevant clinical notes in a timely manner.
Submit patient visit schedule weekly.
Participate in IDG and case conferences.
Provide guidance and supervision to the LPN and supervises the LPN a minimum of every 30 days. Cooperate with other agencies providing nursing or related services to provide continuity of care and to implement a comprehensive care plan.
Participate in staff development meetings.
Prepare the care plan for the Hospice Aide/Home Attendant.
Requirements
Must be a graduate from an accredited School of Nursing.
Must be licensed in the State of Virginia as a Registered Nurse.
A minimum of two (2) years of hospice/home care experience; at least one (1) year of supervisory experience.
Must have excellent assessment skills and problem -solving skills.
Should be skillful in organization and in the principles of time management and have knowledge of case management processes.
Must be a licensed driver with an insured automobile in good working order.
The ability to communicate well, both verbally and in writing.
