JOB SUMMARY
Care Coordinator/Support Brokers coordinate the day-to-day implementation of contractual requirements within the state's Medicaid guidelines. The Care Coordinator works with members on plan/budget development, submission, and revision.

QUALIFICATIONS

Must have reliable transportation. If the employee will be driving, a valid driver's license and auto insurance is required.
Combination of education and experience
