Hope the Mission's ICMS (Intensive CaseManagement Services) CaseManagers are an essential part of our Pathway Home team. This role supports participants placed in Permanent Housing by providing individualized, trauma-informed care designed to promote long-term housing stability and personal well-being. The ICMS CaseManager works with participants who have experienced chronic homelessness, often alongside complex physical and behavioral health conditions. This role is grounded in harm reduction, recovery-oriented, and Housing First principles.
Primary Duties and Responsibilities
Under the supervision of the Program Manager, the ICMS CaseManager will:
CaseloadManagement
● Maintain a caseload of 20 eligible participants from our Pathway Home site, providing high-quality, client-centered, and strength-based services.
● Prioritize clients based on acuity and level of service needs.
CaseManagement
● Develop individualized Care Plans for each participant.
● Conduct weekly or bi-weekly in-person check-ins with clients based on need.
● Assist with securing documents (ID, Social Security card, income verification, etc.).
● Coordinate with DMH, DHS, HFH and other County and non profit providers for clinical and supportive services.
Housing Retention Support
● Conduct home visits and phone check-ins to promote housing stability once housed.
● Provide landlord mediation and support.
● Monitor lease compliance and respond to housing crises.
Service Navigation and Referrals
● Connect clients to mental health, medical, substance use, employment, benefit services and other services as needed.
● Assist with applications for mainstream benefits including SSI, Medi-Cal, and CalFresh.
● Facilitate warm hand-offs to clinical providers and ensure follow-up.
● Submit housing focused service referrals in Comprehensive Health Accompaniment Management Platform (CHAMP) in an effort to place participants in permanent housing placements suited to their needs.
Documentation & Compliance
● Maintain accurate and timely documentation in Comprehensive Health Accompaniment Management Platform (CHAMP) per contractual requirements.
● Track client progress toward goals and update care plans monthly.
● Participate in case conferencing and multidisciplinary team (MDT) meetings.
● Obtain and maintain HMIS access for care coordination purposes with other Non CHAMP providers.
Collaboration & Engagement
● Engage in regular communication with housing providers, landlords, and community partners to maintain good working relationships to benefit participants.
Crisis Intervention
● Respond to behavioral, medical, or housing-related crises with appropriate interventions.
● Develop safety plans and initiate emergency referrals when necessary.
Professional Development
● Attend all required training per HFH and Hope the Mission guidelines.
● Utilize supervision to discuss challenging cases and professional growth.
Knowledge, Skills, and Abilities
● Understanding of ICMS Principles: Trauma-informed, low-barrier, housing-first approaches.
● Cultural Competency: Ability to engage individuals with diverse backgrounds and needs.

Other Requirements
● Experience: Bachelor's degree in social work, psychology, or related field preferred; 2+ years working with individuals experiencing homelessness, mental illness, or co-occurring disorders in a CaseManagement capacity.
● Database Knowledge: Familiar with HMIS, CHAMP, or similar casemanagement databases.
● Language: Bilingual (English/Spanish) preferred.
● Transportation: Valid California driver's license and clean driving record to enable driving agency provided vehicles.
Working Conditions and Physical Demands

Job Purpose

HOTV's Tiny Homes/Cabin Communities provide a safe, low-barrier, and supportive 24-hour residence to persons experiencing homelessness. Participants receive shelter, food, housing navigation, and casemanagement services to support their trajectory in obtaining permanent housing . Working under the supervision of the Program Manager, the Housing Focused CaseManager fulfills the mission of the Hope of Valley Rescue Mission by successfully providing direct services to homeless participants in HOTV's Tiny Homes/Cabin Communities Program.
Primary Duties and Responsibilities
Responsible for providing support to homeless individuals in the following areas: housing focused casemanagement, housing readiness, accessing housing assistance programs, completing housing applications, advocacy, housing search and placement, etc.
Conduct CES assessment to determine appropriate permanent housing intervention and service needs
Navigate persons to permanent housing placement or programs that will assist with safe and stable housing - Rapid Rehousing, DHS- Housing for Health, Homeless Section 8, Tenant-Based Supportive Housing, Housing Choice Voucher Program, HOPWA, VASH, Project Based Housing, SRO, Shared Housing, Low Income Housing, etc.
Responsible to develop a Housing & Services Plan (HSP) to streamline participants towards permanent housing and specific goals
Connect program participants to community resources that will support the goal of permanent housing acquisition (credit repair, legal aid, housing rights, etc.)
Coordinate transportation for participants to housing appointments (Unit viewings, Housing Authority Interviews, etc.)
Assist clients in obtaining move-in assistance resources as available (deposit assistance, furniture, etc.)
Responsible to work in collaboration with shelter support Staff
Follow-up with each participant on a regular basis to ensure they are making progress towards their goals
Responsible to complete and maintain current documents required for the client files, including assessments, individualized housing plans, case notes, housing applications, housing vouchers, lease agreements, exit reports, etc.
Travel is required for this position.
Other Duties Include
Develop and maintain professional working relationships with landlords, sober livings, and other housing providers to establish a “database” of available housing units
Attend and participate in Team Meetings, Trainings, Case Conferencing, CES Meetings, Coalition Meetings, Homeless Services Meetings, etc.
Responsible for maintaining thorough and accurate records in both written form (hard copies) and through the Homeless Management Information System (HMIS) is required
Responsible for meeting specific performance outcomes for applicable LAHSA contracts
Submitting reports, documentation and connecting with outside agencies
Establish and/or maintain good rapport and working arrangements with co-workers, including the management team and other staff
Facilitate and supervise regularly scheduled departmental staff meetings and resident house meetings;
Uphold all HOTV and LAHSA Rules, Regulations, and Policies.
Use de-escalation and mediation techniques to help clients work through conflict

Qualifications
Understand and support the mission statement of Hope of the Valley Rescue Mission
Bachelor's Degree in Social Work or related field or three (3) years' experience working with the homeless or similar population (preferably in a housing related capacity) preferred

Essential Function Examples
Provides direct care to Choices graduates in a therapeutic environment conducive to recovery and healing from addiction. Researches and presents new developments/information regarding addiction and treatment. Possesses a working knowledge of the tools required to maintain recovery.

Job Scope: In collaboration with agency psychiatrists and medical secretary will deliver re-assessment and treatment planning services, brief individual crisis intervention or supportive counseling, symptom management skill building groups, and casemanagement and/or community support individual to a primary population of Adult clients receiving medication treatment only services. As adult therapist, provide individual, group and family treatment that targets improvement in functional impairments.
Primary Responsibilities:
Provide initial intake screening and mental health assessments that identify service needs and recommend options for care
Share assessment findings with clients
Involve client and/or couple in the development of treatment plan goals, objectives and interventions that address their assessed behavioral health needs
Provide face-to-face mental health re-assessment as needed but at least annually for medication only clients.
Meet with medication only clients for treatment planning review and modification as needed but at least every 6 months.
Provide brief crisis intervention, stabilization services and supportive counseling to meds only clients.
Regularly consult with psychiatrists on specific client related clinical issues.
Provide individual, marital, group, or family therapy service
Provide casemanagement services to assist clients and their families with access to needed benefits and services
Provide coordinated care by initiating or participating in client centered consultation with internal and external providers involved in care or services
Reassess and review treatment progress with clients and their families at proscribed intervals
Provide back-up coverage for Medical Secretary.
Assist in establishing effective communication between the Center, other social agencies serving clients, and the Community

* Juvenile Correctional Counselor class, which are experienced counselors who are expected to perform at a higher level of competence with only general supervision.
WORKING CONDITIONS
* Incumbents must be able to work in a restricted, sometimes hostile environment.
* Incumbents may be required to lift objects weighing more than 100 pounds with assistance.

ESSENTIAL DUTIES
This class specification represents the core area of responsibilities; specific position assignments will vary depending on the needs of the department.
* Learns how to ensure the security, daily care and custody of youth within a pod including counseling, mediating disputes, monitoring group dynamics amongst youth, assisting in emergency situations, serving meals and overseeing laundry care and distribution, logging and dispensing inhalers and medicated shampoo as prescribed by medical staff, and performing all other duties to oversee the health and safety of those in custody.
* Learns to provide crisis intervention counseling including suicide prevention counseling, to youth.
* Learns when and how to refer youth to behavioral health.
* Learns to provide general counseling and cognitive behavioral training to give support or to modify youths' behavior; may give personal guidance and counseling to reduce anxiety and tension of youth who are detained.
* Learns to facilitate Cognitive Behavioral Training (CBT) groups; implements behavioral management program; conducts CBT mini-cycles with youth; creates and monitors weekly and monthly case plans.
* Learns to manage assaultive behavior; prepares behavior summaries and written daily observations, including special incident reports for youths on a daily basis or as needed; enforces the policies and procedures of the institution.
* Learns to plan recreation and other activities independently for medium to large groups of justice involved youth of varying ages, gender, ability and sophistication.
* Learns to monitor individual and group behavior and attitudes independently to avoid and manage conflicts; maintains constant observation; conduct safety and security checks; conducts authorized felony or misdemeanor searches of youth and all required institutional searches; supervises work, school and study time, meal time, clean-up, visits, and all other group activities of youth; documents the behavior of individuals by writing timely reports and keeping accurate records.
* Learns to process youth during intake to the facility. Orients all youth to the facility setting and to staff expectations. Instructs youth in social and life skills.
* Learns procedures to transport youth to medical, dental, court or other mandatory appointments.
* Learns to provide emergency medical first aid treatment as needed.
* Inspects incoming and outgoing mail; calculates and records point totals for behavior management; issues supplies.
* Learns to conduct drug testing of youth.
* Learns to assist in providing treatment in a community-based program; assists in rehabilitation by facilitating an integrated system of care, using the expertise of other agencies and personnel; facilitates conferences with families and assigned personnel; acts as a liaison with and supports families; provides vocational assessments and educational support.

POSITION REQUIREMENTS
Education: Completion of thirty (30) semester units is required from an accredited college or university, preferably in criminology, social work, psychology, sociology or a closely related field.
OR
Experience: Six (6) months of related experience supervising groups in a detention facility.
LICENSING, CERTIFICATION AND REGISTRATION REQUIREMENTS
* Applicants are required to possess, or have the ability to obtain, a valid California Driver's license, Class C.
* Incumbents are required to possess a current First Aid and Personal Safety certificate or obtain one within 60 days of appointment.
* Incumbents are required to possess a current CPR certificate or obtain one within 60 days of appointment.
* Incumbents have limited Peace Officer powers as delineated in the California Penal Code and therefore are required to complete training under Section 832 of the California Penal Code or equivalent within twelve (12) months of appointment.
* Peace Officers must meet minimum standards concerning citizenship, age, character, education and physical/mental condition as set forth in Section 1031 of the California Government Code.
* Incumbents are required to complete the Juvenile Counselor Core Course certified by the California Board of State and Community Corrections Standards and Training for Correctional Programs or equivalent with twelve (12) months of appointment.
Note: All licenses, certificates and registrations must be kept current while employed in this class.
PHYSICAL REQUIREMENTS
* Mobility and Dexterity: Positions in this class typically require stooping, kneeling, reaching, standing, walking, grasping, feeling (i.e., sense of touch), and repetitive motion for 50% of a typical shift but may be higher.
* Lifting, Carrying, Pushing and Pulling: Positions in this class require exerting up to 100 pounds of force occasionally, and/or up to 10 pounds of force frequently, and/or a negligible amount of force constantly, to move objects. Employees in this class must have the visual acuity to make observations of surroundings and must demonstrate color vision sufficient to distinguish colors in order to describe events in an accurate manner.
* Hearing/Talking: Positions in this class require the employee to perceive the nature of sounds at normal speaking levels with or without correction and have the ability to receive detailed information through oral communication. Positions in this class require the employee to express or exchange ideas by means of the spoken word. Detailed or important instructions must often be conveyed to others accurately, loudly, and/or quickly.
OTHER REQUIREMENTS
* Must pass a physical exam and psychological exam as a condition of hire.
* Independent Travel: Incumbents are required to travel independently, for example, to attend meetings, trainings, classes and seminars and to transport juveniles.
* Background Checks: The County may conduct a background check and a reference check on candidates prior to appointment to a position within this class. Job Description This is the entry-level class in the Protective Services Worker series.
Adult Protective Services (APS) is a state-mandated program administered by the City and County of San Francisco (CCSF), HSA, and DAS.
Under general supervision by the APS program, the Protective Services Worker (PSW) receives and responds to reports of suspected “elder” and “dependent adult” abuse including neglect, self-neglect, and financial abuse.
The PSW performs professional investigations and casemanagement services to victims of abuse.
This includes activities related to evaluating the level of harm or risk of harm, to the person and the estate of an “elder” or “dependent adult;” assessing biopsychosocial functioning, including the medical condition, cognitive/psychological status, and adequacy of care and support system; orchestrating ongoing services to maximize the use of community resources, and to help protect victims from further harm once the case is closed.
Essential Duties and Functions According to Civil Service Commission Rule 109, the duties specified below are representative of the range of duties assigned to this job class and are not intended to be an inclusive list.
Conducts unannounced visits to see elders and dependent adults in their living environment.
Investigates the allegations and conducts a risk assessment through interviewing the victim, family, relatives, neighbors, and others to assess for immediacy.
Arranges for immediate protective interventions and emergency services as necessary, including medical assessments, emergency shelter, or in-home protection, e.
g.
acts as a liaison on the medical aspects of the case with providers of services and other agencies that provide medical care, until the immediate dangers are resolved.
Gathers, documents, and reviews important detailed financial, personal, and confidential information, and writes investigative reports, findings, and service plan notes.
Conducts a needs assessment for clients in need of protective services.
Develops a Service Plan and organizes the initial care plan as appropriate, including supported decision-making activities.
Provides counseling for clients and significant others to alleviate the identified problems and to implement the service plan to restore optimum social functioning, and health adjustment within the client's capacity; assists them in order to address personal and environmental difficulties, which perpetuate poor health and dysfunction, and interfere with self-care and self-protection.
Monitors the Service Plan and follows up through field visits and telephone contacts with clients, relatives, and other concerned individuals until the service goals have been achieved.
Assists with multidisciplinary team meetings with healthcare providers, hospitals, and the Public Health/Mental Health Department, and other City agencies involved to further their understanding of significant social and emotional factors underlying the client's health, social, or environmental problems.
Designs interventions in collaboration with medical professionals, social service providers, law enforcement, or other relevant systems by contributing with information to help determine the level of care and services pertinent to the client's welfare.
Provides education to individuals and agencies on the role and authority of APS.
Conducts public presentations about casework in a confidential, multi-disciplinary setting; collaborates and cooperates with the community and the CCSF partner agencies.
Uses a personal computer to prepare, review, and maintain records, correspondence, and reports; managescase documentation in an APS database; maintains timely email correspondence.
Qualifications 1.
Education: Possession of a Master's Degree in Social Work from a school accredited by the Council on Social Work Education (CSWE); OR Master's Degree from a two (2) year counseling program from an accredited college or university.
(Qualifying Master's Degrees in counseling include Marriage and Family Therapy, Clinical Counseling, Mental Health Counseling, Addiction Counseling and Counseling Psychology that included a supervised field placement as part of the course curriculum); AND 2.
Experience: Satisfactory completion of two semesters of graduate field placement which involved the provision of direct services to elders and/or dependent adults and/or adults with physical, cognitive or mental health disabilities in one of the following or closely related areas: Adult Protective Services, intensive casemanagement such as MSSP/Linkages, medical social work in a hospital or clinic, mental health program/agency, home health agency, or alcohol and drug counseling and treatment program; OR Six (6) months post-baccalaureate professional work experience which involved the provision of direct services to elders, dependent adults, or adults in one of the areas previously described.

Score Banking: Scores attained on the Protective Services Worker Exam for Adult Protective Services will be 'banked' for two years, starting from the date of the examination.
This means that, during this two-year time period, candidates need not take this test again.
However, after one year, a candidate has the option to either (a) keep the same score and apply their test score to the other announcement or (b) re-take the test.
Re-testing is permitted no sooner than one year from the date of the test.
If a candidate opts to re-test, the re-test score becomes the candidate's official score since it is the most recent.
If a significant modification is made to this exam in the future, the application of “banked” scores may no longer be appropriate.
In such a rare circumstance, candidates with “banked” scores would be required to re-take the exam sooner than the two-year period specified above should they apply to another job opportunity where the modified exam is used.

Job Description
This is the entry-level class in the Protective Services Worker series. Adult Protective Services (APS) is a state-mandated program administered by the City and County of San Francisco (CCSF), HSA, and DAS. Under general supervision by the APS program, the Protective Services Worker (PSW) receives and responds to reports of suspected “elder” and “dependent adult” abuse including neglect, self-neglect, and financial abuse. The PSW performs professional investigations and casemanagement services to victims of abuse. This includes activities related to evaluating the level of harm or risk of harm, to the person and the estate of an “elder” or “dependent adult;” assessing biopsychosocial functioning, including the medical condition, cognitive/psychological status, and adequacy of care and support system; orchestrating ongoing services to maximize the use of community resources, and to help protect victims from further harm once the case is closed.
Essential Duties and Functions
According to Civil Service Commission Rule 109, the duties specified below are representative of the range of duties assigned to this job class and are not intended to be an inclusive list.
Conducts unannounced visits to see elders and dependent adults in their living environment.
Investigates the allegations and conducts a risk assessment through interviewing the victim, family, relatives, neighbors, and others to assess for immediacy.
Arranges for immediate protective interventions and emergency services as necessary, including medical assessments, emergency shelter, or in-home protection, e.g. acts as a liaison on the medical aspects of the case with providers of services and other agencies that provide medical care, until the immediate dangers are resolved.
Gathers, documents, and reviews important detailed financial, personal, and confidential information, and writes investigative reports, findings, and service plan notes.
Conducts a needs assessment for clients in need of protective services.
Develops a Service Plan and organizes the initial care plan as appropriate, including supported decision-making activities.
Provides counseling for clients and significant others to alleviate the identified problems and to implement the service plan to restore optimum social functioning, and health adjustment within the client's capacity; assists them in order to address personal and environmental difficulties, which perpetuate poor health and dysfunction, and interfere with self-care and self-protection.
Monitors the Service Plan and follows up through field visits and telephone contacts with clients, relatives, and other concerned individuals until the service goals have been achieved.
Assists with multidisciplinary team meetings with healthcare providers, hospitals, and the Public Health/Mental Health Department, and other City agencies involved to further their understanding of significant social and emotional factors underlying the client's health, social, or environmental problems.
Designs interventions in collaboration with medical professionals, social service providers, law enforcement, or other relevant systems by contributing with information to help determine the level of care and services pertinent to the client's welfare.
Provides education to individuals and agencies on the role and authority of APS.
Conducts public presentations about casework in a confidential, multi-disciplinary setting; collaborates and cooperates with the community and the CCSF partner agencies.
Uses a personal computer to prepare, review, and maintain records, correspondence, and reports; managescase documentation in an APS database; maintains timely email correspondence.
Qualifications
1. Education:
Possession of a Master's Degree in Social Work from a school accredited by the Council on Social Work Education (CSWE);
OR
Master's Degree from a two (2) year counseling program from an accredited college or university. (Qualifying Master's Degrees in counseling include Marriage and Family Therapy, Clinical Counseling, Mental Health Counseling, Addiction Counseling and Counseling Psychology that included a supervised field placement as part of the course curriculum);
AND
2. Experience:
Satisfactory completion of two semesters of graduate field placement which involved the provision of direct services to elders and/or dependent adults and/or adults with physical, cognitive or mental health disabilities in one of the following or closely related areas: Adult Protective Services, intensive casemanagement such as MSSP/Linkages, medical social work in a hospital or clinic, mental health program/agency, home health agency, or alcohol and drug counseling and treatment program;
OR
Six (6) months post-baccalaureate professional work experience which involved the provision of direct services to elders, dependent adults, or adults in one of the areas previously described.

Score Banking: Scores attained on the Protective Services Worker Exam for Adult Protective Services will be 'banked' for two years, starting from the date of the examination. This means that, during this two-year time period, candidates need not take this test again. However, after one year, a candidate has the option to either (a) keep the same score and apply their test score to the other announcement or (b) re-take the test. Re-testing is permitted no sooner than one year from the date of the test. If a candidate opts to re-test, the re-test score becomes the candidate's official score since it is the most recent.
If a significant modification is made to this exam in the future, the application of “banked” scores may no longer be appropriate. In such a rare circumstance, candidates with “banked” scores would be required to re-take the exam sooner than the two-year period specified above should they apply to another job opportunity where the modified exam is used.
The above test component is considered standardized and, therefore, test questions and answers are not available for public inspection or review.

Job Description
Under general supervision, positions in class 2940 Protective Services Worker within the Office of the Public Conservator in the Department of Aging and Adult Services provide a broad range of clinical/advocacy services to temporary/permanent conservatees and collateral contact with a multidisciplinary team. In general, an average caseload is approximately 65 clients and can be more or less depending on case intensity and program capacity.
The target population of this office is individuals who are gravely disabled, (unable to provide for food, clothing and shelter as a result of chronic mental illness and/or chronic alcoholism), as a result of major mental illness as defined by the Diagnostic Statistical Manuel (DSM). The population also includes individuals who may have involvement with the Criminal Justice System and are being held in jail and State Hospitals or other facilities designated for those who have committed, or are alleged to have committed a crime.
The Office of Conservatorship Services has an average caseload of 700, with approximately half of these individuals placed outside of San Francisco County in either locked psychiatric facilities or residential care homes. Therefore travel is required in order to meet the needs of the conservatee and our service mandate by State and County regulations. The average number of miles driven per month is 350.
The 2940 is responsible for managing an independent caseload and is assigned mental health (LPS) conservatorship investigations as well. This involves interaction with Mental Health Court, including Public Defender and District Attorney, Department of Public Health, hospitals, jails and community agencies. In addition to assessments and making recommendations for levels of care, the conservator is also responsible for advocating these recommendations, which may at time be at conflict with the interests of other stakeholders.
Basic computer skills are required to perform charting on electronic systems (Avatar and Panoramic), emailing (the bulk of inter and intra agency communication is done through e-mail).
ESSENTIAL DUTIES AND FUNCTIONS

Conducts regular in-person visits in a variety of settings including jail, state hospital, inpatient acute psychiatric units, locked psychiatric treatment centers and skilled nursing facilities as well as community settings within and outside the City and County of San Francisco.
Conducts crisis interventions during business hours or after hours.
Completes time sensitive court reports and provision of expert testimony witness to the Superior Court as needed.
Evaluates and approves psychiatric treatment and medications included in clients care plans.
Continually assesses appropriateness of placements and the least restrictive level of care.
Completes various types of applications and forms related to placement, legal, financial, and medical issues.
Utilizes productivity tools such as Microsoft Office Suite to complete various work duties including coordinating case conferences, presentations, and communicating with team members and other necessary service providers..
Documents cases in a clinically significant and timely manner using applicable digital casemanagement system.
Works collaboratively with other city departments, including Public Health, Police, Jail, and City Attorney's Office.

Job Description
Under general supervision, eligibility workers serve clients from diverse cultural and socio-economic backgrounds by reviewing and determining initial and continuing eligibility for a variety of human services programs. Positions in this classification require strong customer service skills, including the ability to communicate clearly, listen actively, and respond with empathy and professionalism. Senior Eligibility Workers are responsible for carrying out rules and regulations pertaining to various social service programs, explaining them to clients and the general public, and providing assistance in a courteous and respectful manner to ensure a positive client experience.

Condition of Employment: Induction Training
As a condition of employment to 2905 HSA Senior Eligibility Worker position(s), candidates will be required to participate in paid Induction Training which may last up to six (6) months. This includes classroom-based training, assessments, and on-the-job training. Candidates offered positions in this class must be available to start employment on a date determined by the Agency. Induction Training is primarily conducted in-person at various Human Services Agency buildings located throughout San Francisco. In addition to in-person training, online/remote sessions may take place. Training will be held Monday through Friday from 8:00AM to 5:00PM. Strict attendance is observed.
The Induction Training is designed to provide new eligibility workers with fundamental information and guidelines to determine eligibility benefits, mathematical skills to perform calculations of income and potential benefits, reading and comprehension skills, skills to interpret and apply local, State and Federal laws and ordinances to aide in the determination of public benefit programs, and program and benefits information to explain eligibility guidelines to applicants, clients and to the general public. On-the job training is designed to provide new eligibility workers with real-world applications of training fundamentals acquired through classroom-based training.
New employees MUST successfully meet the standards of the Induction Training (classroom-based training/assessments AND on-the-job training) as a condition of employment. Failure to successfully complete the Induction Training will be grounds for dismissal.
New eligibility workers that successfully complete the classroom portion of Induction Training will also participate in on-the-job training where real-world application of training fundamentals will be applied.
Qualifications
Education:
Sixty (60) semester units or ninety (90) quarter units from an accredited college or university;
OR
Experience:
1. Two (2) years of clerical work experience which includes the following: use of computer applications to input and retrieve information; regular public contact to provide assistance or acquire detailed personal or confidential information; and interpreting and applying rules, regulations and policies;

Job Description Under general supervision, positions in class 2940 Protective Services Worker within the Office of the Public Conservator in the Department of Aging and Adult Services provide a broad range of clinical/advocacy services to temporary/permanent conservatees and collateral contact with a multidisciplinary team.
In general, an average caseload is approximately 65 clients and can be more or less depending on case intensity and program capacity.
The target population of this office is individuals who are gravely disabled, (unable to provide for food, clothing and shelter as a result of chronic mental illness and/or chronic alcoholism), as a result of major mental illness as defined by the Diagnostic Statistical Manuel (DSM).
The population also includes individuals who may have involvement with the Criminal Justice System and are being held in jail and State Hospitals or other facilities designated for those who have committed, or are alleged to have committed a crime.
The Office of Conservatorship Services has an average caseload of 700, with approximately half of these individuals placed outside of San Francisco County in either locked psychiatric facilities or residential care homes.
Therefore travel is required in order to meet the needs of the conservatee and our service mandate by State and County regulations.
The average number of miles driven per month is 350.
The 2940 is responsible for managing an independent caseload and is assigned mental health (LPS) conservatorship investigations as well.
This involves interaction with Mental Health Court, including Public Defender and District Attorney, Department of Public Health, hospitals, jails and community agencies.
In addition to assessments and making recommendations for levels of care, the conservator is also responsible for advocating these recommendations, which may at time be at conflict with the interests of other stakeholders.
Basic computer skills are required to perform charting on electronic systems (Avatar and Panoramic), emailing (the bulk of inter and intra agency communication is done through e-mail).

Education: Possession of a master's degree in Social Work from a school accredited by the Council on Social Work Education (CSWE); OR A Master's Degree from a two-year (2) counseling program from an accredited college or university.
(Qualifying Master's Degrees in counseling include Marriage and Family Therapy, Clinical Counseling, Mental Health Counseling, Addiction Counseling and Counseling Psychology that included a supervised field placement as part of the course curriculum).

Role description
Under general supervision, positions in class 2940 Protective Services Worker within the Office of the Public Conservator in the Department of Aging and Adult Services provide a broad range of clinical/advocacy services to temporary/permanent conservatees and collateral contact with a multidisciplinary team. In general, an average caseload is approximately 65 clients and can be more or less depending on case intensity and program capacity.
The target population of this office is individuals who are gravely disabled, (unable to provide for food, clothing and shelter as a result of chronic mental illness and/or chronic alcoholism), as a result of major mental illness as defined by the Diagnostic Statistical Manuel (DSM). The population also includes individuals who may have involvement with the Criminal Justice System and are being held in jail and State Hospitals or other facilities designated for those who have committed, or are alleged to have committed a crime.
The Office of Conservatorship Services has an average caseload of 700, with approximately half of these individuals placed outside of San Francisco County in either locked psychiatric facilities or residential care homes. Therefore travel is required in order to meet the needs of the conservatee and our service mandate by State and County regulations. The average number of miles driven per month is 350.
The 2940 is responsible for managing an independent caseload and is assigned mental health (LPS) conservatorship investigations as well. This involves interaction with Mental Health Court, including Public Defender and District Attorney, Department of Public Health, hospitals, jails and community agencies. In addition to assessments and making recommendations for levels of care, the conservator is also responsible for advocating these recommendations, which may at time be at conflict with the interests of other stakeholders.
Basic computer skills are required to perform charting on electronic systems (Avatar and Panoramic), emailing (the bulk of inter and intra agency communication is done through e-mail).
ESSENTIAL DUTIES AND FUNCTIONS

* Conducts investigations to determine eligibility for psychiatric care and services, including interviews and information gathered from collateral sources.
* Conducts regular in-person visits in a variety of settings including jail, state hospital, inpatient acute psychiatric units, locked psychiatric treatment centers and skilled nursing facilities as well as community settings within and outside the City and County of San Francisco.
* Conducts crisis interventions during business hours or after hours.
* Completes time sensitive court reports and provision of expert testimony witness to the Superior Court as needed.
* Evaluates and approves psychiatric treatment and medications included in clients care plans.
* Continually assesses appropriateness of placements and the least restrictive level of care.
* Completes various types of applications and forms related to placement, legal, financial, and medical issues.
* Utilizes productivity tools such as Microsoft Office Suite to complete various work duties including coordinating case conferences, presentations, and communicating with team members and other necessary service providers..
* Documents cases in a clinically significant and timely manner using applicable digital casemanagement system.
* Works collaboratively with other city departments, including Public Health, Police, Jail, and City Attorney's Office.
How to qualify
1. Education: Possession of a master's degree in Social Work from a school accredited by the Council on Social Work Education (CSWE);
OR
A Master's Degree from a two-year (2) counseling program from an accredited college or university. (Qualifying Master's Degrees in counseling include Marriage and Family Therapy, Clinical Counseling, Mental Health Counseling, Addiction Counseling and Counseling Psychology that included a supervised field placement as part of the course curriculum).
AND
2. Experience: Satisfactory completion of two semesters of graduate field placement which involved the provision of direct services to elders and/or dependent adults and/or adults with physical, cognitive or mental health disabilities in one of the following or closely related areas: Adult Protective Services, intensive casemanagement such as MSSP/Linkages, medical social work in a hospital or clinic, mental health program/agency, home health agency or alcohol and drug counseling and treatment program
OR
Six (6) months post baccalaureate professional work experience which involved the provision of direct services to elders, dependents, or adults in one of the areas previously described.
AND
3. License & Certification:
* Possession of a valid California class C driver's license and clear Motor Vehicle Record (MVR);
AND
* Possession of a valid Licensed Clinical Social Worker (LCSW) license issued by the California Board of Behavioral Sciences(BBS) OR
* Current proof of registration as an Associate Social Worker (ASW) intern issued by the California Board of Behavioral Sciences(BBS); OR
* Possession of a valid Marriage and Family Therapist (MFT) license issued by the California Board of Behavioral Sciences(BBS); OR

Condition of Employment: Induction Training
As a condition of employment to 2905 HSA Senior Eligibility Worker position(s), candidates will be required to participate in paid Induction Training which may last up to six (6) months. This includes classroom-based training, assessments, and on-the-job training. Candidates offered positions in this class must be available to start employment on a date determined by the Agency. Induction Training is primarily conducted in-person at various Human Services Agency buildings located throughout San Francisco. In addition to in-person training, online/remote sessions may take place. Training will be held Monday through Friday from 8:00AM to 5:00PM. Strict attendance is observed.
The Induction Training is designed to provide new eligibility workers with fundamental information and guidelines to determine eligibility benefits, mathematical skills to perform calculations of income and potential benefits, reading and comprehension skills, skills to interpret and apply local, State and Federal laws and ordinances to aide in the determination of public benefit programs, and program and benefits information to explain eligibility guidelines to applicants, clients and to the general public. On-the job training is designed to provide new eligibility workers with real-world applications of training fundamentals acquired through classroom-based training.
New employees MUST successfully meet the standards of the Induction Training (classroom-based training/assessments AND on-the-job training) as a condition of employment. Failure to successfully complete the Induction Training will be grounds for dismissal.

Qualifications 1.
Education a.
Possession of a Master's Degree in Social Work from a school accredited by the Council on Social Work education (CSWE); OR b.
Possession of a Master's Degree from a two-year (2) counseling program from an accredited college or university.
(Qualifying Master's Degrees in counseling include Marriage and Family Therapy, Clinical Counseling, Mental Health Counseling, Addiction Counseling and Counseling Psychology that included a supervised field placement as part of the course curriculum).

Role description
This is the entry-level class in the Protective Services Worker series. Adult Protective Services (APS) is a state-mandated program administered by the City and County of San Francisco (CCSF), HSA, and DAS. Under general supervision by the APS program, the Protective Services Worker (PSW) receives and responds to reports of suspected "elder" and "dependent adult" abuse including neglect, self-neglect, and financial abuse. The PSW performs professional investigations and casemanagement services to victims of abuse. This includes activities related to evaluating the level of harm or risk of harm, to the person and the estate of an "elder" or "dependent adult;" assessing biopsychosocial functioning, including the medical condition, cognitive/psychological status, and adequacy of care and support system; orchestrating ongoing services to maximize the use of community resources, and to help protect victims from further harm once the case is closed.
Essential Duties and Functions
According to Civil Service Commission Rule 109, the duties specified below are representative of the range of duties assigned to this job class and are not intended to be an inclusive list.
* Conducts unannounced visits to see elders and dependent adults in their living environment.
* Investigates the allegations and conducts a risk assessment through interviewing the victim, family, relatives, neighbors, and others to assess for immediacy.
* Arranges for immediate protective interventions and emergency services as necessary, including medical assessments, emergency shelter, or in-home protection, e.g. acts as a liaison on the medical aspects of the case with providers of services and other agencies that provide medical care, until the immediate dangers are resolved.
* Gathers, documents, and reviews important detailed financial, personal, and confidential information, and writes investigative reports, findings, and service plan notes.
* Conducts a needs assessment for clients in need of protective services.
* Develops a Service Plan and organizes the initial care plan as appropriate, including supported decision-making activities.
* Provides counseling for clients and significant others to alleviate the identified problems and to implement the service plan to restore optimum social functioning, and health adjustment within the client's capacity; assists them in order to address personal and environmental difficulties, which perpetuate poor health and dysfunction, and interfere with self-care and self-protection.
* Monitors the Service Plan and follows up through field visits and telephone contacts with clients, relatives, and other concerned individuals until the service goals have been achieved.
* Assists with multidisciplinary team meetings with healthcare providers, hospitals, and the Public Health/Mental Health Department, and other City agencies involved to further their understanding of significant social and emotional factors underlying the client's health, social, or environmental problems.
* Designs interventions in collaboration with medical professionals, social service providers, law enforcement, or other relevant systems by contributing with information to help determine the level of care and services pertinent to the client's welfare.
* Provides education to individuals and agencies on the role and authority of APS.
* Conducts public presentations about casework in a confidential, multi-disciplinary setting; collaborates and cooperates with the community and the CCSF partner agencies.
* Uses a personal computer to prepare, review, and maintain records, correspondence, and reports; managescase documentation in an APS database; maintains timely email correspondence.

Condition of Employment: Induction Training As a condition of employment to 2905 HSA Senior Eligibility Worker position(s), candidates will be required to participate in paid Induction Training which may last up to six (6) months.
