    Job Description The Clinical Therapist is responsible for providing therapy in an outpatient setting working with individuals, families, and groups to address and treat mental health disorders and to promote optimal functioning. The Therapist uses a variety of therapeutic techniques to deal with a wide range of mental health concerns including, but not limited to: depression, addiction, suicidal impulses, stress management, problems with self-esteem, issues associated with aging, family, parenting, marital or other relationship problems. Develop treatment plans, using person-centered processes by assisting persons-served in setting goals linked to addressing identified needs. Make certain consumer goals address situational stressors, family relations, interpersonal relationships, mental health issues, life span issues, psychiatric illnesses, addictions, domestic violence, or other trauma. Conduct ongoing assessments of persons-served, and evaluate effectiveness of Treatment Plan goals and interventions, no less than every 90 days. Provide individual, family and/or group therapy using therapy interventions such as: active listening, consistent eye contact, unconditional positive regard, and empathic responses Facilitate, for persons-served, access to community resources and social support networks that provide support, friendship, love and hope. Perform transition planning and follow-up contact with persons served. Complete Transition/Discharge plans within 14 days of event. Identify emergency crisis and make immediate clinical assessments and respond according to accepted crisis intervention methods and techniques; coordinate other services, as needed Perform miscellaneous job related duties as assigned by supervisor. Shift Notes: Monday-Friday, roughly 9-5:30pm, weekend rotation (1 per month) Physical Therapist I : This position delegates responsibility to the Physical Therapist Assistant and Aides. Activities of this position are patient focused and utilize assessment, planning, intervention, and evaluation in accordance with physical therapy standards, physician orders, and governing policies and procedures. ESSENTIAL DUTIES AND RESPONSIBILITIES: Evaluates the patient's function by performing formal/informal testing measures to determine the skeletal, neurological, respiratory, and cardiovascular status. Develops a goal specific treatment regime based on assessment data and the patient's tolerance to carry-out the treatment plan as prescribed. Submits a written treatment regime to be included as part of the plan of treatment authorized by and coordinated with the physician. Updates plan of care as needed to reflect changes in patient status. Co-signs verbal orders received by physical therapist assistant as necessary. Devises a schedule of treatment, which is frequency and duration specific to be discussed with patient/family/caregiver and coordinated with clinical support staff. Recommends adaptive/supportive equipment based on assessment; contacts appropriate representative and/or supplier to place order and assists in securing equipment for patient. Documents the patient/family/caregiver response to treatment and changes in patient condition per affiliate documentation policies and/or standards and in compliance with regulatory agencies. Completes daily documentation necessary for billing procedures per affiliate policy and in compliance with regulatory agencies. Provides treatment intervention and demonstrates competence to facilitate or restore optimal functioning specific to therapist's patient care area.
