What You'll Do Act as a liaison between drivers and those that are scheduling rides. Keep record of driver attendance, tardies on the log central sheet. Provide resources to chauffeurs to fulfill their job requirements, including keys, vehicle, manifest, forms, instructions, etc. Create assigned route schedules with the goal of making 100% on time transport of passengers. Communicate via two-way radio or cell phone (Tablet) any cancelations or changes to the drivers while they are on the road. Answer and direct phone calls to appropriate staff or complete task requested of caller. Assist drivers with emergencies by determining the best way to address the emergency, sending additional staff, fire, police, emergency unit, verbal support and guidance. Ensuring communication with appropriate person(s) as soon as possible. Qualifications Minimum Education required: High school diploma or GED. Minimum 6 months prior work experience in a customer service position. Prior work experience in transportation services, as either a chauffeur or dispatcher working with senior citizens and/or disabled individuals preferred Valid driver's license and acceptable driving record in accordance with Hope Network's policies. Must have, or obtain within 3 months of hire, a chauffeurs license. Addendum: OAKWOOD PROGRAM 1.The Oakwood Supervisor provides case management services for all Oakwood residents. a.Facilitates the care plan for the treatment episode, proactively monitors clinical quality, and facilitates timely discharge from the program. During the treatment episode, they b.Coordinate the efforts of psychiatry, psychiatry, psychology, nursing, PT, OT, SLP, Rec, SW, and integrate the family in the plan. c.Responsible to facilitate real time communication from the treatment team to the external Nurse Case Manager, insurance adjustor, and external physicians serving the patient. d.Serve as an advocate and liaison for and on behalf of consumers' rights and benefits. e.Evaluate, facilitate and monitor services and client progress in relationship to established goals and objectives. 3.Responsible for a smooth intake process for each new consumer assigned to their case load including: •Ensure the consumer is assigned a complete treatment team as determined by their needs and physician orders and that their schedule is complete; •Address any admission day medical, personal care, and rehabilitation needs; •Verify that all priority evaluations are completed and findings are implemented to ensure a safe and comfortable transition for the consumer; •Complete all required intake documentation; •Obtain copies of all necessary documents for invoicing of services including identification, insurance plan, insurance cards and legal forms if not yet received 4.Responsible to oversee the medical care of the consumer by: •Work cooperatively with nursing and direct care staff; and •Ensure medical follow-up and care is provided including but not limited to scheduling and coordinating medical appointments and transportation, securing medical orders and treatment prescriptions at required intervals, and attending medical appointments when appropriate. 5.Serves as the primary point of contact for outside parties such as physicians, rehabilitation nurses, adjustors, families/guardians, and other service providers. Keep all external parties informed, at their desired level and frequency, of the progress being made in the program. Portrays a positive, professional image of the program. 6.Coordinate with the treatment team to ensure that consumer programs are coordinated and outcome oriented showing movement through the continuum of care, as appropriate. Challenge the treatment team if progress slows or goals are not being achieved. Proactively escalates the case for review by a program manager/director when the case presents some challenge to achieving successful outcomes or meeting the predicted outcomes. 7.Makes internal referrals that allow HNNR new opportunities to serve a consumer who has either made progress and can benefit from additional services, or has developed previously unseen concerns that could be addressed by a referral to the appropriate professional. 8.Leads the treatment team in a manner which is conducive to problem solving and cooperation. Works with the team to develop a treatment plan that is functional, measurable and goal directed. Ensure differences of opinion among the HN treatment team are resolved before meeting with external stakeholders. 9.Ensures that extraneous information needed for decision making, planning and execution of plans is provided and available to all team members. Able to generally represent the treatment plan and current issues during team meetings that either support the need for ongoing care or indicate discharge to less intensive services. 10.Communicates urgent information in a timely fashion and in ways that ensure all team members, including external, receive the information needed in a timely fashion. Utilizes multiple avenues of communication to guarantee all team members have received the information. (Note - email is not considered received until it has been read therefore cannot be used for time sensitive communication.) 11.Oversees the consumer's medical chart to ensure that information is updated in a timely fashion, physician scripts for services are current, and written documentation conforms to our licensing and accrediting requirements. Utilizes tools and alerts within the EMR to facilitate team communication. 12.Prepares reports that summarize the consumer's treatment and plan as provided by the entire treatment team, and which can be presented to funders to request funding extensions for the consumer to continue services. These reports include: Team Treatment Plans, Periodic Reviews and Discharge Summaries. Follow prescribed formats and program requirements. Prepare special letters as needed including letters of Medical Necessity. 13.Understands the common funding sources for our level of care including auto no-fault, commercial health, Medicaid MOU program, and worker's compensation. Understands and can explain patient responsibilities based on funding mix and coverage, including family responsibility of copayments and deductibles. Works with families to address collection issues. Reviews financial reports by case load as provided by the Finance Department. 14.Works with Funding Specialist to determine funding parameters, and communicates the limitations to all involved parties. Monitors team adherence to prescribed parameters. This includes monitoring of services for those consumers admitted under per diem agreements to ensure appropriate service provision is maintained. Researches unusual or limited funding situations and modifies program recommendations to best fit the availability of funding and desire of the financial guarantor while meeting the intended program outcome goal. 15.Works closely with the social work department to ensure that a discharge plan is in place and community resources are secured. Refers family adjustment issues and other appropriate concerns to the social work department. 16.Provides a quality review of consumer schedules and program participation. Ensures that all consumers on their case load are receiving the prescribed therapies and medical services at appropriate frequencies. Works with scheduling to resolve any issues of under treatment. 17.Is responsible for the timely, accurate and complete collection of outcome data for their caseload as identified by program administration. 18.Provides organized and effective communication to internal and external sources both in written and verbal formats. 19.Works cooperatively with program administration to address quality concerns stated by consumers, families, and outside professionals. Educational / Talent Requirements: 1.Bachelor's degree from an accredited university or institution of higher learning preferred. (Will consider related years (3+ years in residential-based supervisory role) in lieu of degree.) 2.Demonstrated ability to communicate in verbal and written formats to meet position requirements. 3.Computer skills including use of Microsoft Office. Case Management Responsibilities: Meet with all kids on case management case load at least weekly. Coordinate and ensure child involvement in the Person Centered Planning Process (PCP) for assigned consumers. Assure responsibility for the development & implementation of each assigned child's Individual Plan of Service. Link children to necessary ancillary services. Act as an advocate and/or a liaison coordinating services among providers of service and referral source. Maintain children's records - meeting all appropriate standards according to HNBHS and all regulatory bodies. Participate in legal proceedings including petitioning and testifying as necessary. Facilitate the coordination of children's intakes, discharges, and transfers to and from other facilities programs, agencies. Ensure that children are transported to appointments, accompanying them when medically necessary. Obtain relevant medical/treatment information from appointments. Link and coordinate follow-up of professional recommendations. Facilitate communication with guardians, payees, and family members with regard to children's care, financial and treatment issues. Meet with assigned children face-to-face as medically necessary in their Individual Plan of Service. Facilitate hospitalizations as needed and monitor progress by attending hospital medical/team meetings and coordinating with hospital social worker. Provides consultation to direct care staff regarding clinical and program issue, where applicable. Participate in children's program reviews and team meetings, where applicable. Administrative Responsibilities: Participate in committees, task forces, and other related meetings and activities as assigned by supervisor. Attend agency meetings: clinical, administration and management. Network with relevant agencies and providers as needed. Follow general safety practices. Report any unsafe conditions to the manager of that location. Complete and submit Service Activity Logs (SAL's) in accordance with agency policy/protocols. Coordinate benefits and verify benefit status monthly. Other duties as assigned by supervisor. Educational / Talent Requirements: Minimum Education required: Bachelor of Human Behavioral Science Degree (Social Work, Counseling, Psychology, etc.). Valid driver's license with acceptable driving record according to Hope Network insurance carrier definitions. Must complete a Central Registry Clearance form for the State of Michigan and any state in which residence and/or employment was previously established. Michigan Central Registry Clearance must be completed on an annual basis. History of working with children with intensive therapeutic and behavioral needs is highly preferred. A comprehensive knowledge of Trauma-Informed Care and Gentle Teaching foundational principles and experience implementing and working within these frameworks is preferred. What You'll Do Cover open shift at Neuro Rehab programs within the Grand Rapids area Encourage positive relationship building & Promoting Independence Transportation and Participation in Community Activities Personal Care/Assistance with Activities of Daily Living Medication Administration/Health Monitoring Cooking/Meal Prep/Dietary Support Qualifications Meet one of the below: Certification as a CNA or a bachelor's degree from an accredited university or institution of higher learning (preferred) -or- High School Diploma or G.E.D with three years of experience -or- One year as an HNNR Rehab. Tech. and have completed all HNNR Core and Advanced Medical Rehab Tech Skills requirements. Work experience in a licensed residential setting preferred. Valid driver's license.
