Principle Responsibilities:The following are the essential functions of the position. Contracted doctors are responsible for carrying out these functions:· Assumes psychiatric assessments/evaluations and mental status examinations (MSE's) of patients/clients, and providing diagnosis according to the current edition of the Diagnostic and Statistical Manual of Mental Disorders (DSM) approved by the American Psychiatric Association (APA). · Use Hope's current electronic health record system and follow Santa Clara County Psychiatric Medical Guidelines. Service Delivery

Promote the physical and mental wellbeing of residents.

Wake up residents who have requested an early wake up.

Maintain an accurate account of shelter activity by reviewing the daily logbook at shift change and updating it every thirty minutes.

Answer incoming telephone calls.
Ensure that the medication file remains current and medications are accessible to residents. Complete incident reports, as necessary.

WORK ENVIRONMENT: The work environment varies depending on the shift. The daytime and swing shifts are extremely busy and fast-paced. The work environment is relatively quiet during the graveyard shift. Housing Advocates on all shifts must be prepared to handle emergencies,
e.g. verbal or physical altercations, etc. The HA should also be prepared to collect and bag clothes and dirty linens of former residents who are incontinent.

EOCP's Crossroads is part of the centralized hub that provides housing services, support and referrals for single men and women who are homeless. The Coordinated Entry initiative will assess individuals who are literally experiencing homelessness and will be connected to an array of services, including emergency and permanent housing.
The Coordinated Entry Specialist conducts screenings on the phone, in person, and in the community to determine eligibility for housing services. This position uses Shelter Point to place individuals in shelter and assists non-literally homeless individuals by using triage, problem- solving and providing referrals for services.
ESSENTIAL DUTIES AND RESPONSIBILITIES
Service Delivery
Meet with all homeless individuals on both a scheduled and drop in basis; by phone, in person, and through street and venue outreach

Core Housing Preparation Work
Assess and begin to address client housing histories and barriers - positive references, credit history, rental history and prior evictions, criminal history, registered sex offender status, outstanding debts, outstanding warrants
Use housing history to inform preparation work, complete early to avoid surprises
Get to know members or potential members of the client's household including pets and companion animals
Assess potential to connect client with family/friends in stable housing
Assess eligibility for permanent housing resources - deposit/move-in financial assistance, rapid rehousing, affordable housing, and permanent supportive housing
Assess the client's financial and resource situation and potential budget for housing - help with income and benefits acquisition, develop plan to help fund move-in costs
Help families create tenant resumes - key information to use on housing applications
Securing Housing
Help homeless individuals identify and pursue potential housing opportunities besides permanent supportive and affordable housing. Help clients tour neighborhoods and properties - address rejections as part of reality testing - “at least look at the place, you don't have to take it”; provide options and discuss trade-offs
Help homeless individuals complete and submit required housing applications and other materials, including housing navigator and/or other support person(s) on applications as a contact. Include release of information. Include advocacy/support letters with initial application
Help homeless individuals complete housing program or site specific paperwork to obtain particular units or subsidies
Assist homeless individuals with obtaining the resources necessary to apply for and move-in to housing (application fees, security deposits, first month rent, moving service, furnishings, bedding, etc.)
Moving-in and Transitioning (average of 6 months of support)
Complete unit inspection and document any damage or issues prior to move-in
Review key elements of rental agreement and expectations to ensure understanding Review any subsidy agreement as well, if needed
Establish utilities for the housing unit
Apply for low-income assistance utility programs
Assist with obtaining furniture, fixtures, and other move-in needs
Update address with key agencies and contacts including the post office, health insurance, public benefits, and service providers
Establish method for ensuring rent payments are made on time
Develop a housing crisis response plan outlining plans if challenges arise that may jeopardize housing stability including key emergency contacts for service and housing- related issues. Possible housing challenges include - mental health/substance use relapse, health and cognitive issues such as, non-payment of rent, conflicts with neighbors or landlord, unauthorized guests, hoarding/cluttering, smoking and fire hazards, plumbing/flooding issues
Transition ongoing supports to appropriate service providers and natural supports using a Critical Time Intervention (CTI) model
Review ability of homeless individuals to manage activities of daily living (ADLs) and instrumental activities of daily living (IADLs)
Utilize information and Housing Specialist(s) to find landlords willing to accept housing subsidies for clients approved for voucher or tenant-based housing subsidy programs
Assist clients with move-in to new unit and with transitioning support to permanent supportive housing service provider(s) and/or other resources
Help address any challenges with independent living. Consider need for In-Home Supportive Services (IHSS)
Support IHSS application and worker selection process if needed. Help develop skills relevant to living with others in residential community - conflict resolution, communication skills, raising concerns with neighbors and landlords, etc.
Assist with helping individuals create a sense of home - personalization, inviting guests, art work, etc.
Administration
Update and/or maintain all required tools, drop box and screening forms
Gather statistical data to monitor program effectiveness
Assists with providing input for grant reports
Attend weekly or biweekly program staff meetings, trainings and all-staff meetings that are held during the day
Successfully complete all HMIS Privacy and Security trainings
Maintain confidentially regarding clients, staff and other agency affairs

Qualifications EDUCATION AND EXPERIENCE
Required Qualifications
Bachelor of Arts degree in Social Work, Psychology or related field
Minimum three-year experience of working with the homeless, people who are dually diagnosed, or living with substance
Knowledgeable about dynamics of homelessness, substance abuse, mental health, and the chronically homeless
Ability to build supportive and respectful working relationships with individuals who are diagnosed with a mental illness that instill hope and promotes self-determination
Familiarity with public benefits, chemical dependency, health, and social service programs in Alameda County
Excellent knowledge of local housing market
Knowledge and experience utilizing basic counseling and Motivational Interviewing (MI) techniques
Ability to use conflict-resolution skills to de-escalate volatile situations with clients
Excellent written and oral communication skills

The Prep/Line Cook: Accurately and efficiently cook all food as well as prepare and portion food products prior to cooking. Line Cook is Responsible /performing basic food preparation tasks and assisting the Food Service Manager and Head Cook. They typically complete smaller responsibilities like cutting meat, mixing sauces or chopping vegetables. Line Cooks are often assigned a certain task each time they work and will stay in that section of the food preparation line for a majority of their shift unless they are asked to move about to do other kitchen task. They're responsible for cleaning prep station and making sure it's set up properly before the start of their shift. They'll also stock the kitchen as needed. Line cooks must regularly check the food prep and storage areas to ensure they don't violate any health codes or safety regulations. Line Cook needs to have an up to date SerSafe Food Handling Certification. Must have Valeted Driver's license

Organize donated food stock, regularly rotating using the FIFO system, and ensuring that food items in freezers and refrigerators are covered and labeled appropriately.
Ensuring that the kitchen operation procedures and hygiene meet food safety standards and regulations.
Once a week do a thorough clean in the kitchen.
Line Cook are required to work in a fast pace.
Line cook must be great multi-tasking often have several preparation tasks to complete at once.
Assumes 100% responsibility for quality of products serve d.
Open to learn on the job even if they have prior work experience. This is because EOCP follows different procedures, and there is a wide range of safety and regulatory policies.
Prepares a variety of food items for cooking in ovens, fryers and a variety of other kitchen equipment.
Ability to work overtime if needed.
Complete opening and closing checklists.
Refer to Daily Prep list at the start of each shift for assigned duties.
Knows and complies consistently with our standard portion sizes, cooking methods, quality standards and kitchen rules, policies and procedures.
Stocks and maintains sufficient levels of food products at line stations to assure a smooth service period.

Principle Responsibilities:The following responsibilities represent the essential functions of the position. Essential FunctionsThe following responsibilities represent the essential functions of the position. An employee in this classification is responsible for carrying out the following functions:1. Function independently and autonomously in providing home based early intervention services to infants and families, and maintain assigned caseload.2. Provide evaluation and treatment addressing six major developmental domains including: cognitive, language, social, gross and fine motor and self-help skills.3. Perform comprehensive neurological and physical assessment of infants including muscle tone, range of motion, reflex development and movement patters. Identify neurological sequelae and develop and implement treatment plan based on assessment findings.4. Perform oral-motor feeding assessment and identify oral motor pathology and sensory based feeding disorders, provide feeding therapy and or oral-motor treatment as indicated.5. Provide updated, developmental information at scheduled IEP's and IFSP's and establish appropriate goals and objectives.6. Independently recognize those families in crisis and provide appropriate consultation and referral regarding issues related to child abuse, domestic violence, parental stress/conflicts and substance abuse.7. Presents in-service trainings to HOPE staff and provide trainings to outside agencies.8. Collaborate with physicians and outside agencies regarding developmental and other health related issues.9. Act as consultant to other agency professional including, physical therapists, speech pathologists and infant educators. Help supervise and train occupational therapy and early intervention students.10. May perform other duties as assigned to assure efficiency of program. Qualifications RequiredBachelors degree in Occupational Therapy plus registration as an OTR. plus one year of related experience working with people with disabilities in a rehabilitation environment.

Responsibilities of the Therapeutic Behavioral Specialist:
1. Provides 1:1 assessment, training and follow up in the consumer's home and community as identified in their Mental Health and TBS assessments2. Implements positive behavior programs for qualified clients in the community.3. Assure compliance with safety and confidentiality standards.4. Acts as a program monitor for assigned clients. Complete required forms and accountability documentation, incident reports, case notes, termination, client attendance, billing, and other required documentation.5. Performs behavioral interventions as prescribed.6. Assess and document client progress, performs quality control.7. Maintains all assigned client case records. Participates in case reviews and goal planning. Schedule transportation, prepare required documentation, reporting.8. Responsible for maintaining a professional relationship with consumers, parents, care providers, co-workers and appropriate others. Represents HOPE in the community.9. Acts as advocate on behalf of clients served, providing intervention and consultation as needed.10. Acts as a mandated abuse reporter.11. Perform other duties, as assigned, to assure efficiency of program services.
Minimum Qualifications:
The Therapeutic Behavioral Specialist should possess a Bachelor's degree in Psychology or its equivalent as approved by SCCMH.
