Job Summary/Duties:Performs needed psychiatric assessments, and provides ongoing medication management services for referred developmentally disabled patients with mental health disorders. Be eligible for MediCal/Medicare certification. Authorized to prescribe controlled substances.
Principle Responsibilities:The following are the essential functions of the position. Contracted doctors are responsible for carrying out these functions:· Assumes psychiatric assessments/evaluations and mental status examinations (MSE's) of patients/clients, and providing diagnosis according to the current edition of the Diagnostic and Statistical Manual of Mental Disorders (DSM) approved by the American Psychiatric Association (APA). · Use Hope's current electronic health record system and follow Santa Clara County Psychiatric Medical Guidelines. Interface closely with medical director, nurses, clinicians and case managers for a client-centered approach to mental health care...· Obtains and reviews data, including laboratory and other diagnostic procedures or consultations pertinent to the evaluation and treatment of patients/clients· Provides detailed written reports of assessments/evaluations, MSEs, diagnoses and summaries of treatment provided, and completes all medical records as required.· Provides specialized input in the development and implementation of individualized mental health treatment planning for assigned patients/clients, including diagnosis in collaboration with the clinical staff.· Provides psychiatric services in outpatient clinics (via telehealth, and/or in the office)· Provides consultation, review, and peer guidance for other psychiatrists or therapists in the evaluation and treatment of patients.· Consults with the medical director or clinic Director about policies and issues relating to the evaluation and treatment of mental health patients/clients as necessary.· Participates in quality assurance, quality improvement and performance improvement programs and activities when needed.· Provides expert advice and counseling for client families and care givers regarding patient/client issues, problems and course of treatment, presenting information about mental health diagnoses, intellectual disabilities and how the two intersect. .· Prepares legal and other court-related documents, written reports, when deemed necessary and appropriate.
Minimum Qualifications:Must be Board Certified or eligible by the American Board of Psychiatry and neurology in adult and/or child psychiatry. Reports To: Director Supervisory Responsibility: As Assigned Required knowledge, Skills and Abilities: 1. Ability to perform specialized psychiatric/medical services in general psychiatry, evaluate and prescribe medication, (including controlled substances) and other treatments2. Provide qualified reimbursable services under guidelines of Medicare, Medi-Cal, Regional Centers, and County Mental Health and other funding sources3. Ability to establish and maintain effective working relationships with patients/clients, staff and others.4. A belief that all people are valued members of the community.5. An understanding of the principles of normalization and commitment to community integration.6. Ability to work independently and cooperatively as a member of the team.7. Provides individualized service to HOPE clients to assess and remediate personal and social barriers, which inhibit success in community settings. Takes a lead role in providing job and skill training for HOPE clients working in the community, provides support and on the job training for other job coaches. 2. Implements start-up projects and services, provides support for new locations. 3. Provides needed supports to assure client success in employment placements. May work varied hours depending on client and program needs. Evaluation, training, goal setting and follow-up services for successful job retention. 4. Acts as a liaison with the employer, providing training and supports, consultation on employment related issues for success of long-term employment. 5. An act as a program monitor to assigned clients and provides advocacy and referrals as needed. Assists clients with accommodations and ADA related issues. 6. Represents the Agency in the community. Maintains a professional relationship with family members, employers, and funding sources. Complies with employer policies and procedures while assigned to their locations 7. Principle Responsibilities of the Developmental Specialist 1. Function independently and autonomously in providing early intervention services to infants and families; maintain a caseload of children, with number determined by FTE.2. Conduct assessment and evaluation, addressing five major developmental domains as needed.5. Maintain productivity of 75% based on weekly billable hours.6. Minimum Qualifications: To practice as a Developmental Specialist at entry level, the Developmental Specialist must hold a minimum of a Bachelor's degree in the field of Child Development, Human Development or related field. In addition, the Developmental Specialist must have received specialized training in early intervention and meet the competencies outlined under Early Intervention Personnel Standards. One year experience working with infants and toddlers in the Early Intervention arena. Master's degree in Special Education and bilingual are a plus. About the Licensed Mental Health Clinician role: The Licensed Mental Health Clinician provides assessment and diagnosis to clients with dual diagnosis, provides individual, group and family psychotherapy, prepares individual service and coordination plans, documents clinical outcomes and acts as an advocate. Responsibilities of the Licensed Health Clinician:1. Provides individual, group and family therapy to persons with a developmental disability who are diagnosed with severe mental illness.2. Utilize professional education and training to diagnose and treat the clients' mental illnesses.3. Provide psychotherapy to empower clients to identify and manager their psychiatric symptoms to enable them to be maximally functional in the larger community.4. Constantly assess clients for potential harm to self, harm to others, or potential victimization and take appropriate measures to maintain safety. This includes, initiate involuntary hospitalizations when necessary to maintain safety.5. Use professional training and credential to advocate for clients with other agencies, educate other agencies to recognize and deal with the needs of clients who have a developmental disability and are diagnosed with a mental illness.6. Develop relapse prevention strategies with clients and their support systems to prevent future psychiatric hospitalizations and loss of community placements.7. Participate as an integral part of a multi-disciplinary team designed to maximally meet th ps8ychiatric and psychological needs of clients.9. Regularly attend training to update and keep current skills and maintain license.9. Maintain appropriate treatment records and documentation as required.10. Other duties as assigned to assure efficiency of the program. Qualifications of the Licensed Mental Health Clinician:The Licensed Mental Health Clinician should possess current licensure current as a LCSW, LMFT, or Clinical Psychologist and experience working with persons with developmental disabilities. All licensed clinicians, upon hire or upon obtaining licensure, are required to be Medicare credentialed by submitting a Medicare enrollment application. Required Skills of the Licensed Mental Health Clinician1 Basic computer skills in work processing and spreadsheet application.2. Ability to observe, evaluate, document and communicate verbally and in writing.3. Ability to accept supervision and work cooperatively as a team member with co-workers as well as with the client and community resources.4.
