SUMMARY OF JOB RESPONSIBILITIES : The Hospice and Palliative Medicine Nurse Practitioner (HPMNP) is an advanced practice nurse, providing clinical care in accordance with state and federal practice law for patients in all hospice and palliative care settings. The HPCNP works in collaboration with the Hospice and Palliative Medicine Physician (HPMP) and the hospice interdisciplinary group (IDG) to ensure patients receive appropriate symptom control; facilitates discussions relative to the plan of care and advance directives; and provides critical regulatory and compliance requirements including face-to-face and billable/non-billable visits. The HPCNP collaborates externally with attending physicians, specialists, and other providers as applicable for the purpose of coordination of care to effectively meet the medical, psychological, and spiritual needs of the patient. QUALIFICATIONS : Board Certified Advanced Practice Nurse licensed in the state of Colorado. Prefer two years of experience in home care, palliative care, or oncology. Seeks or maintains certification in Palliative Medicine. Controlled substance license with current DEA license number. Must have and maintain certification (or ACHPN) or obtain no later than the end of third year of employment. Current CPR Certification. Eligible or currently possess a Medicare/Medicaid UPIN number. Eligible or currently credentialed at local hospitals. Eligible to bill insurance for palliative care and symptom management visits. Facilities Technician - Full-time HopeWest is a well-established non-profit organization that has a great opportunity for an experienced Facilities Technician. Come join our 400+ staff and be a part of our amazing Facilities Team that supports our organization with a home base in Grand Junction and occasional travel to our outlying offices in Western Colorado. The right person for this position will have the stamina to perform the physical, fast-paced needs while working with a tight knit team. This includes inspections, repairs, preventative maintenance, and all the required documentation and communication related to each task. This position shares evening, night, and weekend on call to be available for urgent needs. QUALIFICATIONS A minimum of 5 years of related experience. Working of knowledge of electrical and mechanical systems. Ability to add, subtract, multiply and divide in all units of measure, using whole numbers, common fractions, and decimals. This role ensures the delivery of high-quality, evidence-based, trauma-informed psychosocial care across HopeWest. Key responsibility of the supervisor is to serves as a clinical resource and promotes excellence in delivery of patient care, documentation, development of standards of care, and interdisciplinary collaboration. Additionally, the Social Work Supervisor assists in the oversight of time-off requests and scheduling for Social Workers, coverage coordination, supervision of variable staff, compliance monitoring, and facilitation of discipline-specific meetings. QUALIFICATIONS Must be a Licensed Clinical Social Worker (LCSW) with active registration through Colorado DORA. 2 years of LCSW experience preferred. Advanced Certification in Hospice and Palliative Care preferred. Must be able to provide clinical supervision for LCSW candidates. Advanced training in trauma-informed care, EMDR, DBT, and crisis intervention preferred.
