    JOB SUMMARY- On-Site | In-Office The OASIS Review Specialist is responsible for analyzing data integrity and consistency of OASIS documentation and assessment processes. This position will ensure appropriate ICD-9 - ICD-10 coding and sequencing and will work with clinical staff to clarify documentation and data integrity issues. DUTIES & RESPONSIBILITIES Prospectively reviews all OASIS assessments to ensure appropriateness, completeness, and compliance with federal and state regulations and organization policy. Utilizes OASIS variation or alert reports when reviewing OASIS data. Ensures appropriate ICD-9 - ICD-10 coding and sequencing as it relates to the patient's medical condition, including any co-morbidities. Consults with appropriate clinical staff to clarify any data integrity issues and works with clinician to make appropriate corrections per organization policy. Reviews visit utilization for appropriateness of care guidelines and patient condition; reports potential financial losses and/or underutilization to the clinical manager/designee. Notifies OASIS Review Nurse and Agency Managers of problematic trends as a result of OASIS review. Participates in Quality Improvement and Corporate Compliance activities as assigned. Assists with other chart audit activities as assigned. Maintains professional and technical knowledge by attending educational workshops and reviewing professional publications. JOB SUMMARY The medical social worker contracted or employed through the organization is responsible for the implementation of standards of care for medical social work services and for adherence to all conditions in the Service or Employment Agreement. Assesses the psychosocial status of patients and families/caregivers related to the patient's terminal illness and environment and communicates findings to the registered nurse and other members of the interdisciplinary group. Provides an assessment in the patient's identified residence and assistance when this is not safe and another plan is required. 2. Carries out social evaluations and plans intervention based on evaluation findings. Counsel's patient and family/caregivers as needed in relationship to stress, and other identified coping difficulties. 3. Maintains clinical records on all patients referred to social work. 4. Provides information and referral services for organization patients and families/caregivers regarding practical and environmental needs. 5. Provides information to patients and families/caregivers and community agencies. 6. Serves as liaison between patients and families/caregivers and community agencies. 7. Maintains collaborative relationships with organization personnel to support patient care. 8. JOB DESCRIPTION SUMMARY The home health and hospice aide is a paraprofessional member of the interdisciplinary group who works under the supervision of a registered nurse and performs various services for a patient as necessary to meet the patient's personal needs and to promote comfort. If unable to perform a certain task, report to the Case Manager immediately, and document the communication. Meeting safety needs of patient and using equipment safely and properly (foot stools, side rails, O2 etc.) Giving personal care including baths, back rubs, oral hygiene, shampoos and changing bed linen as often as assigned. Assisting in dressing and undressing patients as assigned. Planning and preparing nutritious meals and snacks/fluids as assigned. Assisting in feeding the patient as assigned. Taking and recording oral, rectal and axillary temperatures, pulse, respiration and blood pressure when ordered with appropriate completed/demonstrated skills competency. Providing proper care and observation of patient's skin to prevent breakdown of tissue over bony prominence. Reporting on patient's condition and significant changes to the Case Manager. Aware of the caregiver or other individuals living with the patient, interpersonal issues and works to teach them regarding the POC as appropriate. POSITION QUALIFICATIONS Meets the training requirements of the State. Effective August 14, 1990, a person who has successfully completed a state established or other training program that meets the requirements of S 484.36(a) and a competency evaluation program, or state licensure program that meets the requirements of S 484.36(b), or a competency evaluation program or state licensure program that meets the requirements of S 484.36(b). At least 18 years of age. Ability to read and follow written instructions and document care given. Understands hospice philosophy, comfortable providing specialized care to the terminally ill. Satisfactory references from previous employers. Is self-directed with the ability to work with little direct supervision. Secure with issues of death/dying. Provides a calm manner when in a patient's home. Has empathy for the needs of the ill, injured, frail and the impaired. Possess and maintains current CPR certification. Identifies patient and family/caregiver needs for other home care services. Consults with the supervising registered occupational therapist and assists with necessary referrals, as appropriate. 9. Participates in instructing patient's family/caregiver and other Organization health care personnel in patient's treatment regime as delegated by the registered occupational therapist. 10. Is supervised by the registered occupational therapist according to organization policy and state regulations.
