JOB SUMMARY The medical social worker contracted or employed through the organization is responsible for the implementation of standards of care for medical social work services and for adherence to all conditions in the Service or Employment Agreement. DUTIES & RESPONSIBILITIES 1. Assesses the psychosocial status of patients and families/caregivers related to the patient's terminal illness and environment and communicates findings to the registered nurse and other members of the interdisciplinary group. Provides an assessment in the patient's identified residence and assistance when this is not safe and another plan is required. 2. Carries out social evaluations and plans intervention based on evaluation findings. Counsel's patient and family/caregivers as needed in relationship to stress, and other identified coping difficulties. 3. Maintains clinical records on all patients referred to social work. 4. The physical therapy assistant provides physical therapy services under the direction of the physical therapist and according to the physician's plan of care. DUTIES & RESPONSIBILITIES Provides direct physical therapy according to directions of physical therapist and in accordance with the physician's plan of care. Treats patient to relieve pain, develop or restore function, and maintain maximum performance. Directs and aids patients in active and passive therapeutic exercises, muscle reeducation, gait and functional training, transfer activities and prosthetic training. Utilizes physical agents, including, but not be limited to heat, cold, water, light, electricity, ultrasound and massage. The physical therapist contracted or employed through the Organization is responsible for providing physical therapy services and for adherence to all conditions outlined in the Personnel Services Agreement. II. DUTIES & RESPONSIBILITIES 1. Assumes responsibility for the ongoing interdisciplinary assessment and development of the individualized plan of care in partnership with the patient, representative (if any), and caregiver(s); 2. Provides patient, caregiver, and family counseling. 3. Provides patient and caregiver education. 4. Prepares clinical notes, and progress summaries based on the attainment of goals. 5. Communicates with the physician who is responsible for the home health plan of care and other health care practitioners (as appropriate) related to the current home health plan of care; 6. Provides physical therapy services to patients according to a written physician's plan of care. This may include, but will not be limited to: i. Assessing and evaluating therapeutic/rehabilitative/functional status, and participation in the development of the total plan of care. Evaluates home environment. ii. Initiates physical therapy program and instructs other personnel and/or family/caregiver members in certain phases of physical therapy with which they may work with a patient, as well as instructing them as to the goals of the physical therapy program for the patient by participating in case conferences. 8. May train patient in the use of prosthetic device. 9. Identifies patient and family/caregiver needs for other home health services and refers as necessary. 10. Participates in discharge planning for patient. 11. Provides in-service education programs for nursing organization personnel as needed. 12. Participates in peer consultation process. 13. When therapy is the only skilled service, instructs, supervises and evaluates home health aide care every two (2) weeks. 14. Supervises Physical Therapy Assistants according to organization policy and state regulations. 15. Participates in the HHA's quality assessment and performance improvement program and HHA-sponsored in-service training. Horizon is one of Idaho's oldest and largest Home Health agencies. Qualifications: LPN currently licensed in the State of Idaho Must have a current driver's license and auto insurance policy Must have a reliable vehicle Home Health experience is preferred ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES 1. Provides direct patient care as defined in State's Nurse Practice Act. 2. Implements plan of care initiated by the registered nurse. 3. Provides accurate and timely documentation consistent with the plan of care. 4. Provides patient and family/caregiver education and information pertinent to diagnosis and plan of care. 5. Participates in coordination of home health services, appropriately reporting the identified needs for other disciplines (HHA, OT, PT, MSW, ST, and Dietician) to the registered nurse and/or Clinical Supervisor or Director of Nursing. 6. Uses equipment and supplies effectively and efficiently. 7. Adhering to the organizations documentation and care procedures and standards of personal and professional conduct. 8. Participates in personal and professional growth and development. 9. Performs other duties as assigned by the registered nurse.
