Coordinate the behavioral and physical health care needs of service participants.
Monitor participant's medication and provide psychiatric nursing services consistent with physician orders and standard nursing practices.
Participate as a member of a focused team which provides Assertive Community Treatment services to ensure that service participants reach their maximum level of functioning in the community. Approximately 75% of the work is in the community.
Complete health and biopsychosocial assessments for every active participant.

SUMMARY
The Meetings & Events Planner will support the planning and delivery of meetings, conferences, and events by coordinating logistics, preparing materials, and ensuring smooth execution. This role will focus on organizing the details of events, assisting with vendor and budget processes, and providing on-site support to help deliver successful experiences. On-site support will include assisting with unboxing and repacking materials, moving tables, loading and moving pallets, and other hands-on event setup and teardown activities. The successful candidate must have the ability to perform these physical tasks as part of event execution.
WHAT YOU'LL BE ACCOUNTABLE FOR
The Meetings & Events Planner will coordinate event logistics including site selection research, food and beverage arrangements, AV needs, registration processes, and transportation. They will support the Director of Meetings & Events and Meetings & Events Manager in ensuring events are well organized, align with organizational goals, and provide a positive attendee experience.
The Planner will also assist in tracking event metrics, preparing reports, and collecting post-event feedback to help refine processes and improve future events. Onsite, they will provide operational support, help troubleshoot minor issues, and work with the team to ensure events run smoothly.
ESSENTIAL DUTIES AND RESPONSIBILITIES:

Event Planning & Coordination:
· Consult with key stakeholders to determine program goals, objectives, and budgets.
· Assist in organizing and facilitating pre-event meetings, ensuring active participation from all stakeholders and thorough discussion of event-related agenda items.
· Research and recommend meeting sites and activities that align with event objectives.
· Perform site visits when necessary.
· Develop comprehensive event plans, including meeting specifications, banquet event orders (BEOs), and timelines.
· Coordinate logistics planning, including agenda/program development, food and beverage menu selections, transportation, registration, housing, and audiovisual/technical needs.
· Oversee the management of shipments to and from CEDIA HQ, ensuring timely communication with all relevant departments prior to the shipping date and confirming that all items are properly packed and ready for shipment in advance.
· Track attendance, registration, hotel history, and attendee satisfaction.
· Maintain up-to-date records and compile status reports on event metrics.
· Regularly build, manage, and utilize event and project management platforms to support daily operations.
Vendor & Budget Management:
· Request and collect vendor proposals.
· Assist in the review of vendor contracts, working with the Director as needed.
· Coordinate with vendors to ensure services and materials meet event requirements.
· Help track expenditures, invoices, and order changes to support budget monitoring.
Onsite Event Management:
· Provide onsite operational support for meetings and events.
· Assist with registration, room setup documents, audiovisual needs, and event flow.
· Troubleshoot and resolve issues seamlessly to maintain a positive attendee experience, escalating larger challenges to the Director of M&E or M&E Manager.
· Provide exceptional customer service to all event attendees, ensuring a positive and seamless experience throughout the event.
Specific Event Responsibilities:
· Serve as a point of contact for venue meeting services, maintaining space layouts, schedules, and venue requirements.
· Help manage all food and beverage needs, including cocktail hours, buffet lunches, and continuous refreshment services, while staying within budget.
· Help coordinate setup and breakdown logistics for audiovisual needs, create room setup documents, and manage event flow.
· Support housing coordination including rooming lists and VIP/staff accommodations.
· Help coordinate on-site registration workflow, venue needs, and equipment for attendee check-in.
· Organize logistics for shipping, packing, and transit for event materials.
Post-Event Responsibilities:
· Participate in post-event debriefs and compile comprehensive event reports as needed, with recommendations for future improvements.

The Street Outreach Manager exists to provide direct oversight of Horizon House's street outreach team and coordination of Professional Blended Street Outreach (PBSO), a team of street outreach professionals representing multiple organizations working collaboratively to serve unsheltered individuals experiencing homelessness in Marion County. The position is accountable to the client, Horizon House, and the Professional Blended Street Outreach (PBSO) team. Case Work:
Engage with a diverse clientele to provide case management intervention that may include but is not limited to assessment of client strengths, resources and needs; crisis intervention; information and referral; advocacy and support.
Responsible for assessing and enrolling clients deemed eligible for housing through the CES Housing Support project operated by Horizon House.
Assist with client conflicts or other disturbances, intake and outreach efforts within the agency and community.
Coordination of Services:
Work collaboratively with client to develop effective housing plans with goals and action steps as well as to modify the case plan as needed.
Collaborate with community providers and ensure clients have access to services that may include addiction recovery support and/or treatment, medical care, mental health care and other resources and services specific to client needs and goals.
Provide ongoing support and advocacy with the goal of ensuring housing sustainability.
Record Keeping:
Maintain appropriate and timely documentation including but not limited to client identifying information, service records and case notes.
Documentation of encounters with clients as well as services performed on behalf of clients will be compliant with agency and Indianapolis Continuum of Care standards and protocols and will be entered into the HMIS adopted by the Continuum and utilized by Horizon House.
Legal and Ethical Practice:
Take appropriate action based on agency protocol and in accordance with the law.
Observe policies related to client confidentiality with respect to reports of abuse or neglect or other reports to law enforcement entities.

STH Outreach Professional
represents Horizon House in providing high-quality, housing-focused outreach support to households referred by the Streets to Home Initiative. Services are delivered using a Housing First, trauma-informed, equity-centered approach, consistent with Continuum of Care policies.
The position is accountable to clients, Horizon House, and the Streets to Home team. Accountability occurs within the context of ethical standards, organizational policies and procedures, Professional Blended Street Outreach (PBSO) standards, and contractual obligations.
This position is 100% funded through a contract between CHIP and Horizon House and is dependent on execution and renewal of said contract or securing other funding at the conclusion of the contract with CHIP. Nonrenewal of the contract or lack of a contract for replacement funding will result in termination of the Street Outreach Professional STH position.

·Complete health assessments for participants in the program.
·Maintain a record of medical and behavioral health history and ongoing medical and treatment records. Review all charts at least once a month to ensure that medical needs have been met and that documentation meets applicable standards.
·Consult with and assist team members regarding the development and implementation of the health care needs and goals.
·Negotiate the health care responsibilities of participants with provider agencies.
·Coordinate with team members' routine medical, vision, lab, psychiatric, and dental appointments. Ensure a mechanism for reporting and follow-up on results. May accompany the participant to an appointment when determined by the nurse in consultation with the team, and facilitate communication between the participant's primary care physician (PCP) and psychiatrist.
·Attend team meetings regularly to review medications, emergency intervention plans, and response to treatment, and to review health care needs, participants' response to treatment, and coordinate scheduling appointments.
·Monitor participants' medication. Verify insurance coverage by making phone calls to the appropriate insurance and pharmacies as needed. Assist the psychiatrist in completing prior authorizations if needed.
·Participate in discharge planning for participants from acute or extended care facilities or forensic settings to provide continuity of medical, substance abuse, and psychiatric care.
Qualifications:
· Licensed RN/LPN in the State of PA.
· Prior experience in community mental health and/or substance abuse treatment settings.
· Knowledge of psychiatric clinical and psychosocial rehabilitation practices.
· Excellent communication and interpersonal skills.
· Good writing and computer skills for documenting in an electronic health record.
· Team oriented.
· Detail oriented with strong organizational and analytic skills.
· Must be available for scheduled emergency on-call service.
· Must possess a valid driver's license with an acceptable driving record.

Some duties to include:

Bachelors degree required
Three to five years of experience providing services to homeless or low-income persons preferred (may include employment, educational or volunteer experiences, internships, and academic studies).
Ability to engage and establish positive rapport with a diverse group of individuals experiencing homelessness who are on the street or in other locations not intended for human habitation
Ability to work cooperatively and productively with outreach team members and staff of partner agencies and community organizations
Exceptional communication skills and a strong commitment to a team environment;
Knowledge of human behavior to provide assessment, intervention and support services
Working knowledge of chronic medical and mental health conditions, substance abuse behavior, and best practice treatment approaches
Detail-oriented with strong organizational skills
Proficient in all Microsoft and Google applications and ability to master use of an electronic database

Essential Functions:
Case Work:
Engage with a diverse clientele to provide case management intervention that may include but is not limited to assessment of client strengths, resources and needs; crisis intervention; information and referral; advocacy and support.
Responsible for assessing and enrolling clients deemed eligible for housing through the CES Housing Support project operated by Horizon House.
Assist with client conflicts or other disturbances, intake and outreach efforts within the agency and community.
Coordination of Services:
Work collaboratively with client to develop effective housing plans with goals and action steps as well as to modify the case plan as needed.
Collaborate with community providers and ensure clients have access to services that may include addiction recovery support and/or treatment, medical care, mental health care and other resources and services specific to client needs and goals.
Provide ongoing support and advocacy with the goal of ensuring housing sustainability.
Record Keeping:
Maintain appropriate and timely documentation including but not limited to client identifying information, service records and case notes.
Documentation of encounters with clients as well as services performed on behalf of clients will be compliant with agency and Indianapolis Continuum of Care standards and protocols and will be entered into the HMIS adopted by the Continuum and utilized by Horizon House.
Legal and Ethical Practice:
Take appropriate action based on agency protocol and in accordance with the law.
Observe policies related to client confidentiality with respect to reports of abuse or neglect or other reports to law enforcement entities.
Maintain ethical conduct/behavior with clients, co-workers, and colleagues.
Case Conferences and Staff Meetings:
Participate in multidisciplinary and/or community case conferencing designed to advance client progress toward goals.
Attend and participate in Housing Team meetings and other Horizon House staff meetings as assigned. Represent the agency at community meetings/events as assigned.
Education/Training:
Attend required agency education and training activities, including those that are position-specific. Participate in community training initiatives as available/assigned. Assist in training new staff and community partners as appropriate.
Provide peer assistance to other case management and housing team members.
Research & Development:
Participate in research programs such as the Point in Time Count and other programs when indicated. Participate in program development activities including but not limited to those related to policy & procedure, advocacy, funding, and evaluation with the Housing Supervisor or as assigned.
