We are currently seeking a Per-Diem Hospice Aide to join our group of dedicated and experienced professionals.

Knowledge, Skills and Abilities:

Competently performs a variety of basic patient care activities under the supervision of the RN to meet the personal needs and comfort of patients.

Hospice aide supervision must include following POC by participating in IDT, task competency, compliance with infection control and reporting changes in patient conditions.

The hospice aide must have a competency evaluation by an RN every 14 days to evaluate services of aide.

Completes a variety of clerical and stocking needs to assist with optimal department/agency safety and functioning.

Appropriately documents and communicates all required information in accordance with agency policies and procedures.

Organizes and performs work effectively and efficiently.

Maintains and adjusts schedule to enhance team performance.

Demonstrates positive interpersonal relations in dealing with all members of the team.

Maintains and promotes customer satisfaction.

Effectively demonstrates the mission, vision, and values of Lower Cape Fear LifeCare daily.

Qualifications

Minimum Qualifications:

High school diploma or equivalent and completion of Hospice Aide course.

Must attend 12 hours of in-service training supervised by a Registered Nurse on a rolling 12-month basis.

Competency of the hospice aide is assessed in the home every 14 days. The hospice aide does not need to be present unless there is an area of concern. Otherwise, an onsite evaluation occurs once a year.

Has not had a lapse of 24 consecutive months where the aide has not provided services for compensation.

Current certification with State Board of Nursing, or current registration with Department of Facilities Service in the State of North Carolina/South Carolina.

Minimum of two years’ experience and at least one year in home care or by other individuals under general supervision of a Registered Nurse.

Preferred Qualifications:

Previous CNA experience in hospice, home health, or oncology preferred.

Location-Service Area: Hospice Care Centers in Bolivia, NC (Brunswick County)

Educational Requirements


High School diploma or G.E.D.
Qualified as a Hospice Aide in the State per State and Federal Regulations.


Position Requirements



Must have one of the following: Minimum of one year experience as a Home Health or Hospice Aide i Is currently enrolled as a nursing student who has had a minimum of 6 months clinical experience Has completed a Home Health or Hospice Aide training program or a certified/state tested nursing assisted Completion of PCA course (Florida Only) Provides Personal care to the patient as assigned by the nurse Provides personal care to the patient as assigned by the nurse Assists the patient with transfers and/or ambulation Turns and positions patients who are bed bound Assists with range of motion appropriate for the patient Assists patients with meals Assists Patients with toileting Perform light housekeeping tasis Observes and reports changes in patient conditions and needs to the Registered Nurse/therapist

• High School diploma or equivalent education / experience - required
• Certified Home Health Aide - CHHA - required
• Automobile Insurance - required
• Driver License, Valid and in State - DL Number - required
• BLS - required

Experience

• Minimum one year experience performing duties as an aide in a home care setting or other health care related facility required.
• Direct patient care experience within the past five (5) years required.

Skills and Knowledge

• Working knowledge of medical terminology and health care practices is required as well as understanding of universal precautions.
• Demonstrate a positive customer service orientation; skill working with diverse groups of patients and co-workers; ability to solve customer-service related problems for internal and external customers.
• Ability to recognize the special needs and concerns of diverse groups of patients and others in the home.
• Ability to communicate with patients and significant others in an effective, mature, and caring manner.
• Tolerance for body secretions, strong odors, potential exposure to disease, and varied environmental conditions.
• Competency is evaluated by written test and during joint visits in the care of patients.

License/Certifications:

BLS-Basic Life Support Healthcare Provider - American Heart Association, DL-California Drivers Class C - State Department of Motor Vehicles, HHA-Home Health Aide - California Department of Public Health
