Works under the general supervision of the Admission Team Manager. Essential Duties and Responsibilities: Receives intake and informational calls. Directs department calls to the appropriate party; assists with phone calls to non-admitted patients to facilitate admissions. Assists in the day to day coordination of team clinical functions, facilitates, and schedules assessments. Obtains complete demographic and insurance information from appropriate source. Obtains medical certification and less than six-month prognosis for Hospice patients from the attending physician prior to assessment visit. Obtains and retrieves medical information pertinent to referral, prior to nursing assessment when possible. Notifies Team Manager or Director of scheduling concerns, documentation/ insurance requirements. Ensures nursing staff has current and accurate patient information attached to the patient chart. Understands and applies local coverage Determination Policies in order to assist in the identification of the best system of care for the prospect and assure eligibility for Hospice. Understands and applies Medicare, Medicaid, and third party requirements for eligibility and informs referral sources, as necessary. Participates in verification of insurance benefits. Follows up on third party insurance authorization when necessary. Acts as resource person for insurance information to all staff. Participates in special projects, statistical reporting, and analysis, as necessary. Demonstrates bias for action, acts on what is important, is proactive and creative, possesses a can-do attitude, ability to multi-task, has the ability to work under pressure and keep difficult situations in perspective. Performs necessary clerical tasks to include filing, mailing, record keeping, copying, and other tasks assigned to assure professionally organized medical and statistical records and to facilitate referral processing. Ambulette staff with EMT background will assist DME or Facilities team staff during ambulette downtime. Essential Duties and Responsibilities: Adheres to all HPCB policies and procedures as it relates to the ambulette. Maintains proficiency in the use of ambulette equipment. Demonstrates effective communication skills when interacting with staff in all departments. Receives and coordinates patient transport requests, responding to requests in accordance with established guidelines. Provides timely updates to the Charge RN on duty to confirm patient arrival at planned destination as well as communication on any issues encountered during transport. Documents observations on the ambulette transport form, immediately reporting to the Charge RN on duty any patient concerns noted during transport or upon arrival to pick up location. A Hospice Home Care Registered Nurse renders professional nursing services, in accordance with physicians' orders, for individualized patient care and psychosocial/ spiritual interventions for assigned patients/families. Provides nursing care in accordance with Hospice objectives, as well as, policies and procedures of the organization and the State Nurse Practice Act. Essential Duties and Responsibilities for a Hospice Home Care Registered Nurse: Demonstrates ability to perform comprehensive assessments to identify all physical, psychosocial, environmental, safety, emotional and spiritual needs related to the terminal illness that must be addressed in order to promote the patient's well-being, comfort, and dignity throughout the dying process and documents findings on all appropriate forms and on the plan of care. Performs pain assessments using Edmonton Scale, adhering to pain protocols, knowledgeable in equi-analgesic dosing and assures optimal pain relief using an interdisciplinary focus and timely appropriate interventions. Organizes and prioritizes assignments to consistently meet productivity standards and complete documentation requirements within identified time parameters. Appropriately explains duties and functions of the interdisciplinary team; articulates disease progression of cancer(s), lung disease, heart disease, liver disease, dementia, and other neurologic disease states. Able to articulate etiologies of symptom manifestation (nausea, vomiting, anorexia, fluid and electrolyte imbalance, diarrhea, constipation, mucositis, weakness/fatigue, dysphasia, edema, dyspnea, cognitive impairment, urinary retention, depression, delirium) related to a disease process. Modifies the plan of care timely and appropriately with a demonstrated skill in anticipating problems that may cause or create a threat to patient safety or lack of symptom control. Demonstrates knowledge in usage, indication, adverse side effects, contraindications and negative interactions of most commonly used steroids, NSAIDs, benzodiazepines, antidepressants, anticonvulsants, narcotics, antifungals, and diuretics. Documents to agency standards, timely completion, timely signatures, and documents care using A-I-R protocol to support hospice stay in program and level of care. Elicited no founded patient/family complaints or unusual occurrences (i.e. reportable incidents). Demonstrates proper sharps disposal, hand hygiene, waste disposal, use of personal protective equipment. Adheres to requirements of both CHPC and Hospice Buffalo policies and procedures that are both patient focused (clinical) and organizational focused (administrative); Adheres to HIPPA regulations and policies. Maintains the security of patient electronic/written records. Incorporates computer documentation into patient visit and/or as much as appropriately possible without compromising the quality of care and/or communication. Assures the timely completion and return of all initial, renewal and modified orders and reviews all PPOC's for procedural compliance. Expected to perform additional duties as requested. Qualifications for a Hospice Home Care Registered Nurse: Education/Certification Graduate of an accredited school of nursing with current RN licensure in the State of New York.
