The role The Registered Nurse provides intermittent skilled nursing services and directs, supervises and instructs hospice aide staff in the provision of personal patient care. As a Registered Nurse you will: Key Responsibilities: Assess and evaluate patient needs/problems, identify mutually agreed upon goals for care plan with patients Report patient status, updates and discharge planning process (if applicable) to clinical leadership, attending physician and hospice physician Ability to perform an annual on-site visit to the location where a patient is receiving care in order to observe and assess each aide Attend staff meetings and educational in-services as needed Be responsible to ensure the use of the 4Ms (What Matters to the patient, Medications, Mentation, and Mobility) and provide Age-Friendly Care Skills and Experience Required: A current unencumbered State Professional Nurse License Must maintain a valid driver's license and good driving record Ability to work in a field setting and exhibited ability to make sound nursing judgment Ability to assess patient needs and formulate individualized patient care plans to meet those needs Experience with Microsoft Office

Schedule is 9:00a-5:30p WHAT YOU WILL DO: Partners with Director of Care Solutions Center and Clinical team Leader assisting RN's, LPN's, Social Workers and Admissions Assistants in hospitals, homes and alt home settings completing admissions and visits. They are responsible for visiting patients and their families, representing Hospice of the Western Reserve with a goal of performing assessments, provide education and training about medications, care for physical needs. Identifying and resolving problems, preparing, and completing action plans, managing all clinical lines for improvement and quality assurance programs. They assist to carry out Care Solutions Center operational strategies by conducting needs assessments, performance reviews, capacity planning, and cost-benefit analysis: defining user requirements, establishing productivity, quality, customer service standards, contributing information and analysis to organizational strategic plans and reviews. Builds relationships with staff in hospitals they serve, being a trusted partner. Trained and qualified nurse who can help guide teams to provide support for families and patients with empathy and confidence They implement the Care Solutions systems by assisting the Director, and Chief Clinical Officer with patients, families, referral providers, doctors, and Hospice of the Western Reserve field staff. Collaborate with trainers to implement a comprehensive training program for staff in the field. Determines training needs by observing agent and service encounters, studying all phases of the Care Solutions services, technical, service and customer experience results reports, conferring with management. Updates job knowledge by participating in educational opportunities, reading professional publications, maintaining professional/personal networks Educational awareness with EMR (Suncoast) on a regular basis. Assist in the interview, hire, train, support, and develop liaison staff and admission staff. Supports and reinforces all strategies, plans, and implementations as directed by Senior leadership. Proactively identify operational efficiency and customer service enhancement opportunities Work directly with all corresponding programs to support, meet and or exceed their individual referral/admission goals. Reinforce work procedures, prepare work schedules, and expedite workflow. Evaluate and standardize procedures to improve efficiency of subordinates. Observe and evaluate employees who perform the duties of assessment, liaison. Continuous care and triage to ensure quality standards and service are met. Attend meetings, training activities, courses and all other work-related activities as required. MANAGEMENT Responsible for managing and supervising field staff who see patients outside the scope of the primary care team. SUCCESS CAPABILITIES: High School Diploma or equivalent required Bachelor of Science in Nursing required; Master of Science in Nursing preferred Minimum 2 - 4 years' experience supervising and leading teams Hospital based nursing experience preferred Hospice and palliative care experience preferred Maintain a valid drivers' license and auto insurance for successfully fulfilling business and operational needs. Must understand critical success factors of the Care Solutions Center Excellent customer service skills and relationship building capabilities with referral sources and families Exceptional written and verbal communications skills, with proven experience interacting with a range of personalities and styles, to establish effective relationships at all levels of the organization. Ability to relate to others and exhibit compassion to all parties involved. Demonstrated ability to practice active listening skills. Detail oriented with strong organizational, time management, and effective prioritization skills. Ability to quickly learn systems, processes, and procedures, grasp clinical concepts and adapt easily to change. Ability to manage competing demands, multiple priorities, while remaining adaptable and flexible. Able and willing to maintain professional and technical knowledge by tracking emerging trends in contact center operations management, reviewing professional publications, establishing personal and professional networks, benchmarking state of the art practices. Monitor and evaluate pain and symptoms and provide information and assistance to the patient and family to manage those problems. Serve as a liaison to the patient's physician(s) when needed; provide information to the physician based on knowledge relating to the patient's needs. Provide emotional support to the patient and family/caregiver. Refer patients and/or family members to other services when appropriate and follow-up as necessary. The Clinical Admissions Nurse is responsible for assuring for assuring that HWR programs are effectively and responsibly represented to patients, families, and others who have interest in hospice services through assessment, information sharing and/or admissions in a patient's home, nursing facility, or in a health care facility. This position works closely with the Referral Department and other referral sources to promote the hospice philosophy of care. WHAT YOU WILL DO: Performs all duties and responsibilities in accordance with the Nurse Practice Act and in accordance with basic principles and guidelines of professional nursing. Provides information assessment, and admission services to patients who have been referred to HWR: conducting assessment visits in the home nursing facility or health care facility, as necessary. Communicates with Referral Department, the referring Physician, and Hospice MDs to assure appropriateness of each referral and those necessary certifications are obtained. Keeps the Referral Department informed about the progress of the referral/admission. Applies nursing principles and techniques in the treatment of patients. Provides nursing care, emotional support and education to patients and families. Utilizes computer skills to access medical information and documents in electronic medical records as required. Promotes teamwork by participating actively and appropriately as a member of an interdisciplinary team, shares essential patient information with all members to coordinate care. Completes and submits documentation within agency timelines policies that is complete, legible, and clearly reflects care given. Utilizes appropriate resources to maintain knowledge and skills for the hospice care in all settings including regulatory/ reimbursement guidelines. Collaborates with other members of the IDT in coordinating care to be efficient and financially responsible. Makes appropriate referrals to other discipline services and medical equipment/ supply companies to meet patient care needs in a timely manner. Works with diverse social and economic situations without imposing own values on patients or family, maintaining appropriate professional boundaries. Meets standards for continuing education and maintain licensure. Demonstrates understanding of HIPPA privacy Standards and related Agency policies. SUCCESS CAPABILITIES: Graduate of an accredited nursing program: BSN and/ or CHPN Experience with volunteer management and HR practices preferred Required Nurse Licensure by the state of Ohio, valid driver's license with an automobile that is insured in accordance with the state and/or agency requirements and is in good working order. Minimum of one year of nursing experience required: intake, home care, hospice and/or oncology nursing experience required. Graduate of an accredited school of nursing, with a Bachelors degree preferred. Licensed RN/LPN in state of Ohio. Experience preferred in pain and symptom management, oncology, or mental health. Ability to communicate effectively, orally and in writing Ability to work independently and collaboratively with a team Must have demonstrated knowledge and skills to provide appropriate care to all age groups. Ability to handle stressful and constantly changing conditions; must be able to make good professional decisions quickly despite stressful atmosphere. Ability to access patients and families in any residential and/or public setting in any weather condition (i.e., climbing stairs or snow filled walkways, etc.). PHYSICAL REQUIREMENTS Ability to stand and walk for extensive periods of time. Manual and finger dexterity and eye-hand coordination to provide nursing care and operating equipment. This is heavy work requiring the exertion of up to 75 lbs. of force occasionally as well as 50 lbs. frequently utilizing appropriate safe transfer techniques and body mechanics when working with patients. Ability to stoop, squat, or bend frequently. Corrected vision and hearing within normal range to observe and communicate with patients and professional staff. Possibility of exposure to environmental elements, internal and external, such as disease, bloodborne pathogens, and chemicals and/or chemical fumes, (pet dander and/or exposure to allergens) and adverse effects of inclement weather that may occur as well as potentially difficult patient and family situations.
