Intake and Admissions Representative
Full -Time

ESSENTIAL FUNCTIONS:
Delivers great customer service when communicating with patients, families, referral sources and hospice team
members.
Receives and responds to hospice referrals and inquiries via phone, portal, email, and/or fax at a fast pace.
Creates the initial referral record in the electronic health record and starts the intake process through a virtual intake
platform with a high level of accuracy.
Schedules admission visits with patient families with goal of 4-hour response.
Retrieves and uploads pertinent medical records from hospital electronic health record platforms.
Monitors open/pending referrals and conducts and/or assigning necessary and appropriate follow up with a high level
of urgency.
Verifies patient benefit information and obtains and/or assigns initial prior authorizations to Finance/Patient Accounts.
Collaborates with supervisors/leadership regarding high risk and complex referrals.
Ability to work independently as well as part of a team.
QUALIFICATIONS:
High school diploma or GED required.
A minimum of three years' experience in customer service, secretarial or healthcare required.
Excellent communication skills (verbal and written)
Ability to multitask and prioritize in a fast-paced environment with a high level of accuracy.
Ability to work independently; self-motivated
Proficient at basic phone and computer skills
Must be comfortable answering phones with a heavy call volume.

Job Summary
This role will be responsible for ensuring accurate and timely processing of invoices and payments. The role will also involve coordinating with vendors, reconciling accounts, expense tracking, and other AP tasks as necessary to ensure successful management of our financial operations.
Essential Functions
Invoice & Purchase Order Processing: Review, verify, code to GL account, and process invoices and purchase orders in a timely and accurate manner.
Credit Card & Expense Report Processing: Manage accounts payable credit card purchases & expense reports
Payment Processing: Prepare and process payments to vendors via checks or ACH. Ensure all payments are made according to agreed terms.
Liaison: Communicate with vendors and internal departments to resolve payment issues, discrepancies, or questions, acting as a liaison between accounting and other departments.
Account Reconciliation: Reconcile accounts payable ledger & general ledger accounts to ensure accuracy and completeness of financial records.
Expense Management & Analysis: Review company expenses, ensuring that all expenses are properly documented and coded to appropriate accounts.
Data Entry: Accurately input invoices, credit memos, and payment data, including automated data imports, into accounting software. Enter expenses covered by grants in tracking database.
Compliance: Ensure compliance with company policies, procedures, and tax regulations in the accounts payable process.
Month-End Close: Assist in month-end and year-end financial close by providing accurate AP reports and reconciliations.
Record Keeping: Maintain organized, up-to-date records of accounts payable documents for audit purposes.
Reports: Prepare reports related to accounts payable, such as outstanding payables, aging reports, and payment histories for management.
1099 Filing: reconcile 1099 vendors and file forms 1099 annually
Ad Hoc: Identify and build process efficiencies, and management of additional tasks as necessary
Qualifications
Associate's or Bachelor's degree in Accounting, Finance, or related field preferred
2+ years of experience in accounts payable or related financial roles
Proficient in accounting software (Sage Intacct experience is a plus).
Strong understanding of accounting principles and financial processes.
Intermediate knowledge of Excel and other MS Office applications.
Excellent attention to detail and organizational skills.
Ability to work in a fast-paced environment and handle multiple tasks.

Nurse Practitioner- Pediatric
Full Time, 32 hrs. per week, Day Shift

Day off each week will be Tuesday, Wednesday or Thursday. MUST work Mondays and Fridays.

This role will see patients anywhere in our service territory
Orientation - Must be able to do a full time (40hr/week) orientation for 1-2 months
Functions:
Serves as a member of the Sunflower Pediatric Hospice and Palliative Medicine Interdisciplinary Team (IDT).
Provides critical thinking and clinical judgment to address the physical, emotional, psychological, and spiritual needs and reflect the goals of care of the patients and families receiving hospice and palliative medicine services.
Provides clinical direction and expertise to the Sunflower IDT.
Participates in goals of care discussions and development of plans of care that are clinically appropriate and that meet accepted palliative care and Choices Healthcare standards and guidelines.
Collaborates with Choices Healthcare physicians and/or Pediatric Medical Director and the Sunflower IDT in admission, certification, recertification, and discharge decisions.
Collaborates and maintains communication with the patients' healthcare team (e.g. primary care provider, subspecialty teams, and hospice and palliative medicine physicians) as needed for the coordination of hospice care and palliative medicine services.
Monitors plans of care to assure consistency with Choices Healthcare philosophy, policies, procedures, and protocols.
Provides clinical evaluations for hospice and palliative medicine patients when necessary and requested as part of the plan of care, and documents visits and services according to Medicare and Choices Healthcare standards.
Provides support and education to patients, caregivers and facility staff and acts as a liaison to facilitate goals of care.
Participates in interdisciplinary team meetings as scheduled.
Participates in learner education (e.g. medical student, residents, and fellows) as needed.
May be required to participate in some adult hospice responsibilities as directed by supervisor PRN.
Qualifications:
Certified Nurse Practitioner license in the Commonwealth of Pennsylvania as Pediatric Nurse Practitioner (PNP) or Family Nurse Practitioner (FNP).
Certification in Hospice & Palliative Medicine required within three years from date of hire.
1 year of pediatric experienced required

JOB SUMMARY

FUNCTIONS
Provide consultations and subsequent visits for Choices Healthcare and Palliative Medicine patients.
Document visits and services according to agency standards for communication and billing.
Help assure agency compliance with state and federal regulations.
Consult with attending physicians and, when involved interdisciplinary teams, establish plans of care.
Provide critical thinking and clinical judgment to ensure care plans are medically appropriate, individualized to meet physical, emotional, psychological, and spiritual needs and reflect the goals of care of patients and families receiving hospice and palliative care services.
Prescribe medications according to state and federal standards, following organizational policies for prescribing of controlled substances.
Ensure plans meet Choices Healthcare and Palliative Medicine standards and guidelines and are consistent with accepted hospice and palliative care standards.
Participate in the provision of around-the-clock hospice and palliative medicine services, including on-call responsibilities and weekend/holiday coverage.
Represent Choices Healthcare in interactions with physicians, hospital staff, nursing home staff, and others in the community.
Instruct medical students, residents, fellows, and other health professionals seeking hospice and palliative care education.
Provide medical direction regarding Choices Healthcare financial responsibility for medications, treatments, and procedures.
QUALIFICATIONS:
Licensed to practice medicine in Pennsylvania.
Must be Board eligible and/or maintain certification in their primary specialty. Must also be Board eligible and/or maintain certification in Hospice and Palliative Medicine or as a Hospice Medical Director (by the Hospice Medical Director Certification Board). Has demonstrated interest, knowledge, and skills in palliative care.
Team oriented with ability to provide personalized patient care as well as family support and demonstrate clinical outcomes. Must be able to assess, evaluate, plan, and implement care in complex and life limiting situations.
Able to effectively communicate verbally and in writing, and to motivate, lead, and provide specialized medical direction, particularly in a team setting and also in physician-to-physician relationships.
Must have working knowledge of computer based programs and have the ability to learn and navigate the computer system and platforms.
Current driver's license in the state of Pennsylvania. Able to make home visits to patients.
Member in good standing on the medical staff(s) of accredited hospitals in the service area.

JOB SUMMARY:
The social worker facilitates effective referral coordination and information exchange with the Interdisciplinary Team (IDT), prospective and current hospital-based general inpatient (GIP) patients, their caregivers, and hospital and community teams.
Provides explanations about the hospice benefit and organization services in a manner consistent with the patient and family level of understanding.
Records information necessary to assist the hospice physician in determining eligibility, plans and coordinates discharge needs for the prospective patient, and collaborates with community senior living community staff and hospice teams to facilitate a safe and effective discharge plan.
Utilizes SW interventions, education, counseling, and support services to address the identified psychosocial needs of patients and caregivers.
ESSENTIAL FUNCTIONS:
Collaborates with hospital providers and staff IDT to develop a plan of care, complete initial and ongoing comprehensive assessments, and plan/evaluate SW interventions.
Conducts admission assessments/information visits and follow-ups on hospitalized patients
Conducts high-risk admission assessments/follow-ups in hospitals where psychosocial issues are present, including but not limited to patients living alone, inadequate caregiving/decision-maker situations, and significant mental health/substance abuse concerns.
Documents in accurately and concisely at the time the service is provided.
Initiates, coordinates, and revises the plan of care on an ongoing basis with input from the IDT, patient, caregiver, hospital providers, and staff.
Participates in hospital discharge planning, when needed, collaborating with hospital case management on the plan.
Participates in IDT meetings to report on the plan and update the comprehensive assessment.
Provides education and support to patients and caregivers regarding diagnosis, care, treatments, medications, and end-of-life expectations in a method or manner that they understand.
Provides education and support to hospital staff.
Supports special needs of patients and families including the pinning of Veteran patients through the We Honor Veterans program.
Communicates effectively with organizational staff, physicians, patients, families, hospital staff, outside contractors, and community agencies.
Anticipates, assesses, and responds to pain and other symptoms of physical, emotional, and spiritual suffering.
Works in collaboration with IDT members, managing the psychosocial care of assigned caseload.
Identifies patient and caregiver stressors and strengths to direct interventions throughout the patient's illness, including initiating post-death bereavement support,
Demonstrates proficiency in navigating systems, knowledge of community resources, advocacy, individual/group counseling, and rapport building,
Understands reimbursement and payer source implications to the plan of care and utilizes resources wisely and effectively.
Assists with the orientation of new staff and visiting professionals.
QUALIFICATIONS:
MSW required
LSW or LCSW is also required or must be obtained within 2 years of hire date.
Two years' experience in the health care setting preferred.

FUNCTIONS:
Triages calls assessing pain and symptoms, physical needs and disease progression
Provides direction to the caller or coordinates necessary follow-up, prioritizing for those with uncontrolled

Provides anticipatory guidance and education for disease progression and end of life care. Utilizes patient teaching tools
and handbooks as reference.
Assists with telephonic assessment and coordination of inpatient center transfers
Obtains refills, renewals, new and revised orders for medication and treatments
Supports the initiation of symptom management orders through timely notification to contracted pharmacy
Updates the clinical record with new orders, medications and treatments, and communicates to team. Anticipates
medication, DME and supply needs and bundles orders.
Serves as a resource and mentor to hospice clinical staff
Qualifications
Registered Nurse licensed in Pennsylvania
Associates in nursing required
Minimum of two years medi cal/surgical, home health, or relevant experience required
Must be able to complete 5- 6 weeks of a FT (40 hr./wk.) orientation.

Essential Functions
Assists with the provision of palliative medicine in accordance with organizational, state, and federal regulations. Serves as a comprehensive direct care provider with demonstrated skilled proficiency in symptom management and advanced illness care.
Provides critical thinking and clinical judgment to address the physical, emotional, psychological, and spiritual needs and reflect the goals of care of the patients and families receiving palliative medicine services.
Provides clinical direction and expertise to the interdisciplinary team. Participates in goals of care discussions and development of plans of care that are clinically appropriate and that meet accepted Choices' standards and guidelines.
Provides comprehensive clinical and psychosocial evaluations of patients and their families in relation to their goals of care and serves as a patient advocate in the development of patient centered goals of care.
Collaborates and maintains communication with the patients' attending providers and sub-specialists for the coordination of palliative medicine services.
Prescribes appropriate pharmacologic and non-pharmacologic treatment modalities and monitors the effectiveness of interventions.
Provides cross coverage to hospice services on an as needed basis including in person visits.
Qualifications
Certified Nurse Practitioner license in the Commonwealth of Pennsylvania required
Hospice and/or Palliative Medicine experience preferred
Prescriptive authority required
Unrestricted DEA license required
Certification in Hospice & Palliative Medicine required within three years from date of hire

Essential Functions
Provides accurate and timely information regarding hospice services to anyone inquiring about Hospice care.
Completes admission visits for routine, general inpatient (GIP) and Hospice Response Team (HRT) patients.
Performs RN comprehensive assessments and focused skilled nursing visits on GIP/HRT per discipline frequencies.
Follows up on hospice waiting patients to assure a smooth hospital discharge.
Documents in both the hospital EHR and hospice EMR in an accurate and concise manner at the time the service is provided. Documentation supports the limited prognosis, the level of care, changes to the plan of care, disease progression and care needs.
Obtains accurate patient related data including initial needs assessment, medical history, insurance information and resources available to patient and caregiver.
Immediately communicates assessment information and proposed plan and level of care to the attending physician, hospital care/case manager, and access team, requesting additional information, orders, or follow up as needed.
Collaborates with community residential facility staff (including senior living and other congregate living settings) to develop an effective and safe discharge plan.
Provides ongoing communication and updates with organizational staff, hospice physicians, patients, families, hospital staff including but not limited to care/case management staff and physicians, community palliative care providers, community partners such as skilled or assisted living facility staff and admissions planners.
Assists with hospital transition/discharge planning process, working with case/care management departments, nursing staff and caregivers to facilitate admission to hospice services.
As appropriate, completes hospice enrollment including informed consent.
Assists with outreach to referral sources to coordinate care and facilitate referral process.
Anticipates, assesses, and responds to pain and other symptoms of physical, emotional, and spiritual suffering.
Demonstrates thorough knowledge and application of current palliative care protocols for symptom management.
Provides direct nursing care and interventions as prescribed by the physician and/or symptom management orders.
Works independently and effectively throughout all care settings as required.

Qualifications
Graduate of an accredited school of registered nursing.

JOB SUMMARY
Serve as a member of Choices Healthcare Medical Services and Palliative Medicine Team. Provide clinical and organizational leadership across settings in hospice and palliative care. Provide hospice and palliative care consultations and visits in home, clinic, inpatient, and hospital settings. Participate in the development and implementation of medical policies, procedures, and protocols. Comply with state and national regulations. Instruct medical students, residents, fellows, and other health professionals. Represent Choices Healthcare in interactions with physicians, hospital staff, nursing home staff, and others in the community.
FUNCTIONS
Provide consultations and subsequent visits for Choices Healthcare and Palliative Medicine patients.
Document visits and services according to agency standards for communication and billing.
Help assure agency compliance with state and federal regulations.
Consult with attending physicians and, when involved interdisciplinary teams, establish plans of care.
Provide critical thinking and clinical judgment to ensure care plans are medically appropriate, individualized to meet physical, emotional, psychological, and spiritual needs and reflect the goals of care of patients and families receiving hospice and palliative care services.
Prescribe medications according to state and federal standards, following organizational policies for prescribing of controlled substances.
Ensure plans meet Choices Healthcare and Palliative Medicine standards and guidelines and are consistent with accepted hospice and palliative care standards.
Participate in the provision of around-the-clock hospice and palliative medicine services, including on-call responsibilities and weekend/holiday coverage.
Represent Choices Healthcare in interactions with physicians, hospital staff, nursing home staff, and others in the community.
Instruct medical students, residents, fellows, and other health professionals seeking hospice and palliative care education.
Provide medical direction regarding Choices Healthcare financial responsibility for medications, treatments, and procedures.
QUALIFICATIONS:
Licensed to practice medicine in Pennsylvania.
Must be Board eligible and/or maintain certification in their primary specialty. Must also be Board eligible and/or maintain certification in Hospice and Palliative Medicine or as a Hospice Medical Director (by the Hospice Medical Director Certification Board). Has demonstrated interest, knowledge, and skills in palliative care.
Team oriented with ability to provide personalized patient care as well as family support and demonstrate clinical outcomes. Must be able to assess, evaluate, plan, and implement care in complex and life limiting situations.
Able to effectively communicate verbally and in writing, and to motivate, lead, and provide specialized medical direction, particularly in a team setting and also in physician-to-physician relationships.
Must have working knowledge of computer based programs and have the ability to learn and navigate the computer system and platforms.
Current driver's license in the state of Pennsylvania. Able to make home visits to patients as it relates to Community Based Palliative Care.
Member in good standing on the medical staff(s) of accredited hospitals in the Hospice & Community Care service area.

Seeking an experienced RN to work on our Admissions team. Provides nursing services, responds to inquiries and referrals, and addresses urgent and ongoing needs for patients and caregivers; in addition, the RN supports and enhances the organizational mission and philosophy, promoting excellence in end of life care. This position covers Lancaster, York and surrounding counties. Orientation: 2 weeks full time, 8am - 5pm + additional orientation over the next 16 weeks that can more flexible.
